It’s a film, technically everything is planned, so you need to make the audience feel as if it’s random chaos. Surgeon's death and the Bolivia job wasnt part of an assassination attempt on White Death, just coincidence. White Death just didnt care, took his revenge either way.
I think the logic is, The White Death doesn't care if you were directly trying to kill his wife like Carver, or indirectly linked to her death; like Lemon, Tangerine, his Son and The Hornet. If you COULD be linked to her death in any way, he is going to kill you.
I think the doctor dying was a part of the failed attempt on the White Death’s life. Like, the plan was for Carver to hit the car to make it look like an accident and then if he didn’t die in the car crash, he’d die as a result of not undergoing surgery because the Hornet took out the only doctor that could help him. But as the White Death explained, he was brought elsewhere by fate and his wife instead took his place.
I have a theory what if carver was not sick at all he knew the white death would invite him on the train but he did not show up pretending that he was calling in sick for someone else to take his place

Circumstance just put him against Ladybug who was filling in for the person that is responsible for bringing them all their on the bullet train in the first place.
I love how the entire plot of the film hinges on the fact that somebody called in sick to work 🤣
