  • In this first video, Cathy previews the content of this video series and talks about best practices when doing head-to-toe skin assessments for newly admitted patients.

    Thank you for giving a such clear introduction on the importance of the first skin check assessment!!
    Many nurses are not taking it seriously enough. I still remember on my last placement, a nurse did a very poor skin assessment and documentation for a patient who admitted to the ward with UTI. The patient was significantly underweight, bed-bound and confused.
    I did not believe in what the chart said and I did a skin assessment with another student, yeah bingo, we found second degree pressure injuries on both pt foot. Unfortunately we found it after the first 24 hours, the team leader said our ward officially caused the pressure injuries.

    Please add more wound care videos! I was hoping to find help learning how to stage pressure injuries and with describing wounds and with dressings! I see this video was made in 2019, so maybe the series was put to the side.

    Thank you Cathy for all your videos! I got my LPN in May of this year and you were a big part of it. Cathy, when you started wound care, did you have any issue at first with feeling nauseous when you saw wounds? I had an orientation night with a wound nurse and I felt really nauseous. The funny thing is thou that I’m really interested in wound care but I had to step out for a few at one point. Do you have any suggestions ?

      You are so welcome! So happy I could be part of your journey to becoming an LPN. I have not had any issues with nausea with wound care, but I have definitely known some nursing students (and nurses) who struggled initially, but then overcame that feeling over a short time. It is your body's visceral reaction to seeing the wound, and it is very common. If you spend some more time doing wound care, your mind/body will get used to it, and you probably won't have that issue after a little while. If you are interested in wound care, I would definitely try to get more exposure to wounds and look into classes/education on wound care. 👍

    As a wound care nurse my biggest issue is the admitting nurse missing a pre existing wound upon admission and the center having to take blame and it becoming a HAI.

    What if the patient is being transferred from another floor and the wound is packed and or covered? do you uncover and look at it or what?

      If the previous nurse from the other unit already did the wound care as ordered before the patient arrived, you typically don’t have to take the dressing down and redo it. You can document that the wound is covered and the dressing is clean, dry, and intact. However definitely check with your facility’s policy!

      Yes, patients often have wounds on their posterior side, so turning and lifting is part of the job. Getting help and using assistive devices is important!

      You can get your WCC and do wound care at many SNFs and LTACs.

    If you have sutures from the hospital you need to get them taken out by a licenced professional not a friend who happens to be medical.

    According to the bibles genealogy the world is only 6000 years old which disputes fossil records and all science which prove the earth is 5.4 billion years old
    Jesus said nearly 2000 years ago:Truly I tell you, some who are standing here will not taste death before they see the Son of Man coming in his kingdom."
    And he said speaking of end times when the antichrist would come: Truly I tell you, this generation will certainly not pass away until all these things have happened.
    And speaking of slavery which the bible permits: When a man strikes his male or female slave with a rod so hard that the slave dies under his hand, he shall be punished. If, however, the slave survives for a day or two, he is not to be punished, since the slave is his own property.
    Also the tribe of benjamin was permitted to capture "a damsel or two for himself" and force them into marriage to unwillingly bear children by rape.
    Deuteronomy 13:12 you hear it said about one of the towns the LORD your God is giving you to live in that troublemakers have arisen among you and have led the people of their town astray, saying, "Let us go and worship other gods" (gods you have not known), then you must inquire, probe and investigate it thoroughly. And if it is true and it has been proved that this detestable thing has been done among you,you must certainly put to the sword all who live in that town. You must destroy it completely, both its people and its livestock.(including people who had nothing to do with it).
