      The guy is a load of BS.. I really wish him an issue for life so he will understand . Not 1 month.. not 1 year.. he is abusing the theory about central sentization so they can lay the responsibility with the patients.. I think this approach is new to the USA.. but in Europe and Australia already a long time.. and low and behold.. still all in pain with severe spine issues or nerve damages. He doesn't really differentiate.. and thinks he has invented the wheel again.. so.. I wish I could tell you a different story.. but there is none here. Managing with certain techniques and exercises ( not the one size fits all) but specific for your triggers might be helpful though. Lifestyle changes must happen. Stress must be minimal since it triggers that pain into the whole body.. I manage already 15+ years to live with it.. it takes a lot of self-care and boundaries. I train every other day very specific exercises for my issues. I focus on things and keep my brain busy.. when I feel my nervous system gets triggered I take rest and let it pass... not fight it or medicate it.. But with all that discipline and dedication.. my complete spine is damaged and I will never become pain-free before death. I get so annoyed by this guy putting all on the same central sensitization thing. Ofcourse there is CS with ongoing issues. It will worsen your situation a lot. So you need to manage that fysical stress on the body. But the pain is still there when your spine, your knee, your nerves has certain issues..

    This is a 2 part question. If you continue to move the part of your body that was damaged and the pain is not created by the tissue can you cause more damage even after it heals? Secondly if the pain isn’t caused by the tissue why are we still doing PT? This is a very important topic for me.

      Really interesting questions.
      1) This video breaks down "safe pain" ruclips.net/video/hPEY-1Hssu8/видео.html
      2) Let's be clear here: Pain is usually disproportionate to tissue damage. Let's call it "tissue irritation" instead. So we can still address the tissue irritation, but hurt doesn't always equal harm. Additionally, with PT we do NOT need to get lazer-focused on one specific tissue like a specific muscle or joint. Instead, we see that ALL the joints and muscles in the region are going to be impacted by chronic pain. This means we need to focus on a zoomed-out full body strength approach instead. The body still needs to get strong, but we don't need to get obsessed with some magic muscle.
      3) Watch the masterclass, it breaks this whole thing down. www.shapeshiftwellness.com/LBP-masterclass

    I had figured there was something to my neck pain that is related to the whole injury that I had. I was basically going crazy with lateral delt raises, I was doing 40 reps or more to failure. Few days later I"m under my car tugging on something really hard while laying down. I end up straining the entire chain from the arms, traps, neck, and upper back. Noticing more cracking going on in my neck I stopped moving it and slept upright for a few weeks. This caused the neck muscles to get weak. 2yrs later I'm still dealing with chronic pain. Static strengthening exercises of the neck with a band has helped a lot. I'm going to keep going with that. I was assessed by a PT right after the injury to make sure nothing major happened and it was all clear. No nerve issues or shooting pain.
    I track my HRV with a heart rate monitor, when I wake up in pain it knows right away that I'm stressed and says "your sympathetic nervous system is more active today". My resting heart rate will also be higher. What I'm trying to say is that the body's response is measurable. I had a feeling the stress and cortisol was contributing to my weight gain. I'm taking "ashwagandha" powder which is said to help block cortisol. Along with Turmuric/Curcumin to help with chronic inflammation. Sometimes the body needs a little chemical help

      @ShapeshiftWellness  2 месяца назад

      Yep the stress response is a tangible, measurable, neurochemical thing. I hate it when people think stress is just in their head. It's literally in the entire body, every system, every tissue.

    Ok, I'm picking up what you're layin' down. In the last segment of this presentation you said " ...we have a message (of potential pain) to deliver to the brain..." , every last person reading this _Knows,_ when they stub their toe, the physical message of the toe stub comes _BEFORE_ the pain message. Actually, its about 2 seconds. WTF is that about??
    We even brace for the pain message we _Know_ is about to arrive! About 6 years ago, I had the "excruciating pain" of sciatica (and some foot numbness), which was "likely" caused by Piriformis syndrome - which I deduced as when I did Piriformis screeches/exercises to floss the nerve, the pain subsided. For my _Individual_ situation right now, similar sciatica issues, but NO leg pain - worse: L4/L5 impingement numbness on left side of calf, and foot, to where I can't feel enough to maintain balance on my left leg, thus making it difficult to even walk straight.
    I took your advice, and remained as active as possible, all things considered. its going on 4-weeks, and I see some healing progress. ...SOME. Now, my concern at age 65, and a former quasi-elite athlete in my younger days, is that my body chemistry IS NOT HEALING like it was. My last few annual blood test sowed hypogonadism (very LOW T levels). T was 132, and Free-T was 3.3.
    IMO, it ain't about "pain", its about _Recovery._ As a young Man, those in the gym on some form of T-juice talked about the _Recovery_ benefits...allowing them to HEAL from wicked workouts. I have had "back pain" off and on throughout my life, and have always recovered. Now my back issues are chronic - AKA: NOT HEALING. I assume you can not prescribe TRT (or any other pharma), but when it comes to "old(er)" folks like me, and since you are an out-of-the-box thinker, it would be interesting to hear your perspective on what I just articulated.

      Sounds like quite the journey you've been on. Basically, we want to take a holistic approach and look at full body strength exercise, nutrition, mental health, and sleep/recovery. Usually if we zoom out, we can make progress where previously people have plateaued. If you haven't seen the masterclass, I break it all down in there: www.shapeshiftwellness.com/LBP-masterclass

    My back was good up until a few days ago. I can walk, jog and move around, no problem. After sitting for a few minutes, I get a sharp pain in my lower back trying to straighten my back while standing

      One of my favorite pieces of rehab to give clients are "movement snacks." Basically, frequent movement breaks: Do 10 seated cat/cows, some bodyweight squats, some standing back-bends. Whatever feels like it loosens things up.
