    The sciatic nerve runs from the back, down the back of the leg into the foot. Sciatica is a term used to describe symptoms of pain, numbness, or tingling that runs down the back of the leg (usually worse below the level of the knee) due to nerve irritation. There are many causes of sciatica, the most common being a herniated disc.
    One of the first things people are recommended to do (or feel like they want to do) are sciatica stretches. Sciatica stretches are also known as sciatic nerve glides or sciatic nerve flossing.
    The problem with these commonly recommended sciatic nerve exercises are that they can easily irritate the nerve if performed improperly (at the wrong time or wrong dose).
    In this video, I walk you through a mindmap and live case studies of real patients dealing with sciatica, in hopes that you can better understand sciatica and identify which types of exercises are safe or NOT safe to do.

    Great info, Dr. Charlie! Only one minor suggestion, as a medical massage therapist. Instead of using the words "bad side" and "good side" consider using "affected side" and "non-'affected side." Words are powerful and the body picks up on them. Wishing all the best to you and yours and I look forward to viewing more of your videos. :)

    Been dealing with piriformis syndrome for 14 months now. Have gone through physio, chiropractor, osteopath, massage, and nothing really helped. The pain actually gotten progressively worse, can’t sit for more than 5 minutes, and if I do it’s extremely painful to stand up, my right glute area locks up and can take anywhere from a couple minutes to a couple hours to release. So basically I spend the whole day standing up. Usually the worst pain of the day is trying to get out of bed in the morning. I stretch every single day, use a tens machine… I really need help, thank you

    Everything you said in the first 20 seconds about when, why, how, should you be doing it ....omg I’ve asked my doctor, numerous physiotherapists, osteo etc those questions and they just don’t cover it all! I asked an exercise physiologist to give me a plan to follow and finally got some understanding and exercises for hips, knees, shoulder.

    • @krpurple2678
      @krpurple2678 3 года назад

      It can be so complicated, I had severe sciatica a few years ago, since then I still have a bit of numbness in the thigh. I have had bursitis in the hip knee and shoulder all diagnosed via ultrasound, some arthritis...I have good range of movement but my glutes and thighs are so tight if I don’t stretch I walk stiff legged.
      Now I know I haven’t been doing the right exercises and was over stretching. I am light headed due to anxiety when I’m walking and I can’t take any painkillers except for paracetamol.
      The moral of the story, it does improve if you are guided to the correct treatment and keep as active as possible. Currently doing meditation and trying to retrain the brain (neuro plasticity) from being overly reactive to pain.

      @@drcharliejohnson I was diagnosed with degenerative disk disease in 2016. I experienced discomfort but not as bad as now. I'm afraid my ddd has further gotten worse. I regularly exercise and do stretches but I experience little relief now. I already arranged to have a physical therapy session next week. Hope it resolves my problem and I'll find comfort in sitting again. I'm halfway across the globe but I'm glad to avail of your expertise through YT. Thanks again.

    Thank you for this video, I had herniated L4&5 over 9 months ago, and it was my left side, three cortisone shots later and had finally felt It had healed
    Then the right leg I started have pain beginning in back of just the knee eventually from the knee to the ankle, can not sit, walk without pain, getting up,from seated position is very painful, can not climb stairs , i never have pain when laying down, no tingling …I thought it might be sciatic, but when I did this test, when I lift the leg that I thought was sciatic my opposite leg is where I feel the pain, and I do feel a lot of that pain in my lower back. So I am pretty sure I do not have sciatica now

    So I have L4 L5 S1. Strong Sciatic pain down right leg to foot when standing. When doing the leg test and curling my foot I feel pain just as bad when raising my left leg but pain transfers over to right butt and leg. Is this normal?

    • @drcharliejohnson
      @drcharliejohnson  Год назад +2

      Hey Tracy! Thanks for watching! So, yes this is not out of the norm. We call this a CROSSED straight leg raise test- and it's a sign most consistent with a disc issue. Basically, raising the good leg reproduces pain in the "bad" leg.
      Make sense?

    Hey i have sciatica caused by herniated disc, my right leg can just raise half or less than the left one.
    I try nerve flossing but i cant barely move the nerve cause of that so what should i do? Just keep it with this minimun rang of motion that i have till i can raise the leg more or what? Thanks

    • @drcharliejohnson
      @drcharliejohnson  2 года назад +2

      Hey! Not sure how things are currently- but I would avoid flossing the painful nerve.. try flossing other side- or treat elsewhere!! Have to let symptoms calm before trying to get things moving in the painful area again.
      Make sense?
      Dr. Charlie

      @@drcharliejohnson I agree with that from experience. I've been doing your core strength training and have marked improvement,although I pretty much avoid sitting now. I use a heat pad but around three am the cramp wakes me and I switch to ice to calm it down. Then back to heat. I do the strength training as much as the stretching because I was stretched enough,although I still do some. But in the morning if its bad I walk up and down the driveway to 'calm things down' because it would be too painful to do stretching or strenght right away. When i didn't know what was wrong with me I literally walked in circles and hardly slept, the lack of sleep was what was killing me. 'Trouble' is that by afternoon I have no real pain at all, and thats even without pain killers (but sometimes tens, that machine is wonderful) but when the physio did the leg test, there is no pain so she said it wasn't this. I asked if she'd come over at three AM when its a far different story but she laughed. However, when that cramping starts, as soon as I put ice on my lower back it goes right down. Apart from once at three am and six am, its strength training that bothers my underdeveloped core more. However, I'm knocking wood because its always dangerous to think you have an edge over a body problem, I'm only two weeks in.

    When I raise my left leg, the muslce weakness/soreness/pain intensifies on my right leg (piriformis and tibialis). I threw out my back after a quad strain on the right.

    • @drcharliejohnson
      @drcharliejohnson  Год назад +1

      Sounds like what we call a positive CROSSED straight leg raise... raising good leg causes pain in bad leg. HIGHLY specific or diagnostic of a disc herniation.
      What are you doing for treatment.

    • @AM-qw8zd
      @AM-qw8zd Год назад

      @Dr. Charlie Johnson, PT you got back to me, and so fast! Sorry for the late reply. I'm trying to walk as much as I can. I didn't sleep much for work a couple days ago and noticed it was much harder to lift up my toes on my right foot, felt dead. Peroneal nerve?
      Anyway, I have much more free time and I'm planning on strengthening my quads (initial weakness that lead to strain) and moving as much as I can in a pain-free motion. Lifestyle changes too.
      I'm hesitant to go to a PT just because I want a better understanding of this condition myself and don't want X-rays or surgeries. But I still should go nonetheless, would appreciate any advice you have on the matter. Thanks for putting a name to the condition! I should also note my left side has always been very tight, esp. hip flexor.
