For patients at home, they could minimize wasted medication in the hub by going ahead and taking a dose right then. There will be a tiny amount in the hub, but just once instead of a second time.
I love the idea of using this syringe for this procedure. I personally would add 0.5mL of BAC to the vial - but I think Canadian folks are splitting their single dose Mounjaro vials that don’t come with BAC without issues as long as they are practicing clean habits.
Good info for sure. I would like to offer two words of constructive criticism as a photographer/videographer. Keep it brief and succinct. This video should have been no longer than 5 minutes. Also improve your video equipment to something that focuses fast and keep your hands in the field of view for the viewer. An overhead camera and holder is great for this kind of thing. Doing these two things will greatly improve the quality of your presentations which are good and informative which is why I subscribed.
I appreciate the constructive criticism. Do understand this is no where near my main job and for me its really about the information. I probably wont be many more videos where an overhead camera/holder would be needed. Could I use the same needles used for injection (30G 1/2") also for filling up the vial? And if so, can I simply fill the vial with 50% of the dose and then inject the remainder (since I plan on splitting the dose in 2)
@@alderi2 larger needle allows for the smaller needles on the autoinjector to easily fit inside the needle you have on the syringe. Large needle is easier to get through the rubber top on the vial. Smaller needle can can also go through but then is a bit dull when you inject and some folks complain it hurts a bit more.
Want to make sure I'm gonna be doing this right, I have a couple 7.5's that I want to turn into 3 doses so should I be getting the 10ml syringe for extracting? I'm in England and it seems we get a clicky pen, so we click say 18 times to get the lowest dose. I assume if we wanted to microdose we would just halve the amount of clicks? Am not on the medication but am considering it. Thank you.
So what would I do to get a 12.5 dose out of a 15mg zepbound pen....insurance always has a problem with filling it and my wife went from 10 to the 12.5 but they have to call in a PLA to my insurance and omg every moth the same thing. After transferring all the contents of the Autoinjector pen to a sterile vial like I did in the video I would add .7 mls of BAC water to the vial and swirl it well so you have a concentration of 15mg/1.2 mls. Now every 0.2 mls you inject will contain 2.5mg of tirzepatide. To inject 12.5mg you would need to inject 1 ml of your newly mixed vial. Depends on how long you want to keep it in the vial. There is nothing written in stone but if you plan on keeping it for more than a week after you split the dose then its a good idea. Have I had patients do fine without it for longer? Yes.
@@GoodDrGI had my monjauro pens in my fridge like 4 months and I bought some luer lock pens to suck it up, but it wouldn’t work. It was a 22 gauge and it went over it. I swear the medicine was like to jelled up to suck out. Also, what I did wrong was inject the pen into it but from the wrong side!!! So I did lose some of the medicine, I will probably use my same pens but make sure to inject it inside by unscrewing the needle from the top. Don’t do it the other way, or it will come out through the needle part!!
Storing either way is ok. I prefer fridge because we live in south Florida and deviations in the middle of the day due to power going out could ruin your meds. Why take the chance but if you want to you can leave at controlled room temp. I explained in the video why I do not recommend not injecting directly into the vial. 😉

@ Compounding pharmacies will do this for the patient based on the prescription from the doctor . You will receive a vial with multiple doses as well as the insulin type syringes, with no need to extract the medication from a pen into a separate vial . The cost is usually less as well , although not covered by insurance.
Great question! If you check your autoinjector you will notice the all have the same volume (0.5ml of liquid) inside. What changes is the concentration of medication in the fluid. 2.5, 5, 7.5, 10, 15mg respectively. If you are going to try to split dose anything above 5 mg I would suggest breaking out the calculator!!
@@GoodDrG So all the dosages are 0.5ml - Its the ocentration of the dose (2.5 mg through 15.0) that's in the 0.5 ml. In other words, the volume is the same always, the amount of concentrated mediciine in the liquid varies based on dose. I'd go to 5mg (two shots in a 10mg dose), so that's easy math. Once I get it in a vial, I just use half the starting amount pulled out of the pen. Thanks!

@ well let’s think this through. If you wanted to half it and use i1 pen for 8 I’m going assume you will inject the first one when you split the pen. That leaves 7 weeks. In my experience the meds don’t fall off a cliff in effectiveness once they reach BUD. So one dose past 6 weeks isn’t terrible. If you are concerned then make the first dose a full dose and problem solved.
@GoodDrG thanks. Reason I ask is my insurance covers for 6 months. After that, who knows. I pay $26 a month. I've hit my target after 3 months and I'm on 5mg of zep. If I continue to titrate up to 15, one pen could last 3 weeks. I guess that's actually fine. 1 month of four pens, could last almost 3 months...for $25!! And I could build up a stockpile of pens too. Hey Dr g I just did my first splitting of pen 12.5 mounjaro , when I put it in the insulin needle it ended up being 20 unit and 1 line after in each needle so around 42/43 units is this correct ?
Be sure the syringe you are using is the same size or larger than the original in the injector otherwise you may denature the protein. Add to vial by running down side of vial. Do not foam.
Wish your camera focus was better on the close ups 😒 Good option tho, thanx! I've seen others who use a larger needle to sheath over the injector pen needle and do it that way. Then they inject the medicine into a sterile vial. Similar method to urs, just one less step.
@@dreamgirrrl9 you really don’t miss anything on the close ups. But yeah the autofocus didnt cooperate. As for that one extra step I’m sure it’s absolutely worth it. I’ve seen and tried that over the needle method and let me tell you threading that needle isn’t exactly easy. Especially since most of the folks using these meds are a bit older and our post 40 year old eyes aren’t what they used to be so threading a needle on a needle just to skip a single step is high risk low reward. I respect those that can tho! ✊🏾

Doctor if I am going to be using two doses in the same week can I just let the medication stay in the syringe and just change the needle on the second day?
I’m a Registered Nurse, and I wouldn’t do this. First of all, the “insulin syringes” we are given from the pharmacies for this medication don’t have a needle that is removable, so you can’t change the needle. Even if you could, it’s not best practice to leave in syringe until next dose,; there is greater chance of contamination any time medication is removed from the vial. And there is potential for bacterial growth as it sits in the syringe because it is preservative-free. This practice would never be allowed in the hospital setting as it’s not considered safe.
@@joelle6933 you can and should if you plan extending the doses past 7-10days. Not needed of you decide to use it in the same week. Remember adding BAC changes the amount of medication per ml.

I start mine tomorrow….my doctor is having me do it twice a week, and also a shot of vitamin B 12 with MIC once a week. This is different from most.

There is. I prefer this way since the needle on the injector is really thin and if not done exactly right will bend and will spill your medication.

@ I think you may be mistaken. Take a look at the pen. It will say the amount of liquid in there. It Should say 0.5 ml.

@@joelle6933 you can and should if you plan extending the doses past 7-10days. Not needed of you decide to use it in the same week. Remember adding BAC changes the amount of medication per ml.
Hello and Thank you for this video. Can you clarify what the size is of the syringe you used, was it 1/3/ or 5ml? In addition, the size of the needles to transfer to and drawing up from vials. There are several choices on Amazon for both items. Thank you in advance!

No one ever talks about the environmental impact of all the pens. You know they aren’t being recycled. The syringes and single vial I use once a month seems more environmentally friendly.
Impact is much less then obese people over consuming food. I’d call it a win
If you contact the company they will send for free a recycling container that you send back to the company.
