    Im having to pack a wound in the bottom of my foot I had infection in my foot cleaned out I'm taking IV antibiotics right now trying to save me from losing my foot as I'm a diabetic

      Me too, my two wounds are on my leg from abscesses drained by surgery. I just finished packing it by myself at home for the first time!

    Thanks for the info. I have had problems getting my packing out a few times. It's unbelievably gross sticking my fingers past the threshold of the wound. So now, I know to leave a bit of the gauze sticking out to pull on. (I can't have tweezers. Long story.) Thanks again.👍🏾

    Hi Alyssa,
    When I was on placement at a surgical ward I saw some wound packing but unfortunately I only got to do one wound packing myself. Primarily we were packing wounds from incision and drainage of abcesses post operatively.
    1. How can you ensure you are correctly packing the wound and keeping it 'fluffy' or not packing it too densely, if you cannot see the base of the wound and the actual incision is very small. For example I have seen nurses packing perianal abcesses, and the actual incision is very small and you really cannot tell how large the wound is inside so it seems like they just pack it until it feels full and they have packed a similar amount as they have taken out.
    2. I do know that we want to keep a tail for packing dressings, but have not learnt about taping it, how could you do this while being sterile, even if using sterile gloves you can't get sterile tape unless you used like a steri strip or something? I see nurses leaving the tail hanging out but they don't tape it.
    3. For outer dressings I have seen absorbent dressings put ontop of the packing dressings such as a combine and then this is attached to the patient with fixomull. However what confuses me is that I rarely see waterproof dressings being put ontop of packing dressings. Could you explain why waterproof dressings are rarely used for outer dressings, and when it would be ok to use a waterproff dressing as an outer dressing? Would it still be ok to shower if the patient wrapped the area in a plastic protective bag if they had no waterproof dressing?
    4. Why do we typically only clean the periwound area when doing a dressing change for a packing wound and not actually inside the wound? I have only seen once that a patient with an axilla abcess had the wound irrigated with saline, but even then the wound wasn't cleaned like you would with a typical wound using a cotton ball, it was only irrigated with saline. Other than that axilla I have never seen the actual wound be cleaned. How do you decide to clean the actual wound vs periwound only?
    5. When removing old saline soaked ribbon gauze it can become dry and kind of stuck to the wound, should you just remove it, or would it be beneficial to kind of soften it by putting a bit of saline so it doesn't 'rip' away from the wound? This would also help with pain. The issue with this is that you may put too much saline into the wound and it kind of 'pools' at the bottom and it might be very hard to get out.
    6. With the perianal abcesses it seemed quite challenging to put the cover dressing on in a way that did not interfere with the patients toileting, at times you cannot put the cover dressing on perfectly as it would block the anus so you adjust it so its not blocking but its not covering the packing in an ideal way. It also seems due to how close the dressing is to the anus it will get contaminated after the patient uses the bathroom. Is there no way to really avoid this, just do the best you can? Also this area seems very prone to the packing accidentally being removed just due to the nature of the area. A patients packing fell out from their perianal abcess.
    7. Would you be able to make a video showing a demo how to wrap ribbon gauze around the forceps in a dressing pack? I feel like this is a super simple thing but I haven't been able to find a video on this on youtube and I forgot.
    I remember at university we were taught how to pack wounds with saline soaked ribbon gauze and we were taught a very nice and neat way to wrap the soaked ribbon gauze around the forceps so its easy to control and then pack into the wound but I forgot that. I have only packed 1 wound so far, and when I did that one I did wrap the gauze around forceps but was not very neat. I feel like the main thing is ensuring you are sterile, and correctly packing the wound, which I'm more confident in. But this was just an area of the skill that I feel I could improve on.
    8. Wound packing was very painful for patients even with strong analgesia, do you think going slow would be better as you can be more gentle or you should aim to do it quickly? I feel like quicker is better as they have brief pain rather than prolonged.
    9. After removing the old packing and before packing the wound, do you think it would be ok to use lignocaine gel inside the wound if pain is extreme? I feel like this would assist with pain a lot. But the lignocaine would get of get washed out by the saline of the new packing.

      1) it takes practice but you go into the wound when u hit the base you pull back about 3 mm then you keep going in with the cotton tip application when you hit any resistance stop this is the packing you are feeling any you do not want to pack it down its all about feel. hallow out an apple or orange and get a feel for it
      2) you do not have to put the peace of tape; this is optional use steri strips these are sterile. also you are taping it away from the actual wound
      3.the goal of packing it to keep a just moist wound a tagaderm will not absorb excess moisture.. although you wound use an allevyn dressing this is shower proof.
      4) if you can feel the wound base with the cotton tip applicator you can flush it with saline, you may need to go in with packing to ensure there is no pooling of fluid in there if it is really small hole and fluid is not returning. note if you cant feel a wound base you do not pack or flush
      5) unless the dr ordered wet to dry dressing you get that packing wet saturate it leave for 5 mins and it will release without over saturating it. if this is a problem maybe a small amount of hydrogel should be suggested to the dr
      6) yes this is something we all tend to struggle with as nurses we can only do our best
      7) I will have to see if i have any sterile wound trays here
      8) if the pain analgesia is not working you can ask the dr to order morphine gel this helps alot. always take your time do not go fast as it will cause pain and always tell the pt they can say break and you stop no matter what. pain can also be increased with stress of dressing changes knowing it will cause pain . you need to advocate when something is not working especially when it comes to pain.
      9) you wound clean the wound 1st ( flush with saline then apply the topical (you must have a dr order) you let it sit for 5 mins before continuing this will allow you to perform wound care without pain.

    How long for this type of wound to completely heal? The reason is my wound after 2 weeks still like moist and wet with pinky blood color.

      It depends on many factors. Is the wound progressing ?

      @@ba5tardhow long did it end up taking to heal? You may have been deficient in zinc helps with blood clots
