In my experience you can fold the end in (known as the Fly End) just enough so you have a clean edge to start your first 90. It's derisively called a "cheater fold." It might also screw up your ending, and you won't have enough to tuck in, so experiment with this beforehand. But no, it's not necessary.
The change you are seeing is camera angles. You actually see the first fold twice from a different angle. If I remember right, Wesley was having trouble with that fold staying taut so they filmed it a few different ways. I'll be sure to pass your comment onto the videographer.

Depends on the size...usually the person holding the union jack side will make a fold in the opposite direction to fit in the tucked side
