He was hit by a car and he damage his shoulder badly. This injury he kept hidden from the company for a while, but then it kept getting worse with all the saving etc. So he had to get surgery in 2020.

I’m a Psychology Professor in UK. It’s understandable if your heart feels heavy but, shows he’s now at peace & able to share it, as it was. I’am proud as a Psych. I hope all we talk to, no matter how long it may take, they come out the other end & don’t give up. Anyone struggling use this to help heal, he’s not just done this for himself, to let Army in but, to also send the message don’t suffer in silence. Yoongi is in peace with himself already, he can share what had haunted him for years. Ppl who had no idea what trauma is wont know haw hard for them to open their heart and talk about it afraid of rejection, what people see them, feeling alone isnt something to be proud of for ppl like them. They need someone beside them, even just sitting with them without talking, knowing that someone beside you mean a lot to them. Amygdala and The Last really give me a deep thoughts on life.

He did talk briefly about his struggles in BTS context too. He mentioned his mother's surgery during the song "Move" and in his HYYH pt1 album review, as well as his shoulder injury. But I do believe in order to heal and move on, he needed to do this on his own. So I see this as a very brave move to make a whole song about his struggles that affected not only him but the whole family and his possible guilt that he was far away when his father was diagnosed with cancer. Writing this song must have been quite uncomfortable for him, but from psychology perspective, it probably helped him one way or another.
I think he referenced the lotus flower cause it symbolizes resiliency, hope,strength and rebirth. It can bloom despite challenging environment and even grows in mud. The fact that he managed to choose to live and eventually be successful despite all the hardships and suffering he went through, makes him a lot like a lotus flower.

As for as we know, his mom received heart surgery around the time he was born, and after that, in 2012 he had to work a part-time job and got into a car accident, as well as going to school and being a trainee at BIGHIT. He didn't tell BIGHIT he had a shoulder injury cuz he was afraid they will kick him out of BTS and he endured the pain all these years until 2020 when he actually had surgery to fix it, all while taking antidepressants for depression, anxiety as well as hearing the news about his father's liver cancer. Suga has been thru so much, and in his song "The Last" he talked about going to counseling, the doctor asked if he had committed suicide/harming himself and he said yes he did. The one on the scooter is young Suga when he was out on delivery and the one who has the scar and is trapped is "Agust D". Suga said Agust D is his alter ego and the side where he have to carry all the heavy burdens and negative emotions that the "BTS Suga" didn't reveal to the public. So the last scene is Suga trying to save Agust D but he couldn't. In Daechwita and Haegeum he has always try to kill off the corrupted AgustD side of himself as they were being corrupted by greed and power, but we can see as of now Suga has tried to accept his alter ego and all the negative sides of himself.

I had so many emotions because we know the battle he had to face and also for his team. There were countless criticisms, prejudice and even racism. I think his music help him endure many hardships. His experiences made him stronger and he was able to create great music because of that. He needs to trust us because Army will always love him.

Many others are just giving facts about what happened to him in the past, which he plainly explains in the song. However, the importance of the song is how he has dealt with the trauma. His scared mind is left trapped but manifests itself through his actions. In the other songs (Daewitcha and Haegeum) we see the effort to get rid of these manifestations by shooting them or using his new self to overcome them. But it is always knocking and the pain always remains fresh in his memory (bloody scar). We hope Yoongi continues his fight as this gives hope to many others like him who have this struggle. Thank you Yoongi for this!
The accident is a memory. His stuffed his shoulder but dare not tell Big Hit for fear of losing his place in BTS. It was never treated for almost 10 years until the end of 2020. There were pills too, and then rushing in what looked like an emergency rush in a hospital.
