You want to be close enough to cause the pupils to constrict and see their response.

what the numbers mean.. for instance if a question states, Pupillary response was 3.. Can you explain that please?

When you shine a light into a patients' eyes, their pupils will constrict (if the response is normal). You will check their eyes in the dark first and note their pupil size. When you shine the light into their eyes, you note their pupil size again, which should be smaller than in the dark. Let's say their pupils in the dark had a diameter of 5 mm and in the light a 3 mm diameter. The correct way to chart this is "Pupils respond from 5 to 3 mm" in addition to the other things we check with this assessment - equal response and accommodation). Look at your penlight - it may even have a grading scale of pupil sizes.
