(VOLUME 21 SPOILERS) From what I've gathered, Tohka's alternate universe self originated when that world's Shido was killed, and her mind was shattered far beyond a typical Inversion state, she eradicated all of the Spirits of that world and went on a killing spree, devastated that she lost the most precious person in her life.
Not just eradicated but full on had their angels turned into swords of themselves with Kurumi being the only spirit to successfully escape the manslaughter that left her angel at half capacity. Its a really brutal reality. The idea that a broken spirted Tohka could be so powerful is actually fluffing scary ASF.

Tenka's form isn't just not holding back but she actually enjoys going ALL OUT 100% of the time. Meaning up until Tenka selflessly for Tohka's sake agreed to protect Shidou just so Tohka would cool her head. Persona wise Tenka is the true personality and Tohka is more like a secondary inner child personality that Tenka kinda created as a fragment of her former child self. Like a pre spirit personality. That is super innocent but also Tenka's like largest weakness.

World continues without Shido in this timeline after Shido got blown by inverse Origami's power so probably Mio goes something about this that we all know that Mio already met Shido before (after Mio in Phantom appearance give Kotori fire sephira crystal). Also Mio can manipulate timeline with her power to make Shido alive at all cost. New timeline: Started after Shido get blown 5 years ago while saving Origami's parents from inverse Origami and wake up in his room. The timeline is in 11-12 in light novel volume and continue until vol 22.

I don't know why Tohka appeared at last since Shido is no longer live to meet Tohka in the future of this timeline...
