Kojima admits that if it were up to him, MGR would've starred Grey Fox. But it seems nobody else at KojiPro is even capable/willing to spearhead a Frank Jaeger story. Kojima himself appears not all-that motivated, either...

Since Kojima wanted to hand development to newer people while sticking to production, he went with it. It only happens if you fail the QTE, wich seldom happens (happened to me once of my 20+ playthroughs. The reason he failed is because he didn't spam swift attacks in Blade Mode when he tried to cut his heart open, he chose to manually aim wich sucks at that particular moment.
