A lot of people believe that this is about a sadomasochistic relationship and he is the one inflicting the pain, but When he sings
"Ich seh’ im Spiegel dein Gesicht" " I see your face in the mirror"
hints that the person he’s singing about is actually himself. I believe that the song is instead about self-harm and the pain and pleasure associated with it.
This is a twist for those who register it.
Also the fact that Till likes pain makes me believe that he's talking about himself...the 2 examples that always come to mind are from the piano version of Mein Herz Brennt the cuts on his body are real.
The flogging in the Rosenrot video is real and all the members participated, with (of course) Tills back being the worst followed by Pauls back(i think)..they all put in sacrifice in Rosenrot..one last ex. Is asking to get hit harder in the "Ich Hasse Kinder" Video....

Finally someone mentions this, plus the bracelet they sell based on this song has a metal plate which looks like a razor on it with ‘ich tu dir weh’ written into the metal and I think that’s a pretty big hint from the band that it is about self harm considering how many people use razors to self harm.
Song is not about being a sadist. It's actually the complete opposite. It's a masochist having a conversation with himself while looking in the mirror. Many of Rammsteins songs are purposely twisted like this just to mess with people that are perhaps a little bit ignorant. Du Hast is a perfect example. Their English version is not even remotely close to the original.

The line "Ich seh im Spiegel dein Gesicht" --> "In the mirror I see your face" indicates that this song is not about two people but one person talking to themself. Therfore this song is about self-harm.
It's almost a bit "Fight Club" maybe?
You can also check out Rammstein "mein Herz brennt Piano cover" it's basically the same as the normal version but way more intense
They made a song about a real cannibalism case in Germany. The songs name is "Mein Teil". You can even read about this case on wikipedia.

It is about S&M (sadism-masochism , not pure sadism) the song was censored in Germany when it came out in 2009
