Oddly, my wall connector failed in the same fusion too. I bought it Jan 2021. I've been having derating issues for years but just thought it was because it was hot in my Central Texas garage. Though after reading several forums and talking to Tesla support, it seems like it was derating in conditions it shouldn't have been derating in. Tesla warrantied it. I was using the "Wall Monitor" app and tracking the reported internal temperature and as soon as it reported a 200 degree PCB Temp, it would derate the charge session. Mine, a lot like yours, never felt hot to the touch and if it really was getting to 200 degrees inside, you'd think the case would feel hot... It's installed on 4 Awg wire and properly torqued to 50in/lbs. We've took a temp sensor to the wire and it's getting nowhere near hot enough to worry about.... Issue was obviously internal to the wall connector.
Also, to add some info around the flashing green provisioning light on the wall connector. That light always come on after a power cycle regardless of if it's already been provisioned or not (or you can enable provisioning mode at any time by pressing and holding the button on the charge handle for several seconds). Once the provisioning settings are turned on, they stay on for 15 minutes. All the flashing light means is the built-in WiFi network is broadcasting and you can connect to it for provisioning. After 15 minutes, the light stops flashing, and the self-broadcasting WiFi network is turned off. The provisioning settings are also locked out at that point. In older firmware, you could still get to the settings via it's IP address on your WiFi network once the wall connector joined but that's been locked out in more recent versions. You can only modify its settings while it's in provisioning mode now and your connected to it's built-in WiFi network.

I'm curious if this is still an issue or not for you. I'm also curious what wire size you've used. I've heard those running 6awg have been running into heat issues. I've maxed mine out at 4awg, but haven't had a chance to properly put it through its paces.

Er had this problem. And the Tesla people forced it to start remotely every time we called them. It started smoking really bad.
I’m gladly sending you a picture of it.
They didn’t want to replace it. Not even after they almost burned our garage down . Lol

I've also had the red light flashing issue, but there was no way it was due to heat here in NY. It started happening in September, and the nights here are now fairly cool - the days of 98 degree F are over by this time of year. Moreover, I only charge at around 1 am, when it's frankly a bit chilly. I think this can often be a warranty issue. Don't assume it's overheating.

The same thing was happening to ours after it was installed. Turns out it was a simple fix as the electrician did not tighten the wire to the specific torque specifications. Now it is charging at 48v.

Even though it is rated for 48 amps you should throttle down the charging to 25-32 amps and only use 48 amp charging when fast charging is needed. This is mainly to reduce heat on the wires breakers and a plug if you use one. It's less stressful on the battery too. Feel the breaker after 3 hours and you will know what I mean by heat when using 48 amp charging.

I agree with you. 32a charging is a good sweet spot for charging and battery stress.
