[2018] HIV Doctor Visit (Prescribed Biktarvy)

    Well, you're in luck, kids...my doctor (Dr Jay Gladstein) was gracious enough to allow a camera into my appointment so that we could capture it all for you.
    During my appointment:
    -My HIV medication was changed from Genvoya to the latest and greatest ( Biktarvy )
    -We discussed whether it's safe to take fitness/bodybuilding supplements while on medication for HIV
    -We went over my vitals
    -I got my blood drawn
    -And gave a urine sample (no behind-the-scenes footage there..)

    Raif, thank you for this video. You will probably never know how helpful this is to guys with HIV. As a teacher/counselor of young people, I can assure you that it is not only noble of you to make these videos but that you are helping tremendously to relieve a lot of anxiety in others.       @@RaifDerrazi hey can you please give me some advice i have tested negative at 5,6 and 7 months since exposure and i dont know if i should move on or what to do i am just stresssed out every single day. Sometimes i feel suicidal everyone says the tests are accurate even the social worker but i am just afraid.

      @@RaifDerrazi rafi bhai, pls help me. I had oral sex with HIV positive girl. After 5 days body itching started on neck and spreads to remaining body. Today is 36th day. My PCR DNA at 14th day is negative. Up to today I faced all symptoms except fever and fatigue. (I faced itchy rash , tonsil swellings, white tongue, joint aches, pimples on scalp, hot head) now I m waiting for 45 days PCR test. Can I assume my symptoms are for other Std or anxiety? If I infected, how can I manage my family?

    Wow. Excellent video. I’ve been using Biktarvy for about 6 months and I feel great. I was on Odefsey for 2 years prior and didn’t realize I wasn’t 100% until now. I feel the healthiest I’ve felt in 3 years.

      @@RaifDerrazi Yes your doctor is the best. He examined you without putting on a hazmat suit. I went to an ER and the doctor put two pairs of gloves and even put a glove over her thethoscope so it wouldn't touch my skin

    Hi Raif ! I'm a pharmacy student curently studying HIV drugs and I really liked the way your doctor explained how HIV is similair to chickenpox virus. I will use the analogy with my patients to explain how those medication work for sure ! You really have a great doctor btw. He makes sure you understand how to take your medication and expained reeaaally well the differences between all of them. Thanks a lot for posting !

    You're a great guy Raif. If and whenever you feel down again for any reason, just know there are people in this world that you've made a lasting impression on. I can honestly say that this world is waaaay better off with people like you around. You stay working out, following your meal plan, etc. I know it's burdensome at times, but we all want to keep seeing you around bro.
    And thanks for letting us witness your medical appointment. This has to be one the most informative videos on HIV treatment I've seen. Somebody should hire you to be some sort of HIV treatment ambassador or something. Just a regular guy going through the process who can explain things in a simple way. I'd argue that we need lots of guys and gals like you helping to dispel ignorance about this condition. Some folks to explain PEP, PrEP, antiviral meds and their side effects, the whole gamut.
    So many people lack knowledge about this and the doctors, bless their hearts, can at times come off as a bit too academic when explaining stuff. Plus they've an air of authority about them. It'd help to have regular folks talking to newly infected people. While I think it's okay to have HIV negative social workers/case managers and what not, I think folks like you should be considered when hiring of social workers comes up. You really can look folks right in the eyes and say "I know how you feel. I been there." That'd really help I think.

    Wow... thank god for research I'm so glad they have medicine that will keep you healthy. I'm recovering from Lymphoma and the day I heard I had cancer I thought "I'm going to die " but with all the medical advances I've been in remission for 2 years. Stay well ❤️

    These videos are awesome! Thanks for posting these and eliminate the stigma and fear of HIV. Get tested and know your status ... AND there IS treatment IF you have HIV. Basically having diabetes or high blood pressure is worse in that respect. Its sad this treatment was too late for people like Rock Hudson and Freddy Mercury.

      Yes, the world though of it as a death sentence for a while after the first case was diagnosed in 1985. Today, it is not and we have learned so much.

    I’m currently on Triumeq. The only negative I would say is my sleep has been terrible since being on it. On my next visit to the clinic I’m going to ask to be put on something else. Lots of love from the U.K. and a huge thanks again for doing what you’re doing.❤️🙏

    Hey Raif ! Oh wow it's been for a while seens I said hello, firstly thank you very much for sharing, really appreciate, been busy traveling and breaking the bridge of stereotype minds on people who are blind and visually impaired.. Anyhow it's so nice that you made a video on this medication subject course if you can remember how I turn visually impaired, just a quick recap due to CMV virus and doctors trying on medication on me . Not being negative but it's very true everyone's body functionality is different and me myself was confusing to the medical community, the best outcome is I'm alive and getting stronger.🤗 . I'm on Trumeq and for my maintenance of CMV is Valganciclovir Aurobindo, in addition my doctors prescribe me with a high doses of vitamin D combined with Calcium, I've also been asked to take a high doses of magnesium hydroxide, and very true what your doctor said do not mixed it with the time together with HIV medication, so in the morning I take all the medication and late evening comes my HIV med.. There is also a new medication in the future through injection and this injection should be taken every 3 months only, can't wait for it , no more tablets. Anyways it's always a pleasure seeing you looking good and being so positive and helping the community, keep it up my friend and can't wait to see what you share the next time..     Raif, thanks for creating these videos. It’s so helpful for those dealing with HIV. I’m curious about your comments about being celibate for 2 yrs. Are you fearful of passing along the disease? Just curious about that aspect of dealing with the disease. You’re a brave guy for putting yourself out there, Raif! 😊

      I don't have any fear or concern about passing on the disease. I'm undetectable. I won't pass it on. And it was not the basis for my not having sex for so long!

      Haha...yea people get weird about it and are afraid to be associated w/ or assumed that they might have HIV. Just speaks to the need for this kind of dialogue around reducing stigma

      Robert Paredes it’s gonna be a while before I can do that. Still finishing up my doses of Genvoya. But I like that idea! As soon as I finish I’ll create a vlog where I try it for the first time and then talk about my experiences periodically over the course of a month 😉🙌🏽

    Seems like you have the same bad thoughts or worries, that I do when the thinking about medication. Some big war rages on and cant access your meds. Well if that dose happen think of the people on blood pressure diabetes medications and heart issues. Well people with HIV will go on for years without medication gives time for everything to return to normal but for others above they face much bigger problems. very good video

      Scotty Nelson so I still have about 6 months of Genvoya to get through before I start taking the Biktarvy (even tho I’m getting the medicine now)...that way once I start that medicine I’ll have a stockpile saved up of that too

      saw my ID doc. he is not against biktarvy, but he didnt see any benefit either . and even tho after 3 years and still cant get my tcells over 500 (so close just wont climb up LOL) he is extremely satisfied with my progression so ill be staying on genvoya.

      Scotty Nelson Yea, I totally understand that. And if it ain’t broke, don’t fix it! Haha I know how tricky it can be with CD4 count too...mine jump ALL over the place. I’ve gotten it as high as 800 and then the very next time it was back down to the 400’s...up and down. But like you, mine tends to linger towards the lower end of the spectrum.

      Raif Derrazi and @Scott, firstly, please excuse my long message as I have not much time now despite my real interest in the matter. Secondly, would you be so kind as to elaborate a bit on your CD4 and CD3 counts? For the record, I was personally involved in changing the then existing regulations in Brazil, which as originally drafted and followed, would only release anti-HIV meds once patients had lower than 500 CD4 counts/markers. Our group was able to demonstrate that the then existing policy was a huge mistake as it led to patients enter AIDS status hence much complicated (expensive, time consuming) treatments being required. Nowadays, once HIV is detected, medication and treatment start promptly. However, it is well known that CD4 counts may fluctuate, but under proper medication, levels should naturally rise - though not reaching back to the most desired levels pre-HIV/AIDS contraction. 700 to 800 are great results. Nonetheless, fluctuations that lead to counts below 500 or counts that never reach back at least 500, may be associated with many other issues that we are still investigating (e.g., resistance to some meds components, other viruses that may be linked to other types of HIV or not, damage to an organ that wasn’t originally detected, years of treatment with previous medications or no treatment at all during a longer period of time, amongst others). (Un)fortunately, the HIV type that is vastly encountered in Brazil (and most South American countries limits local research. I do have my theses, hypothesis and research, but I am in the psychiatric field dealing with chronic illnesses and the need to reassure a quality-life, -head and -wellbeing condition, mostly ignored in developing countries, specifically when treating patients within the public health system (> than 99.99% of our HIV/AIDS patients in Brazil receive medication and treatment through the public system). Meaning the public health system basically ignores state of mind conditions when already spending funds in the primary focus of chronic treatments in all practice areas (immunology, oncology, endocrinology, hematology, hepatology, amongst others). Arguments therefor vary from lack of resources, possibility of causing “intoxication” to other organs - such as liver, kidneys -, loss of focus, irrelevance of psychological conditions, amongst others. Frankly, I firmly believe this is horse shit as no patients with chronic diseases are free from having or developing even more complicated emotional conditions, mainly depressive related. Imbalances of hormone and neurological chemical elements will naturally impact the organism (and wellbeing) as a whole. This is also true for the other way around, that is, imbalances of other nature in any system, in this case, immunological status (CD4 in special) may lead to imbalances regarding emotional/brain’s wellness. Having said that, what usually would you hear from doctors and friends dealing with hiv regarding CD4 levels/fluctuations? And how does this CD4 concern impact your experience/expectations?       I got diagnosed this year 2018 in april and biktarvy is the only anti viral ive been on, so i cant compare it to others .i like it. The only side effect that i notice was like dry mouth the next morning sort of like a hangover and only at the beginning . Now i dont feel any side effects. Can i ask how long it took for your cd4 count to go above 200 if it was ever under.

    Your dr. seems awesome. Biktarvy is a great regimen. Has three drugs. Juluca which was FDA approved last Nov. has only 2 drugs. Less drugs is better.

    While I champion the idea of medical advancements, I always get the feeling that doctors get together somewhere with pharmaceutical companies to pimp medications, and somehow get a kickback from pharmaceutical sales. They act like salesmen. If there was a cure, I have to wonder... what the result would be.

      @@RaifDerrazi all the best I want you to live a full happy and healthy life I had my exposure and I AM on my testing my western blot was negative at 93 days.I have all ARS symptoms and I did not know about prep and pep got to know about pep after four days of exposure currently I am on my 19th week. I gave a p24 blood test hope it turns negative.)

    I can never forget how scared I was when I tested positive to HIV, it seemed like my world has literally come to an end and I'll probably be dead in a week 😂.... and yea it's difficult to jump right back into your sex life after testing positive to HIV, almost 2 years for me too without sex and my doctor's keep asking me why, but they don't understand how difficult it is to resume your sex life after this😂😂😂...

    Bro I saw your video about the symptoms, I wanna know that, at this status my body is not showing any inflammation or rashes but I'm having a sore throat and I'm sleepless at night and kinda feel like I'm loosing weight but I'm not sweating in night either. I know you will tell me to go for a doctor. But I'm in India if the people in here knows that if I'm having AIDS, HIV. They would probably reject me out of there groups . so before letting them know I have to shift an other country, So basically I Wanna ask you about this. If you can help me out a bit.

      muffin 03 Honestly, I’m not too sure about the availability of these specific drugs outside of the US. As for supplements...I can’t really recommend things as everyone has a different situation and needs for their body and how they handle their medication. All I can do is share what my doctor had to say and what supplements I personally take. If you want, I did a GENERAL overview of my supplements I take in another vlog...check that out. I may eventually do a more detailed listing of what I take/brands etc

    apologies if i missed a previous video where you explained this, but im curious about insurance/cost. i noticed below you mentioned continuing with your current meds while stacking up on your new medicine. insurance allows you to get both meds? what kind of insurance do you have and how costly are these meds?

      My insurance is 100% covered by the government. No, I do not get both medicines at once...I get a full monthly dose of medicine approx every 25-28 days...so over time I have a surplus. Also, I forget to take my medicine on certain days here and there and that also contributes to the surplus over years.

      Yep. I was diagnosed with AIDS...i began taking ARV and now i'm perfectly healthy with no side effects or symptoms

    I mean I appreciate it, but my heart still breaks for these souls. They are stuck in a sense to going to a dr. all of the time every single day, etc. This sucks in the end to be stuck on medication all DAY, YEAR, LONG. It's still a death sentence and time killer.

      uKnow it’s not a death sentence nor a time killer. I go to the doctor twice a year...a lot of people without HIV go to the doctor way more than I do. Certainly not a time killer. Also, how many people take multivitamins every day...or fish oil every day? Same thing HIV medicine. It’s easy. Not an issue

I am no longer shy about asking if a person has any kind of disease. AIDS is still a death sentence and an ugly one at that, for MANY people. People who have AIDS take AIDS lightly because they HAVE TO no offense.

      Raif Derrazi yes check on Ryan White- I use to help HIV patients apply for it(I’m no longer working there), but it is a health Insurance for low income HIV patients only, they cover pretty much almost 100% (Medical, Dental, Food bank, and some cases-Housing, Also Prescription, but you have to apply for “ADAP”.)...Hope this help some of you, and remember God still loves you.😘
