Thank you Doctor…. Very informative video. I have few doubts regarding my mixed feeding schedule. My baby is 45 days old. During the first week after birth I was not being able to breastfeed. Later I started breast feeding. His weight gain was not sufficient. Then doctor said to give Formula two - three times. Now I give him formula three times- around 9 am, 3 pm and 10 pm. Rest of the time I breastfeed on demand ( 2-3 hours gap). He keeps feeding for 40-45 minutes, sometimes even more. Sometimes after breastfeeding he sleeps and give 2-3 hour gap, other times he cries even after 1 hour feeding. Sometimes he even cries after 60ml of formula feeding. I am confused whether he is getting enough breastmilk or I should increase frequency of formula. Please help. Urine count is more than 7-8 times. Most of the time baby does nipple feeding. And I experience extreme pulling pain in the veins around breast near armpit while breastfeeding. Is there any cure for this? Please suggest. Presently I am using two times Lysoflam medicine for the pain otherwise not being able to feed.

Hi sir, I have a week old girl baby with 2.6kg birth weight. She seems to properly latch but only sucks for 5mins and then sleeps. Then for another 10 to 15 mins, she sucks very slowly while sleeping. Is it normal? Why does she sleeps so soon when she can't be full?
How is the weight gain, urine output etc? Consider pumping after each feed for 2-3 days to optimize output and give the baby a little rest and you could go back to full direct feeding-but if weight is already increasing, just wait 2-3 days more and no need to give feed while sleeping
@@SridharKs Thank you very much for responding sir. Weight gain seems to be normal as per the last paediatrician visit 2 days back and urine output was 6-7 yesterday and 4 today (so far). Even though she sleeps while feeding, she doesn't sleep if I put her down and wants to keep feeding (again sleeps while feeding). Also she passes motion with a noise and it was found to be frothy.
@@SridharKs But everytime while feeding for 5mins she sleeps even after she is hungery. So should I abruptly stop feeding at that point and wait for her to be awake again?

Hi, nasal congestion is quite common at this age, and frequent use of saline drops or spray is okay for that, with a slightly elevated head end of the bed. Regarding choking, avoid too frequent or too prolonged feeding, try to encourage baby to regulate intake.

Just wanted to check with you, as our baby was showing CMPA symptoms like blood in stool, uncomfortable while sleeping, colic we switched to znutramigen by mead jhonson. for initial few days she was reluctant and wasn't accepting thew new weird flavour however after 3-4 days of our persistence l, she has now started to drink the weird taste of Nutramigen. The question is - She is now 2.5 months and only drinking 60 ml in 3.5 hours and may be once in a day at maxxxx 90ml. She used to take 60 when she was 2-3 weeks which hasn't gone up yet. Is that normal? or can we do anything to increase? Cause she has stopped breastmilk due to the CMPA and 100% on formula, need tour guidance doctor. Cause my family and everyone are tagging her "Weak" which concern me as a father.
Hi, when she is in the recovering phase, the first focus is on regular intake. You could give 2.5-3 hourly if the volume does not increase in the next week or so (but no forcing, just a trial to give a bit earlier). Mother can breastfeed if she is avoiding all dairy products. If it works, it is okay to do both
@@SridharKs QQ - We switched to nutramigen on 3rd Dec and today there were some blood spots seen her poop again. Obviously she was cranky the whole day but i am just worried if Nutramigen is working or is there anything that needs to be done ?
@@udaymalhotra4462 make sure there is no contamination in your wife’s diet. Avoid eggs in her diet as well if still breastfeeding. If not on breast milk and this persists you need to go for neocate or Elecare amino acid formula

Hi, you can give sips of water with the food itself, you do need more water (use a bottle or sipper cup if helps)-continue breastfeeding as usual, consider expressing if needed (can top up soon after a meal). Avoid formula unless no other option-continue breastfeeding. Monitor weight monthly, that is adequate to assess (don't worry, baby will guide you)

Hi, usually we don’t need the second dose beyond 48 hours or so. However, if there is acute respiratory failure due to other factors like pulmonary hemorrhage even in an older infant with low compliance lungs, we could consider surfactant therapy. And in extreme preterm babies of below 24 weeks, some consideration for a delayed top up dose as well as described in Prof Stein’s talk

@@udaymalhotra4462 avoid forcing her to feed. Growth depends on family growth pattern too. How tall are you guys (etc)? Focus on her comfort and regular intake now, and the growth from now on (she could have dropped during the previous phase). And catch up later is possible

Try to express once in 3-4 hours, and top up 30-60 ml of expressed milk after breastfeeding for 20 minutes each time (3-4 hourly). Use formula occasionally if not even 30 ml of expressed milk available. Adequate rest is needed for both mother and baby. What you are doing is tiring for both

@@shainisimon3855 hope you are using a breast pump to express, as hand expressing has limitations. You can continue what you are doing but give 60 ml of expressed milk/ formula at each feed after a quick breastfeed (so baby can sleep 3 hours or so after). When your output improves on pumping after 4-5 days, reduce this amount and increase direct breastfeeding, then transition fully to breastfeeding. Use formula only if essential

Your baby has poor weight gain, need to consider factors like milk allergy that would also explain the restlessness. Discuss with your doctor about a trial of extensively hydrolyzed formula, and your wife has to stop dairy products fully. Review my videos on milk allergy

It will settle down, don’t stop as you would need to treat. Follow the pattern, consider some simple colic remedies and remember your baby will not be affected

Hi, you can give sips of water with the food itself, you do need more water (use a bottle or sipper cup if helps)-continue breastfeeding as usual, consider expressing if needed (can top up soon after a meal). Avoid formula unless no other option-continue breastfeeding. Monitor weight monthly, that is adequate to assess (don't worry, baby will guide you)

Hi, usually we don’t need the second dose beyond 48 hours or so. However, if there is acute respiratory failure due to other factors like pulmonary hemorrhage even in an older infant with low compliance lungs, we could consider surfactant therapy. And in extreme preterm babies of below 24 weeks, some consideration for a delayed top up dose as well as described in Prof Stein’s talk

@@udaymalhotra4462 avoid forcing her to feed. Growth depends on family growth pattern too. How tall are you guys (etc)? Focus on her comfort and regular intake now, and the growth from now on (she could have dropped during the previous phase). And catch up later is possible

Just strange thing happened- She's been on complete Nutramigen for the last 2 months and have been great! Her pain and irritation was gone for one month or so and then she was cranky since ytd and blood spots were noticed in her stool. Will monitor again but any concerns u see over here ??

Hi sir my baby is 25days old at birth she was 2.6kg now she is 3.1kg since I had a low milk supply we were giving her formula milk 30ml to 60ml every 2 to 3 hours now my milk has improved but I express every 2 hours I get only 30ml now I am direct feeding her about 1 hour.. after feeding she always cry so we are giving her 30ml formula is it okay to mixed feed like this or my milk supply is low(after feeding when I squeeze i could see milk is coming) what could be the problem plz help sir
Try to express once in 3-4 hours, and top up 30-60 ml of expressed milk after breastfeeding for 20 minutes each time (3-4 hourly). Use formula occasionally if not even 30 ml of expressed milk available. Adequate rest is needed for both mother and baby. What you are doing is tiring for both

@@shainisimon3855 hope you are using a breast pump to express, as hand expressing has limitations. You can continue what you are doing but give 60 ml of expressed milk/ formula at each feed after a quick breastfeed (so baby can sleep 3 hours or so after). When your output improves on pumping after 4-5 days, reduce this amount and increase direct breastfeeding, then transition fully to breastfeeding. Use formula only if essential

Your baby has poor weight gain, need to consider factors like milk allergy that would also explain the restlessness. Discuss with your doctor about a trial of extensively hydrolyzed formula, and your wife has to stop dairy products fully. Review my videos on milk allergy

It will settle down, don’t stop as you would need to treat. Follow the pattern, consider some simple colic remedies and remember your baby will not be affected

Yes, seems okay. You could substitute some of the formula milk with expressed milk, which could increase your breastmilk output as well
