I think 'Nightmare Mode' is just a recurring nightmare that the dorrman your playing in 'Arcade Mode' is having. All of them have very similar features to the residents of the Apartment, and the fear of failing because your not looking at every detail must be really bad for them - hence why it's harder and the dopples kill you immediately - and all of these residents are demon like creatures obviously. This might be obvious but aw well.
I think so too especially with the clown (in arcade mode doorman is warned not to play any games if a certain clown visits them in nightmare) and the starting video are strongly implying it to be a nightmare
