  • Patient‐centered care is the evolutionary model of primary care delivery. As such, it is important for all primary care providers better understand how to provide interdisciplinary, team‐based, patient-centered care in their offices with the physician as the leader of the team. Current and future healthcare professionals must recognize how to effectively manage patient care within the Patient Centered Medical Home (or PCMH). This includes PCMH principles, roles and responsibilities within a PCMH, how to participate in a PCMH, what contributes to the likelihood of success of a PCMH, and the professional opportunities and risks associated with a PCMH.
    The PCMH training site was developed by the Department of Family Medicine at the Ohio State University Wexner Medical Center to train healthcare professionals on this important topic. The site’s modern design and use of video make it an easy-to-use tool. The training material is organized into topic-specific modules, and each module includes a pre- and post-test as well as a video that explains and synthesizes more complex concepts.
