    It's crazy how kinesiophobia and pain can make some people literally not move in certain ways for decades. Had a patient who actually couldn't retract his scapulae without any neurological issues. His secondary motor cortex already threw out that movement pattern because he never used it. That's more of the issue as you said, just not moving enough or being afraid of it. And then some people wonder why their back hurts if they constantly avoid any movement that could strengthen it.

      Bingo. Seems intuitive at first that if it hurts, don't do it.... But we know from research that this spirals into fear-avoidance, and that the less you move the more it hurts, because of deconditioning (and other reasons). Movement is medicine.

    I have a friend that is a personal trainer who I was having a discussion with about this topic (and he also sent me this video later), and he talked about everything this doctor did. It's good to see more people looking at evidence based care! I didn't know any of this before, so thank you!

    Thank you this is so true I have two herniations L4 L5 for 6 months stiffness and burning free to move my spine much the chiropractor told me I'm so tired that I need a steroid injection I'm starting to move my spine more thanks for your videos to teach me that it's okay to move

    Then what does cause herniations? Because I exercise, I work out, I don’t drink, I was in the best shape of my life and I got a herniated disc and my life has stopped for 7 months. So please explain to me why this did happen to me???

    The lumbar spine is slightly arched anatomically, loss of that curvature often results in disc herniations due to the fact that the nucleus travels posteriorly when the spine is in flexion. There's plenty of pubmed articles on that topic. What are your thoughts?

      Yes the lumbar spine is arched (lordotic) but there are zero studies showing that chronic flexion causes disc herniations in living humans. I cited more than a dozen studies here showing the opposite. There's no relation between sitting/flexion and back pain or disc issues. I challenge you to cite a single study showing causality or even a correlation.     I understand you say the back needs all kinds of movements to be healthy but I’m in a situation that to avoid pain . I need to avoid some kind of triggers twisting and bending are trigger and also going in narrowing places . I have went to several doctors and spine specialists they had gave me no solution and said I have nothing serious , I have 2 annular tears L4-L5 and L5-S1 and a minimal spodylolysthesis ( maybe less than 5 % ) on L5 , all that pain started July 2021 really out of nothing and I have no history of back pain previously to it , but you are right as much I try to avoid the pain , as easier the pain will come , there is also a big connection with the mind and fear that I don’t know how to get rid of it . I would really love to go tho the gym again but that is just not possible, a wrong movement and I’m gonna be in pain for many days .

      There’s a fine balance between doing too much, and doing too little. Sometimes we do need to temporarily take it easy, but the long term goal is to ease back into normal activity.       @@ShapeshiftWellness yes I will watch it , the problem really is that my back have become too sensitive and many movements now trigger pain . The real question is how to revert this .

    I'm interested to know your thoughts on Stuart McGill's recommendations of increasing spine stiffness and relying on hip hinge instead of bending the spine when reaching out for something that's on the floor, even things pretty light, like a sheet of paper, etc. This in combination with improving my posture and stopping sciatic nerve stretching/bringing knees to the chest seem to have resolved my constant low back pain in a matter of hours (I've been pain free for 2 weeks now). In this case I'm rerferring more to my posture while walking or when sitting for short periods of time, as for work I continue laying on my bed/couch in a relaxed position as I've always done without any pain, switching positions when one becomes uncomfortable. Working at a desk just ends up killing my cervicals and my right shoulder due to external mouse usage and I've never seen any benefits from it. I'm kinda puzzled for why that simple change (walking posture, hip hinge, no stretching) has had such a good effect on my body despite my working position, which goes against every recommendation I've come across :O
    For the record: I have slight scoliosis on my lumber spine with some overdeveloped muscle on the left side of that area, I work out (calisthenics + weighted split squats/lunges) and I'm in front of the computer 9 hours everyday, but take lots of 5-10 minute walking/standing breaks throughout the day.

      Short story is that while posture and core bracing can provide temporary relief since it limits the motion of the low back, it should not be the long term goal of rehab. A healthy spine needs to move, bend, twist, and lift.
