I've been suffering with adult cystic acne for about six years now and it's so often not seen as a medical condition to be taken seriously. There is so much misinformation and misunderstanding out there, and getting proper help for it is so elusive. In my experience, it absolutely has an enormous impact on quality of life and mental health. This impact is so often trivialized or just plain ignored, and I'm so so helped just by hearing you discuss acne with a doctor who takes it as seriously as those of us who suffer with it do. Please continue the discussion, I'd love to hear more of Dr. Sam's insights on acne treatment in future videos. Thank you for what you do, love the videos, love the blog! x
I took accutane at aged 38 after many years of acne and treatments that didn't work and it honestly saved my life. Yes there were some side affects but the results far outweighed these. I am now 42 and my skin is usually pretty clear and skin is much less oily and I now concentrate on trying to repair the scarring which is an ongoing thing. I spent thousands of pounds on treatments that didn't work and this really was a last resort and something I thought very hard about doing and took very seriously whilst on the medication! Just happy to be able to be confident again and enjoy wearing make up!
I've never struggled with acne but I worry that it will hit me someday and I'll panic. You were both so comforting during this video. I imagine those suffering from acne took solace in your cozy voices :)
I'm so glad your doing videos like this. I'm so overwhelmed when watching this and have surprised myself by crying all the way through. I am 24 and have had spots/acne since I was 11. I have been prescribed everything possible from a GP and have had 3 referrals to dermatology which has resulted in roacutane every time. It had given me so awful side effects (mental health) but has worked but after 6 months my spots always come back. I'm so at a loss about what to do. My face is so painful and I have honestly tried so many different things. I am so sad and don't know what to do. Thanks to you the quality of my skin around the spots and scarring has improved with your help and recommendations so thank you. I don't know what to do though and my self esteem is so low. It's been 13 years of this and I don't know if it will ever change. Sorry to vent my feelings but I really don't have anywhere else to say these things. Nobody understands. Anyway. I was prescribed accutane (decutan) 6 years ago. The process wasn't easy as it had side effects on my skin in terms of redness and itchiness. Although I'm left with some scarring, it was one of the best decisions I've ever taken as it got me my confidence back. Looking forward to more videos with Dr. Sam. Thanks Caroline x
Iv been on roaccutane 3 times. Im now in my early 40's, Male, and was still getting the odd big spot on my face and back, shoulders. I clean my skin, look after it (to the best that I know how). However, recently for Lent I decided to drink 2 litres of Water every day (plain Water, nothing added into it) and I have to say iv had hardly any spots. My skin is clearer, toned, smoother, more plump. People forget the simple things. Sometimes I find myself drinking more... I will add, I don't drink alcohol very often (2/3 times per month I'll have a couple), and don't have much sugar in my diet... H2O - Drink it people!
Interesting to hear about retinoids, looking forward to your video on scarring/hyperpigmentation as someone who has only just got my acne under control after about 8 years of trying everything under the sun (and I'm only 21!). Caroline, your advice has been invaluable and I really wish that in the UK we embraced dermatologists, would have saved me a lot of time and money in the long run I suspect. This couldn't have come at a better time...I am currently looking at going into he cosmetic dermatology industry so this was very interesting and helpful! Acne was definitely something that was brushed aside by my GP growing up and not taken seriously. I'm hoping this is set to change in the future! x
I am currently 1 month into my roaccutane treatment and my skin has improved dramatically. I never thought my acne was that bad, I have cystic acne and only have 2 or 3 at a time, but they are so painful and leave awful scars and that holds you back. I always felt like I would be a nuisance to a GP, like it wasn't a proper medical issue, it's great to know that there are doctors that do understand and will take time to listen. It took me about 4 years to get my treatment jumping through hoops trying different antibiotics WHICH DONT SOLVE THE ROOT OF THE PROBLEM!!! I am praying it's worth it but so far so good!

I'm so excited for your Dr. Sam Series! The skincare industry is always coming out with new lotions and potions and it's difficult to keep up or know what is mainly marketing hype with little else to back up their claims or what is really revolutionary and has amazing efficacy. I would also love to know what high-end, drugstore and/or other skincare products that are her favorites for skin that usually behaves with occasional breakouts &/or mild rosacea. It seems that the older I get the breakouts and rosacea get increasingly worse. If possible to determine such a generic question, what high-end products are worth the investment or what drugstore products will give similar results without the expense. There are so many theories and perspectives on whether to splurge on anything that penetrates the surface but not to spend on surface-only products. Some favorite lines from both camps would be helpful.
As someone who has suffered from acne since the age of 11 (and still now at the age of nearly 24 my skin isn't the way I want it to be) I find your approach to acne and other skin conditions so refreshing and helpful. Early last year I went to see a dermatologist (after using rubbish antibiotics for months!) and the decision was made that I would go on Roaccutane for 6 months. I finished the course in September and since then all of the acne has gone, and the redness on my cheeks has improved hugely, but I now have acne scars on both cheeks. I'm excited to watch the more detailed videos and see if you cover acne scarring! I'm only 18 and I really don't want the scarring to be permanent (it's still pink/red) but I'm scared to put something on my skin that could make it even worse. Thank you for this video, it's great to hear more about the subject of acne.
I had acne from age 13 to 31. I had low self esteem from it and it affected every aspect of my life. It got very bad when I hit thirty. Up until then, I went to over ten different dermatologists begging for help. All I was ever given was antibiotics and harsh topical treatments that didn't do anything for my acne. When I turned 31, I finally told the last dermatologist to put me on accutane. My skin cleared within two weeks of taking accutane, and I took it for a total of three months. I'm 37 now and still have clear skin. I do find that diet affects my skin, though. Foods like dairy, yeast, and packaged foods make me break out, so I stay away from them. BTW, I was the happiest when I was on accutane because my skin looked so good. I was depressed before accutane when I had no control over my acne.
I've never had acne all over, but chin breakouts were driving me nuts for months. I finally changed my diet and took milk out of the picture. My skin has improved and I haven't had a major breakout since. But I really sympathize with people who struggle with it for life.
Thank you so much for this video. I have it extremely lucky when it comes to skin, but I loved watching this video because for awhile I wanted desperately to become a dermatologist because I wanted to help people with acne. I see how much my friend would beat on herself just because of that and it pained me. It also frustrated me that when I did see dermatologists, they didn't seem to really care or take it seriously. Hello Caroline. I really like your channel and i think that you are very honest with your reviews. However I believe there is a misconception that when doctors prescribe antibiotics for acne is because of bacterial infection when in reality is because certain antibiotics have anti inflammatory properties, and I think dr Sam was too polite to mention this. By the way I am a medical student and skin enthusiastic. Thanks for all your hard work and honest reviews.
Great video Caroline! I've had a horrible experience talking to a GP about my acne/rosacea, I was told I couldn't possibly have rosacea because I'd have a big nose(!!!!!!!!) they sent me away with antibiotics and a gel that burnt my face. Your blog has changed my skin so much but I do still struggle so any extra info. Like this is much appreciated 😘
This is so great, thank you. I'm nearly twenty and have struggled with acne (on my face but mostly my back and thighs) since I was 13. I never thought it was worth seeing a doctor or dermatologist about as I thought it was just something I had to "deal with" as a teen, so it's so good to see you talk about it like it matters. It really does have an effect on self esteem. I've been on Oxytetracycline pills since I was about 13 and do wonder if they actually do anything at all. Do you have tips on what skincare products are best to use on acne on the body? So many products focus on the face, but a little (and a lot of money) doesn't go a long way on the shoulders, back and thighs. I hate having to cover up my body just because I'm ashamed of my skin. X
I have to agree, i've tried multiple tetracyclines and theyve done nothing but make me extremely sensitive to the sun and empty my wallet. I would love recommendations for body products! I do find it effective to be vigilant about using a loofa and really clearing the area when you shower, and not to use soap heavy products (just like on our faces). I like body products that have salicylic acid, and to moisturize with things that do not clog pores (often times body products are cheaper, and have cheaper ingredients, so stay away from things containing shea butter or mineral oil/ petroleum). I totally know what you are going through, but keep your head up! xx
I think as society we need to change this whole thing about teens just getting on with having acne. Yet if you get to 21 the same thing becomes "adult acne" and becomes a disease that everyone wants to treat. X
My boyfriend and I are highschool sweethearts. In early highschool his acne was extremely bad that he needed medication, I can't remember which, because it physically hurt him as well as mentally. He couldn't lay on his side in bed because it hurt that badly. He was one of my good friends at that point and he had to go for weekly blood tests to check his cholesterol and all sorts. It cleared up beautifully! It did scar quite bad, and it left him with some lumps on his jawline but they're just victory marks that he can embrace.

Dr Sam is gorgeous. I didn't really think that there was a lot of information in this video, as in I didn't walk away knowing what to do now. Apart from that I'd even book a flight to be seen by Dr Sam.
After over 10 years of suffering from acne I've been on accutane since September (now my last month) and it was the best decision I ever made, to get referred. I've never felt so confident in my own skin.
I took Roacutane a few years ago. I remember myself crying a lot in the streets while walking my dog. That's how depressed I got BUT I knew that it was fictitious. And I preferred that fictitious depression over the real one that came from looking at myself in the mirror with all those horrible and painful spots and foruncules. The pain in my back and joints was also awful. But the spots disappeared almost immediately. If I could turn back time, I would have urged my dermatologist to prescribe it to me YEARS BEFORE. My skin looks wonderful today.
I'm about to leave to go to the dermatologist right now. I was on Roaccutane when I was 17 and my skin was excellent, but recently my skin has broken out again. I just want my skin to look good.

Thank you for this video! I've suffered with acne since my early 20s, it's come and go with medication and the pill but I find the route of 'treating' acne that GPs go down is hopeless. I've had antibiotics which haven't worked and further referred to a NHS dermatologist where the only thing they could do was to prescribe Roaccutane. The side affects scared me and I didn't think would be a good match for myself so I never went ahead with trying it. I'm still suffering with acne now at almost 29, I've tried dairyfree diet, new skincare etc. it's such a pain in the backside so I may revisit the idea of Roaccutane! Thanks again!
I've taken two courses of high dose of roaccurate. The first course lasted about 6 months. My skin cleared up within the first couple of months. I felt very hopeful it would stay clear but once I had stopped taking the treatment the spots came back. I went back for a consultation and decided to have another six months prescriptions. As before my skin became breakout free even quicker than last time. I got told that in 80% of patients the acne doesn't come back. Since I'd stopped taking the second course of the roaccutane my skin has improved, I only get hormonal type spots or spots that come as a result of trying a different (new and not suitable product). The downside is that as a result of taking this harsh, but effective treatment my skin got a lot thinner, drier and prone to cuts just from a small scratch, particularly on my hands. This hasn't changed much since I stopped taking it 5 years ago. In relation to the psychological effects, I did not suffer from depression. However, since taking the treatment I became socially anxious. It is still a problem I am dealing with until now. Overall, the effects roaccutane had on my skin were remarkable, however, the side effects are just as significant.

I've recently finished my (6 months ish) course of roacutaine. It took me months and months of going through my GP to finally get seen by a dermatologist after just been given antibiotics and some face washes and creams which did not work. Thankfully the only side effects of the drugs was dry lips and skin which is normal. Having acne has seriously ruined my teenage years and has been making my life a missed, but at least I had finally been prescribed with a drug which would help. My acne on my face had completely gone and although I was left with a few scars, they have no mostly faded. However I still have inflation on my chest and back and I am receiving a course of injections to make them go down. I have also been given a skin camouflage which is like a concealer and foundation and setting power to cover the scars and redness.

Thank you for this video Caroline! Always so helpful when you do these kind of videos! My acne was really severe and required accutane and it was absolutely fantastic, however I still do have acne but can remain completely clear by following Caroline's advice. I went to Caroline saying that I thought I needed to take accutane again but she got me on the emma hardie balm, glycolic acid and clarions oil and it keeps me clear. And while I would say that accutane is a fantastic drug, it's also hell to put your body through so bare that in mind. (sorry for rambling in your comments section caroline)

i've had acne since I was 12, I'm 21 now and it's better than it was 10 years ago, but i've had no real relief. I'm on doxycycline (have been for about a month) and seeing no help, so it came as no surprise when you mentioned how going on an antibiotic doesn't work. I want to go on accutane but I'm on my parents insurance and they just won't let me do it and paying for it out of pocket would be the end of my tuition savings. Feeling trapped in my own skin, I'm spending so much time and money on my makeup just trying to look presentable when I go outside. It's embarrassing, all my friends at my age don't have this problem and I feel pretty isolated. i actually feel uncomfortable when i go to sleep and wake up with my boyfriend because i don't feel pretty. I need some help!
My acne is due to hormonal issues and stress. Nothing works. I've taken birth control, oral pills and topicals and I am too afraid to take accutane. I've got anxiety and a few years ago I think I had depression for a month or two but I wasn't diagnosed with it. The doctor simply said I just had anxiety, but it didn't feel like JUST anxiety and I'm afraid that if I take accutane that feeling will come back. I'm stuck and I don't know what to do. Obviously I should visit another dermatologist, but I feel as though they don't really listen.
I had a history of depression and anxiety and like you nothing was working for my skin, 80% of my skin was covered in acne! My sister had already gone through the treatment and not realising that I was already depressed I went through with it. Although the medication sustained my depression, I don't regret any of it! The only thing I regret is not admitting to anyone how I was feeling and just being strong in my little corner getting through it. I let my dermatologist put me on an extremely high dosage, I felt like it was worth it and I was scared she would want me to stop the treatment. I had perfect skin for 1 year and now it's come back, but not as severely. I'm trying new things, taking vitamin A and the pill that has been effective on a lot of other people. However, if it were to not work out, I know I will get back on roaccutane no matter the risk of it. Except this time, I will communicate and I will go on a low dosis. My advice is to go consult with your dermatologist and get on the medication starting with a very low dosis (10-20mg/day not more!!) and if you feel you are strong enough and it is going well then you can always step it up :)
My husband has always suffered from depression and anxiety but he had bad acne and tried everything and he tried accutane and he loved it. It's a little rough when you are going through it bc it is strong and your skin gets super dry and stuff. That being said it didn't exacerbate his depression at all. I think if you really have bad acne and tried everything you should go for it.
Have you tried the Retin-A cream? You use it 3 times a week and it gives you the effects of Accutane but as a topical treatment. Side effects are mainly peeling/drying skin
Shpresa Sulo Just a tip... Maybe try changing your diet. The Hormone Cure by Sara Gottfried is a good one if you want to balance your hormones with diet and lifestyle changes.
I'm on roaccutane now, The way the sort the dosage is based upon how sensitive you have been to other meds, if you allergic to anything and weight. I'm on 60 mg a day, but i'm a chubby girl. You have to go for monthly visits to your doctor, so you can easily discuss side effects and such. I have had acne since i was 5, so for me going onto it was a no brainer. Apart from the dryness, not had any side effects. Talk toa doctor you are happy with and make an informed decision.
I'm waiting for the day that there's an accutane (roaccutane) "light" that people with milder acne can take to just freaking knock it out once and for all, i know for me personally my skin isn't "that bad" but it realllllly bothers me and it is persistent...dermatologists have always brushed me off because its not disfiguring enough i guess? They don't understand that it really is super subjective

Hi Caroline, I'm not sure if the formulation of Accutane varies in the UK or the EU but in America, one has to sign up for the iPledge program (a pregnancy prevention program) when prescribed Accutane due to severe risk of birth defects in women that may be pregnant while taking this drug. The point isn't that one doesn't get the drug they need, as an acne sufferer I completely support getting treatment, but I do think that understanding this risk is as important as the mental health risks you share in this video. I wanted to point in case the UK did a better job managing public health than the US, where this information is only shared when someone is prescribed Accutane and made to sign up for iPledge. Under this program, any woman of childbearing potential is required to show proof of contraception, or abstinence before they can get this medication. They also make you take a pregnancy test every month, and only give you one month's supply at a time.

poisonedchalice Hi! I've had scaring on my face after acne, too. Microdermabrasion plus deep acidic peels really helped me with the problem. But find a good dermatologist-cosmetologist who knows what she's doing. Good luck!
I took almost 3 courses of aaccutane. I had acne from the age of 15 i guess when i was in 9th grade and now i am 23. I went to many dermatologist. they gave me anti biotics, ointments etc but nothing worked. Then one of the doctors gave me accutane. it worked miracles. but after my first course my acne showed up again after 2 months. i went to my doctor and he told me to take it again. i took again when i left i started breaking out again. and then i had to take it again. my skin gets flawless when i take accutane. i love it. it makes me so happy and confident. but now i'm hopeless as when i stop taking accutane my acne returns. now i am married and i dont wana use accutane as it has serious side effects. my acne has returned again. its so bad.. my forehead is full of big and small pimples and i get breakouts near n inside my eyesbrows as well as my skull. more near my hairline. i am so depressed. there's no way out. i dont wana go for accutane again though it makes my skin good but it will damage me from inside. my skin looks so so bad. acne irritates me. dont now when my skin will actually be good naturally so i can get my confidence back. helppp! :(
I've had acne since I was 8/9 years old and I am now sixteen. I have had all the antibiotics prescribed to me; oxytetracycline, dual gel and I have previously been on roaccutane. But my acne has still come back on my face. Roaccutane did clear up my back and chest acne, but after about a month the acne on my face came back. I am sick and tired of having horrible skin now and have to deal with the scars that will come in the future. What should I do now? Should I go back on Roaccutane or try somethig different?
My son had years of useless antibiotics he had acne from teenager until he took accutane a year ago waited on list to see dermatologist on NHS finally was prescribed it. Had lots of side effects most minor like severe dry skin. 6 months later his skin is clear he has scars but small price to pay for his nice skin now. But it was a major decision and it is a scary drug wish he had done it earlier.

Hi Caroline, I was wondering if you could help me out and give me some tips on some skincare and products. I'm 16 and have a skincare routine where I exfoliate twice every week and use a skin cleansing brush every other wash when I cleanse my skin. I get bad acne on my chin, forehead and cheeks and also have hyper-pigmentation down my nose and on my cheeks. Would it be possible to recommend some skincare products for treating and calming hyper-pigmentation and acne scarring please. I have very oily and sensitive skin and haven't found any products that help reduce the oiliness of my skin that don't end up irritating it as well so your help there would be greatly appreciated too. I Once went to the doctors about a mole on my face. Came out with a prescription of acne gel! I do not have acne what so ever, I was not impressed with the products they gave me either. I can not remember the name of the prescription gel but basically i was harsh for my skin then they suggested using a Aveeno body lotion to stop the drying of the skin caused by the prescription gel they gave me!!!! WHATTTT!!!! safe to say, i didnt use it! I only had a couple of pimples on my forehead! xx
I went experiences a sudden onset of extremely cystic acne 2-3 weeks after having the mirena coil fitted (I am childless) and I tried treating the skin myself with all the obvious choices, I used more natural approaches, I used Nicam, I constantly cleaned the area with the help of my poor boyfriend and eventually gave in and went to the doctor which in my heart I knew would not give a toss. I was prescribed 6 months of Tetralysal. I don't believe in taking antibiotics for that long. Should I pack it in and go to a dermatologist? is this going to be very pricey?
Hi Caroline, I was wondering if yourself and Dr Sam will be covering Seborrheic dermatitis? I've had it for 6 years around my nose and chin although no one can tell me for sure that's what it is (also looks like perioral dermatitis). I've seen doctors and a dermatologist about it who've just taken a quick look before prescribing creams, lotions and antibiotics which never work. I'm beyond frustrated so any tips or information on how to control it or improve it would be massively appreciated. Thank you.

HI. I'm severely depressed in general. I'm working on that. My skin is bad and nothing seems to work. Not HORRIBLE, but bad. It's frustrating, but I'm not depressed because of it (clinical depression). Short of accutane (which I think might be dangerous for me), what step should I take next for skin? Please help!
I started uni with severe rosacea and cystic acne, and was absolutely miserable. My GP immediately tried to get me on birth control, but I was not down for that so i went to a dermatologist (free in Canada), and went on antibiotics for sooo many months. It did absolutely nothing for me but make me miserable because I was popping 4-5 pills daily. It was the right medical decision because I had horrible acne on my back (the scarring doesnt look like it will ever go away even now) and face, but after so many months I stopped completely and cut out all these ridiculous acne solutions. I'm now using oils and gentle products which really did wonders after years of being extemely abrasive to my skin. Alas I have yet to fix my acne but it is so much more tolerable and accepting and I don't spend hours obsessing in front of the mirror.With all that said, video on scarring while still dealing with active acne??? That is my biggest annoyance is I want to start treating the scarring, but I feel the need to be consistent with treating the spots!
First step to reduce skin inflammation is to change what you eat, many foods cause inflammation that manifest itself in acne, rosacea, spots, bumps. And of course treatment creams or/and medicines. Following anti-inflammatory diet is the way to go. It helped me a lot with my rosacea and spots.
Avoid coffee, bad carbs, white bread and pasta, milk and dairy, sugar, refined sugars. Eat lots od berries, melon, pears, apples, vegetables i.e. greens like avocado, spinach, kale, quinoa, chickpeas, beans, turmeric. Onion, garlic, broccoli, asparagus, sockeye salmon, green and white tea, water, cocnut and olive oils. Of cousre proper creams and maybe vpeven oral medicines will help but inflammation and spots start inside our bodies and we need to start fighting it from within not only rely on drugs. I got rid of zits, spots and rosacea by changing eating habits. Before creams only were helping temporarily because you cannot rely on them forever.

Dr Weil's anti-inflammatory diet and pyramid www.drweil.com/drw/u/ART02012/anti-inflammatory-diet Of course creams and other prescriptions drugs are in many cases helpful (but read about possible side effects of oral medicines) but it all starts from within.
These products are quite good- Tea tree oil, Alpha hydroxy acids, Azelaic Acid, Omega-3 Fatty Acids, Vitamin A, Zinc, Calendula, Topical benzoyl peroxide. "Apple cider vinegar. Considered a “natural acne cure,” apple cider vinegar has natural antibiotic properties that kill off acne-causing bacteria. It also helps you balance the skin’s pH level, which can be particularly important after cleansing. Mix one part natural, organic apple cider vinegar with two parts distilled water to create your own natural toner, and apply after every washing. For a cool, refreshing mist, store in the refrigerator." Try natural apple cider vinegar with the 'mother' like Bragg. You can add it to salads etc.
I just got Avene TriAcneal and I was wondering if I should just use it where I have acne scarring or all over my face. Does anyone know? How does retinal work? Thank you! Help is much appreciated
+Lucía Kmaid use all over, avoiding around the eyes and lips. Retinoids stimulate collagen production, unblock pores and reduce pigmentation. They do everything, basically!
Does anyone have any advice on serums/treatments that are good for getting rid of acne scarring? Fingers crossed I have managed to get my skin under control after having acne for around 2 years on my forehead, chin and round my nose but its left quite bad scarring. Really don't want to have to wear makeup to cover it up on holiday!
+gailybab 93 prescription Retin-A might help a bit with superficial scarring after a long period, like 6 months plus. But scarring typically needs a procedural approach, with laser or micro-needling.
Caroline Hirons No, it didn't. I stuck with it for 6 months - after 4 I saw quite an improvement, but I also saw that the side effects were having an extremely negative effect on my life. I was basically acting like an a**hole all day every day. Not cute. I had to quit it for that reason. Once I came off it, my spots returned immediately.
Yeah well feeling like a normal person is so much more important. Just stick to basic skincare with cleanser, toner and moisturiser (non-perfumed always) but do keep trying different ones if nothing is working. For moisturiser I'm now just sticking to Simple because I just don't think there are any other good, cheap, non-perfumed ones out there
Caroline Hirons I was on roaccutane for a whole year, almost the strongest dose as well, and it worked wonders. I didn't have psychological side effects but the dryness of every ounce of your body inside and out was harsh (lips and eyes esp). But it worked, my treatment has been over for about a year and a half now, I still get breakouts (hormonal) but that's it. I just stick to a basic routine cleanser, toner, moisturizer. Works for me
In the UK gp's are the ones giving antibiotics? That is what our dermatologists do in the US, I swear until you smack them upside the head they just switch you from brand to brand of antibiotics and look confused as to why it isn't working. GP's are just useless for skin issues, dermatologists only slightly less so, at least from my experience.
I love the fact that both of you acknowledge that skin issues can impact your earning potential. As difficult it is to accept, Acne affects every aspect of a patients life.

I've been suffering with adult cystic acne for about six years now and it's so often not seen as a medical condition to be taken seriously. There is so much misinformation and misunderstanding out there, and getting proper help for it is so elusive. In my experience, it absolutely has an enormous impact on quality of life and mental health. This impact is so often trivialized or just plain ignored, and I'm so so helped just by hearing you discuss acne with a doctor who takes it as seriously as those of us who suffer with it do. I took accutane at aged 38 after many years of acne and treatments that didn't work and it honestly saved my life. Yes there were some side affects but the results far outweighed these. I am now 42 and my skin is usually pretty clear and skin is much less oily and I now concentrate on trying to repair the scarring which is an ongoing thing. I spent thousands of pounds on treatments that didn't work and this really was a last resort and something I thought very hard about doing and took very seriously whilst on the medication! Just happy to be able to be confident again and enjoy wearing make up!
I've never struggled with acne but I worry that it will hit me someday and I'll panic. You were both so comforting during this video. I imagine those suffering from acne took solace in your cozy voices :)

I'm so glad your doing videos like this. I'm so overwhelmed when watching this and have surprised myself by crying all the way through. I am 24 and have had spots/acne since I was 11. I have been prescribed everything possible from a GP and have had 3 referrals to dermatology which has resulted in roacutane every time. It had given me so awful side effects (mental health) but has worked but after 6 months my spots always come back. I'm so at a loss about what to do. My face is so painful and I have honestly tried so many different things. I am so sad and don't know what to do. Thanks to you the quality of my skin around the spots and scarring has improved with your help and recommendations so thank you. I don't know what to do though and my self esteem is so low. It's been 13 years of this and I don't know if it will ever change. Sorry to vent my feelings but I really don't have anywhere else to say these things. Nobody understands. Anyway. I was prescribed accutane (decutan) 6 years ago. The process wasn't easy as it had side effects on my skin in terms of redness and itchiness. Although I'm left with some scarring, it was one of the best decisions I've ever taken as it got me my confidence back. Looking forward to more videos with Dr. Sam. Thanks Caroline x

Iv been on roaccutane 3 times. Im now in my early 40's, Male, and was still getting the odd big spot on my face and back, shoulders. I clean my skin, look after it (to the best that I know how). However, recently for Lent I decided to drink 2 litres of Water every day (plain Water, nothing added into it) and I have to say iv had hardly any spots. My skin is clearer, toned, smoother, more plump. People forget the simple things. Sometimes I find myself drinking more... I will add, I don't drink alcohol very often (2/3 times per month I'll have a couple), and don't have much sugar in my diet... H2O - Drink it people!
Interesting to hear about retinoids, looking forward to your video on scarring/hyperpigmentation as someone who has only just got my acne under control after about 8 years of trying everything under the sun (and I'm only 21!). Caroline, your advice has been invaluable and I really wish that in the UK we embraced dermatologists, would have saved me a lot of time and money in the long run I suspect. This couldn't have come at a better time...I am currently looking at going into he cosmetic dermatology industry so this was very interesting and helpful! Acne was definitely something that was brushed aside by my GP growing up and not taken seriously. I'm hoping this is set to change in the future! x
I am currently 1 month into my roaccutane treatment and my skin has improved dramatically. I never thought my acne was that bad, I have cystic acne and only have 2 or 3 at a time, but they are so painful and leave awful scars and that holds you back. I always felt like I would be a nuisance to a GP, like it wasn't a proper medical issue, it's great to know that there are doctors that do understand and will take time to listen. It took me about 4 years to get my treatment jumping through hoops trying different antibiotics WHICH DONT SOLVE THE ROOT OF THE PROBLEM!!! I am praying it's worth it but so far so good!

I'm so excited for your Dr. Sam Series! The skincare industry is always coming out with new lotions and potions and it's difficult to keep up or know what is mainly marketing hype with little else to back up their claims or what is really revolutionary and has amazing efficacy.
I would also love to know what high-end, drugstore and/or other skincare products that are her favorites for skin that usually behaves with occasional breakouts &/or mild rosacea. It seems that the older I get the breakouts and rosacea get increasingly worse.
If possible to determine such a generic question, what high-end products are worth the investment or what drugstore products will give similar results without the expense. There are so many theories and perspectives on whether to splurge on anything that penetrates the surface but not to spend on surface-only products. Some favorite lines from both camps would be helpful.

Early last year I went to see a dermatologist (after using rubbish antibiotics for months!) and the decision was made that I would go on Roaccutane for 6 months. I finished the course in September and since then all of the acne has gone, and the redness on my cheeks has improved hugely, but I now have acne scars on both cheeks. I'm excited to watch the more detailed videos and see if you cover acne scarring! I'm only 18 and I really don't want the scarring to be permanent (it's still pink/red) but I'm scared to put something on my skin that could make it even worse.

I had acne from age 13 to 31. I had low self esteem from it and it affected every aspect of my life. It got very bad when I hit thirty. Up until then, I went to over ten different dermatologists begging for help. All I was ever given was antibiotics and harsh topical treatments that didn't do anything for my acne. When I turned 31, I finally told the last dermatologist to put me on accutane. My skin cleared within two weeks of taking accutane, and I took it for a total of three months. I'm 37 now and still have clear skin. I do find that diet affects my skin, though. Foods like dairy, yeast, and packaged foods make me break out, so I stay away from them. BTW, I was the happiest when I was on accutane because my skin looked so good. I was depressed before accutane when I had no control over my acne.

I've never had acne all over, but chin breakouts were driving me nuts for months. I finally changed my diet and took milk out of the picture. My skin has improved and I haven't had a major breakout since. But I really sympathize with people who struggle with it for life.

I see how much my friend would beat on herself just because of that and it pained me. It also frustrated me that when I did see dermatologists, they didn't seem to really care or take it seriously. Hello Caroline. I really like your channel and i think that you are very honest with your reviews. However I believe there is a misconception that when doctors prescribe antibiotics for acne is because of bacterial infection when in reality is because certain antibiotics have anti inflammatory properties, and I think dr Sam was too polite to mention this. By the way I am a medical student and skin enthusiastic. Thanks for all your hard work and honest reviews.

I've had a horrible experience talking to a GP about my acne/rosacea, I was told I couldn't possibly have rosacea because I'd have a big nose(!!!!!!!!) they sent me away with antibiotics and a gel that burnt my face. Your blog has changed my skin so much but I do still struggle so any extra info. Like this is much appreciated 😘

This is so great, thank you. I'm nearly twenty and have struggled with acne (on my face but mostly my back and thighs) since I was 13. I never thought it was worth seeing a doctor or dermatologist about as I thought it was just something I had to "deal with" as a teen, so it's so good to see you talk about it like it matters. It really does have an effect on self esteem. I've been on Oxytetracycline pills since I was about 13 and do wonder if they actually do anything at all.
Do you have tips on what skincare products are best to use on acne on the body? So many products focus on the face, but a little (and a lot of money) doesn't go a long way on the shoulders, back and thighs. I hate having to cover up my body just because I'm ashamed of my skin.

Omg oxytetracycline did nothing for me! It was a waste of my time too.
I have to agree, i've tried multiple tetracyclines and theyve done nothing but make me extremely sensitive to the sun and empty my wallet. I would love recommendations for body products! I do find it effective to be vigilant about using a loofa and really clearing the area when you shower, and not to use soap heavy products (just like on our faces). I like body products that have salicylic acid, and to moisturize with things that do not clog pores (often times body products are cheaper, and have cheaper ingredients, so stay away from things containing shea butter or mineral oil/ petroleum). I totally know what you are going through, but keep your head up! xx
I think as society we need to change this whole thing about teens just getting on with having acne. Yet if you get to 21 the same thing becomes "adult acne" and becomes a disease that everyone wants to treat. X
My boyfriend and I are highschool sweethearts. In early highschool his acne was extremely bad that he needed medication, I can't remember which, because it physically hurt him as well as mentally. He couldn't lay on his side in bed because it hurt that badly. He was one of my good friends at that point and he had to go for weekly blood tests to check his cholesterol and all sorts. It cleared up beautifully! It did scar quite bad, and it left him with some lumps on his jawline but they're just victory marks that he can embrace.

After over 10 years of suffering from acne I've been on accutane since September (now my last month) and it was the best decision I ever made, to get referred. I've never felt so confident in my own skin.
I took Roacutane a few years ago. I remember myself crying a lot in the streets while walking my dog. That's how depressed I got BUT I knew that it was fictitious. And I preferred that fictitious depression over the real one that came from looking at myself in the mirror with all those horrible and painful spots and foruncules. The pain in my back and joints was also awful. But the spots disappeared almost immediately. If I could turn back time, I would have urged my dermatologist to prescribe it to me YEARS BEFORE. My skin looks wonderful today.
I'm about to leave to go to the dermatologist right now. I was on Roaccutane when I was 17 and my skin was excellent, but recently my skin has broken out again. I just want my skin to look good.

Thank you for this video! I've suffered with acne since my early 20s, it's come and go with medication and the pill but I find the route of 'treating' acne that GPs go down is hopeless. I've had antibiotics which haven't worked and further referred to a NHS dermatologist where the only thing they could do was to prescribe Roaccutane. The side affects scared me and I didn't think would be a good match for myself so I never went ahead with trying it. I'm still suffering with acne now at almost 29, I've tried dairyfree diet, new skincare etc. it's such a pain in the backside so I may revisit the idea of Roaccutane! Thanks again!

I've taken two courses of high dose of roaccurate. The first course lasted about 6 months. My skin cleared up within the first couple of months. I felt very hopeful it would stay clear but once I had stopped taking the treatment the spots came back. I went back for a consultation and decided to have another six months prescriptions. As before my skin became breakout free even quicker than last time. I got told that in 80% of patients the acne doesn't come back. Since I'd stopped taking the second course of the roaccutane my skin has improved, I only get hormonal type spots or spots that come as a result of trying a different (new and not suitable product). The downside is that as a result of taking this harsh, but effective treatment my skin got a lot thinner, drier and prone to cuts just from a small scratch, particularly on my hands. This hasn't changed much since I stopped taking it 5 years ago. In relation to the psychological effects, I did not suffer from depression. However, since taking the treatment I became socially anxious. It is still a problem I am dealing with until now. Overall, the effects roaccutane had on my skin were remarkable, however, the side effects are just as significant.

I've recently finished my (6 months ish) course of roacutaine. It took me months and months of going through my GP to finally get seen by a dermatologist after just been given antibiotics and some face washes and creams which did not work. Thankfully the only side effects of the drugs was dry lips and skin which is normal. Having acne has seriously ruined my teenage years and has been making my life a missed, but at least I had finally been prescribed with a drug which would help. My acne on my face had completely gone and although I was left with a few scars, they have no mostly faded. However I still have inflation on my chest and back and I am receiving a course of injections to make them go down. I have also been given a skin camouflage which is like a concealer and foundation and setting power to cover the scars and redness.

My acne was really severe and required accutane and it was absolutely fantastic, however I still do have acne but can remain completely clear by following Caroline's advice. I went to Caroline saying that I thought I needed to take accutane again but she got me on the emma hardie balm, glycolic acid and clarions oil and it keeps me clear.
And while I would say that accutane is a fantastic drug, it's also hell to put your body through so bare that in mind.

i've had acne since I was 12, I'm 21 now and it's better than it was 10 years ago, but i've had no real relief. I'm on doxycycline (have been for about a month) and seeing no help, so it came as no surprise when you mentioned how going on an antibiotic doesn't work. I want to go on accutane but I'm on my parents insurance and they just won't let me do it and paying for it out of pocket would be the end of my tuition savings. Feeling trapped in my own skin, I'm spending so much time and money on my makeup just trying to look presentable when I go outside. It's embarrassing, all my friends at my age don't have this problem and I feel pretty isolated. i actually feel uncomfortable when i go to sleep and wake up with my boyfriend because i don't feel pretty. I need some help!

My acne is due to hormonal issues and stress. Nothing works. I've taken birth control, oral pills and topicals and I am too afraid to take accutane. I've got anxiety and a few years ago I think I had depression for a month or two but I wasn't diagnosed with it. The doctor simply said I just had anxiety, but it didn't feel like JUST anxiety and I'm afraid that if I take accutane that feeling will come back. I'm stuck and I don't know what to do. Obviously I should visit another dermatologist, but I feel as though they don't really listen.
I had a history of depression and anxiety and like you nothing was working for my skin, 80% of my skin was covered in acne! My sister had already gone through the treatment and not realising that I was already depressed I went through with it. Although the medication sustained my depression, I don't regret any of it! The only thing I regret is not admitting to anyone how I was feeling and just being strong in my little corner getting through it. I let my dermatologist put me on an extremely high dosage, I felt like it was worth it and I was scared she would want me to stop the treatment.
I had perfect skin for 1 year and now it's come back, but not as severely. I'm trying new things, taking vitamin A and the pill that has been effective on a lot of other people. However, if it were to not work out, I know I will get back on roaccutane no matter the risk of it. Except this time, I will communicate and I will go on a low dosis.
My advice is to go consult with your dermatologist and get on the medication starting with a very low dosis (10-20mg/day not more!!) and if you feel you are strong enough and it is going well then you can always step it up :)
My husband has always suffered from depression and anxiety but he had bad acne and tried everything and he tried accutane and he loved it. It's a little rough when you are going through it bc it is strong and your skin gets super dry and stuff. That being said it didn't exacerbate his depression at all. I think if you really have bad acne and tried everything you should go for it.
Have you tried the Retin-A cream? You use it 3 times a week and it gives you the effects of Accutane but as a topical treatment. Side effects are mainly peeling/drying skin
Shpresa Sulo Just a tip... Maybe try changing your diet. The Hormone Cure by Sara Gottfried is a good one if you want to balance your hormones with diet and lifestyle changes.
I'm on roaccutane now, The way the sort the dosage is based upon how sensitive you have been to other meds, if you allergic to anything and weight. I'm on 60 mg a day, but i'm a chubby girl. You have to go for monthly visits to your doctor, so you can easily discuss side effects and such. I have had acne since i was 5, so for me going onto it was a no brainer. Apart from the dryness, not had any side effects. Talk toa doctor you are happy with and make an informed decision.
I'm waiting for the day that there's an accutane (roaccutane) "light" that people with milder acne can take to just freaking knock it out once and for all, i know for me personally my skin isn't "that bad" but it realllllly bothers me and it is persistent...dermatologists have always brushed me off because its not disfiguring enough i guess? They don't understand that it really is super subjective

I'm not sure if the formulation of Accutane varies in the UK or the EU but in America, one has to sign up for the iPledge program (a pregnancy prevention program) when prescribed Accutane due to severe risk of birth defects in women that may be pregnant while taking this drug. The point isn't that one doesn't get the drug they need, as an acne sufferer I completely support getting treatment, but I do think that understanding this risk is as important as the mental health risks you share in this video. I wanted to point in case the UK did a better job managing public health than the US, where this information is only shared when someone is prescribed Accutane and made to sign up for iPledge. Under this program, any woman of childbearing potential is required to show proof of contraception, or abstinence before they can get this medication. They also make you take a pregnancy test every month, and only give you one month's supply at a time.

poisonedchalice Hi! I've had scaring on my face after acne, too. Microdermabrasion plus deep acidic peels really helped me with the problem. But find a good dermatologist-cosmetologist who knows what she's doing. Good luck!

I took almost 3 courses of aaccutane. I had acne from the age of 15 i guess when i was in 9th grade and now i am 23. I went to many dermatologist. they gave me anti biotics, ointments etc but nothing worked. Then one of the doctors gave me accutane. it worked miracles. but after my first course my acne showed up again after 2 months. i went to my doctor and he told me to take it again. i took again when i left i started breaking out again. and then i had to take it again. my skin gets flawless when i take accutane. i love it. it makes me so happy and confident. but now i'm hopeless as when i stop taking accutane my acne returns. now i am married and i dont wana use accutane as it has serious side effects. my acne has returned again. its so bad.. my forehead is full of big and small pimples and i get breakouts near n inside my eyesbrows as well as my skull. more near my hairline. i am so depressed. there's no way out. i dont wana go for accutane again though it makes my skin good but it will damage me from inside. my skin looks so so bad. acne irritates me. dont now when my skin will actually be good naturally so i can get my confidence back. helppp! :(

I've had acne since I was 8/9 years old and I am now sixteen. I have had all the antibiotics prescribed to me; oxytetracycline, dual gel and I have previously been on roaccutane. But my acne has still come back on my face. Roaccutane did clear up my back and chest acne, but after about a month the acne on my face came back. I am sick and tired of having horrible skin now and have to deal with the scars that will come in the future. What should I do now? Should I go back on Roaccutane or try somethig different?
My son had years of useless antibiotics he had acne from teenager until he took accutane a year ago waited on list to see dermatologist on NHS finally was prescribed it. Had lots of side effects most minor like severe dry skin. 6 months later his skin is clear he has scars but small price to pay for his nice skin now. But it was a major decision and it is a scary drug wish he had done it earlier.
Caroline, do you and the Dr have any advice for treating acne scarring. I'm Indian skin and will be 29 in July. I've tried ALL sorts, since my teens, including skin peels (which didn't work) and last year I was on roaccutane via NHS referal to Dermatologist - skin was clear 6 mths after I finished the course...now I'm scarred again where I've had spots:( Please, please help! Hi Caroline, I was wondering if you could help me out and give me some tips on some skincare and products. I'm 16 and have a skincare routine where I exfoliate twice every week and use a skin cleansing brush every other wash when I cleanse my skin. I get bad acne on my chin, forehead and cheeks and also have hyper-pigmentation down my nose and on my cheeks. Would it be possible to recommend some skincare products for treating and calming hyper-pigmentation and acne scarring please. I have very oily and sensitive skin and haven't found any products that help reduce the oiliness of my skin that don't end up irritating it as well so your help there would be greatly appreciated too. I Once went to the doctors about a mole on my face. Came out with a prescription of acne gel! I do not have acne what so ever, I was not impressed with the products they gave me either. I can not remember the name of the prescription gel but basically i was harsh for my skin then they suggested using a Aveeno body lotion to stop the drying of the skin caused by the prescription gel they gave me!!!! WHATTTT!!!!
safe to say, i didnt use it! I only had a couple of pimples on my forehead!

What do you recommend for severe Eczema in a child of 6?
I went experiences a sudden onset of extremely cystic acne 2-3 weeks after having the mirena coil fitted (I am childless) and I tried treating the skin myself with all the obvious choices, I used more natural approaches, I used Nicam, I constantly cleaned the area with the help of my poor boyfriend and eventually gave in and went to the doctor which in my heart I knew would not give a toss. I was prescribed 6 months of Tetralysal. I don't believe in taking antibiotics for that long. Should I pack it in and go to a dermatologist? is this going to be very pricey?

I was wondering if yourself and Dr Sam will be covering Seborrheic dermatitis? I've had it for 6 years around my nose and chin although no one can tell me for sure that's what it is (also looks like perioral dermatitis). I've seen doctors and a dermatologist about it who've just taken a quick look before prescribing creams, lotions and antibiotics which never work. I'm beyond frustrated so any tips or information on how to control it or improve it would be massively appreciated. Thank you.

HI. I'm severely depressed in general. I'm working on that. My skin is bad and nothing seems to work. Not HORRIBLE, but bad. It's frustrating, but I'm not depressed because of it (clinical depression). Short of accutane (which I think might be dangerous for me), what step should I take next for skin? Please help!
I started uni with severe rosacea and cystic acne, and was absolutely miserable. My GP immediately tried to get me on birth control, but I was not down for that so i went to a dermatologist (free in Canada), and went on antibiotics for sooo many months. It did absolutely nothing for me but make me miserable because I was popping 4-5 pills daily. It was the right medical decision because I had horrible acne on my back (the scarring doesnt look like it will ever go away even now) and face, but after so many months I stopped completely and cut out all these ridiculous acne solutions. I'm now using oils and gentle products which really did wonders after years of being extemely abrasive to my skin. Alas I have yet to fix my acne but it is so much more tolerable and accepting and I don't spend hours obsessing in front of the mirror.With all that said, video on scarring while still dealing with active acne??? That is my biggest annoyance is I want to start treating the scarring, but I feel the need to be consistent with treating the spots!

First step to reduce skin inflammation is to change what you eat, many foods cause inflammation that manifest itself in acne, rosacea, spots, bumps. And of course treatment creams or/and medicines. Following anti-inflammatory diet is the way to go. It helped me a lot with my rosacea and spots.
What foods do you avoid to prevent acne?
Avoid coffee, bad carbs, white bread and pasta, milk and dairy, sugar, refined sugars. Eat lots od berries, melon, pears, apples, vegetables i.e. greens like avocado, spinach, kale, quinoa, chickpeas, beans, turmeric. Onion, garlic, broccoli, asparagus, sockeye salmon, green and white tea, water, cocnut and olive oils. Of cousre proper creams and maybe vpeven oral medicines will help but inflammation and spots start inside our bodies and we need to start fighting it from within not only rely on drugs. I got rid of zits, spots and rosacea by changing eating habits. Before creams only were helping temporarily because you cannot rely on them forever.

Of course creams and other prescriptions drugs are in many cases helpful (but read about possible side effects of oral medicines) but it all starts from within.
These products are quite good- Tea tree oil, Alpha hydroxy acids, Azelaic Acid, Omega-3 Fatty Acids, Vitamin A, Zinc, Calendula, Topical benzoyl peroxide.
"Apple cider vinegar. Considered a “natural acne cure,” apple cider vinegar has natural antibiotic properties that kill off acne-causing bacteria. It also helps you balance the skin’s pH level, which can be particularly important after cleansing. Mix one part natural, organic apple cider vinegar with two parts distilled water to create your own natural toner, and apply after every washing. For a cool, refreshing mist, store in the refrigerator." Try natural apple cider vinegar with the 'mother' like Bragg. You can add it to salads etc.
I just got Avene TriAcneal and I was wondering if I should just use it where I have acne scarring or all over my face. Does anyone know? How does retinal work? Thank you! Help is much appreciated
+Lucía Kmaid use all over, avoiding around the eyes and lips. Retinoids stimulate collagen production, unblock pores and reduce pigmentation. They do everything, basically!
Does anyone have any advice on serums/treatments that are good for getting rid of acne scarring? Fingers crossed I have managed to get my skin under control after having acne for around 2 years on my forehead, chin and round my nose but its left quite bad scarring. Really don't want to have to wear makeup to cover it up on holiday!
+gailybab 93 prescription Retin-A might help a bit with superficial scarring after a long period, like 6 months plus. But scarring typically needs a procedural approach, with laser or micro-needling.

Caroline Hirons No, it didn't. I stuck with it for 6 months - after 4 I saw quite an improvement, but I also saw that the side effects were having an extremely negative effect on my life. I was basically acting like an a**hole all day every day. Not cute. I had to quit it for that reason. Once I came off it, my spots returned immediately.
Yeah well feeling like a normal person is so much more important. Just stick to basic skincare with cleanser, toner and moisturiser (non-perfumed always) but do keep trying different ones if nothing is working. For moisturiser I'm now just sticking to Simple because I just don't think there are any other good, cheap, non-perfumed ones out there
Caroline Hirons I was on roaccutane for a whole year, almost the strongest dose as well, and it worked wonders. I didn't have psychological side effects but the dryness of every ounce of your body inside and out was harsh (lips and eyes esp). But it worked, my treatment has been over for about a year and a half now, I still get breakouts (hormonal) but that's it. I just stick to a basic routine cleanser, toner, moisturizer. Works for me

In the UK gp's are the ones giving antibiotics? That is what our dermatologists do in the US, I swear until you smack them upside the head they just switch you from brand to brand of antibiotics and look confused as to why it isn't working. GP's are just useless for skin issues, dermatologists only slightly less so, at least from my experience.
