    Pain is subjective. For some people this procedure would be seen as uncomfortable and others might rate it a 10/10 on the pain scale. If available I highly recommend using a Lidocaine gel to make this procedure easier for your patient. 🙂

      If you think that is painful, try having flexible cystoscopy. The scope is about twice the diameter of the foley.

      I'm having surgery in few days will they put this while I'm sleep and does it hurt when they take it out plz someone answer me

      @@hass7245 they will likely put it in before surgery so you will be awake. Taking it out takes a few seconds most people don't report pain but if it is painful it's very quick.

      That's true. I've broken my arm twice and dislocated my shoulder but it was only a bit sore. However I am the biggest baby when it comes to my back pain and headaches. When I have migraines I'm a sobbing wreck

    I have some advice for those of you preparing to do this. I have done this to myself 5 to 6 times a day for the last 2 years. My catheters do not have the balloon because they do not remain in place. They are used to void my bladder then they are withdrawn. The catheters that I use are typically 14 French, 16" in length and most have coude (curved) tip. For comfortable insertion the complete catheter should be lubricated. Not just the tip. Go slowly and when you reach the prostrate slow down. Continue with slow pressure as the sphincter relaxes. As the catheter goes into the bladder the push gets easier. Telling the patient to relax does not work. Have them exhale as they feel the pressure at the sphincter. Remember go slow. There is no reason this should be painful at all. My first experience with this procedure was in an emergency room and it was a bloody mess. I am an adult male and I was beyond embarrassed. I was mortified! After having it done to me several times it got less embarrassing but it didn't get easier until I started doing it my self. After doing this thousands of times now I believe my experience has given me some insight. For those of you dreading this procedure there is no need to worry. Just prompt your provider to go slowly. Most providers have done this a handful of times and many of them dread it as much as the patient. Remember......Go Slow.

      Thanks for the tips, I also plan to do this to myself but for different reasons; and mine will stay in for at least a month. Looks a bit intimidating at first but seems simple enough.

      Yes sir, you hit the subject matter greatly. This “nurse”, I guess never puts us, the students on the proper side of the patient to visibly see the procedure. I give her teaching skills a C-. My evaluation comes from my teaching skills, NEVER TALK TO THE CHALKBOARD, TALK TO THE PATIENT, PUT THE STUDENTS IN A POSITION TO SEE AND BE ABLE TO FULLY UNDERSTAND THE TAUGHT PROCEDURE! The matter being taught is difficult to understand if the student has to look around or over the teachers shoulders when an open view is missed by the student!

      Agree with geek11 100%. I had the same experience with rehab from a severe bladder infection that put me in the hospital for five days. And the experience with nurse administered Foley catheters vs self-administered single use 14 French coude catheters.....well, there is NO comparison. Nurse administered catheters are too often inserted while the patient is laying down. As I discovered, that is much more difficult and painful. After very unpleasant experiences in the hospital and rehab with Foley catheters, I was SHOCKED to discover how easy self-administration was. And it was all because I was in a standing position. Once early on I had difficulty with self-administration and went to the ER. They insisted on putting in a Foley, and I insisted that they do it while I stood up. The doctor seemed to think that a bit odd, but....whatever. PIECE OF CAKE!! I'm telling all nurses. Whatever catheter you are inserting, if the patient can stand, ENCOURAGE HIM TO STAND. It will be substantially easier for both of you.

    You are an OUTSTANDING teacher. Explaining the terms, demonstrating, breaking everything down. I've had many, many teachers in my life & the ability to non-judgmentally break down knowledge into whatever sized bits are required is a skill and talent. Your students are blessed. I mean it.     When I was in nursing school I was being skill tested for this procedure. The practice patient was one of the older models and had been used a lot. When I started to put the Foley in the penis fell off. Not missing a step I grabbed some tape, tape the penis back on, changed my gloves and preceded with the procedure. I got an A for skill procedure and a bonus B for keeping my composure. My professor kept her cool during the procedure, when it was over she burst out laughing.

    Excellent instruction. I was advised to never reach across sterile field to discard. I was told by urologist, whom I inserted a foley into in his home, that I could insert Foley catheter fully to bifurcation, inflate & then pull back. He advised urethral lengths are variable & foley cannot puncture bladder, but this method eliminates the danger of inflating balloon in urethra. Also, when I use syringe to remove balloon. I remove fluid, then I do it again to completely flatten the balloon, especially larger ones. Jeanne ( retired RN)

    I was self-catheterising at home for some weeks before having a TURP operation due to an enlarged prostate. Obviously not this type of catheter. No balloon.
    Scary at first because of the length of the catheter but becomes routine and pain free. Had to do it three times a day, empty the bladder and then remove the disposable catheter each time.
    The TURP operation was a success. No after effects. 5 years ago, I think.

    Very well done and very professional. Important to keep the patient, especially one that’s the opposite sex, as comfortable as possible, particularly with this procedure. I’m sure most RNs know this and are very sensitive to their patients.

    Great technique. I've been at both ends of catheters and I couldn't imagine having this done to me by a male. My gentlemen patients often look so embarrassed and I feel so sorry for them. If a male nurse is available, I offer to get one. If not, I keep the patient covered as much as possible, for as long as possible and recover as quickly as possible. A patient is a person with a sick body. If we're only caring for the body, we're only caring for half the patient.

    The anaesthetic gel is essential for a virtually pain free insertion, though it stings a bit on application it also acts as a lubricant all the way through so once you are numbed inside there is a little discomfort. Removal is a different sensation and is likely to be done quickly as there is no more lubricant but well worth the discomfort and relief once out of you body. Had it twice in my life so far pre and post operations and the nurses were so proficient at making the procedure as easy as possible.

    There are studies that discourage the use of normal saline to inflate the ballon, and others that say its fine. The kits usually come with sterile water. I recommend using whatever comes with the kit.

      @@happykat1795 go with the smallest standard size for an adult. If the patient is petite you may need to use a catheter intended for a child. If their urine has stuff (e.g. blood) in it you may need to use a bigger size. Hope that helps.

    I have had quite a few of catheters over last 18 months. First one ☝️ nurse caused trauma. But other than that it was okay 👍. I used to change my own bags. Thanks. Great video

    I just went through this before my ablation. The pain I had from the insertion and the removal was the most difficult part of the experience. It hurt just as much to remove it as it was to put it in. The nurse did use Lidocaine, but it really didn't help.

    Where I work (in the UK) we always insert 11ml lidocaine gel (provided in a syringe) and if possible wait a few minutes for it work, much kinder and easier especially on older gentleman with BPH.

      Wow, that might have helped me avoid the most painful experience of my life. I went to the ER with Acute Urinary Retention already in a lot of pain, and the guy who did the catheter said he knew what he was doing and had been doing it for 12 years, but I'm usually OK at bearing pain, but I was yelling and cursing. He just told me to wiggle my toes.
      I had the portable catheter and bag strapped to my leg and the other thing to mention is LEAVE SOME SLACK IN THE TUBE OR YOU ARE TORTURING YOUR PATIENT. When I left I was a wreck and had to drive home. Then the tension on the catheter to my bladder was so great I could barely move to adjust or empty the bag.
      People need training to do this, and oversight, and probably some feedback until they know what they are doing. So far it's the worst pain I have ever experienced, and I had a kidney stone 20 years ago.

    I was taught how to do this on my male resident from his wife, I'd never done one before, I wasn't nervous cause it had to be done right then cause she had her hand bandaged up, it went well, nice and smooth

      When i was about 14/15 years old I had a catheter inserted while I was under anaesthetic. The next day my nurse let me pull it out 😬 it was a strange feeling but it didn’t hurt at all surprisingly. That’s my catheter story, thanks for attending my ted talk.

    One final point,on inserting the catheter and you feel resistance,perhaps there could be a urethral stricture present and would be unwise to ‘plough’ through with the catheter,as this would cause significant trauma to the urethra.Thus resulting in surgical repair to the urethra or perineum urethroplasty.. ..

    I've had this done a half dozen times suggest you ask for liquid pain meds be applied down there. Have Dr squirt IL down there first.

    Really well done. I have couple of questions. 1. As a patient should I be making sure all these sterile protocols are being followed and what questions should I be asking the nurse about specifics around the procedure. Also what if the patient has a urethra l stricture that has closed but is in retention..is their a protocol.I’m asking these questions because so many people have such horrible experiences being catheterized especially in ER.Thanks and well done and helpful to patients.

    I had a enlarged prostate, so they started out with a 14 French could not get past my prostate. Then tried 12 French, same thing. They ended up using a 10 French and they had hard time with that but finally got it through. That was rough and does this happen a lot with enlarged prostates?

    All of my year's of nursing , I never inserted a Foley catheter in a man !! Women yes , men no .
    Don't know how I got out of that ! Lol
    I have taken care of men who had them in and have taken them out , but never put them in.

    This video needs to go further and provide additional info on preparing and wearing the bag , such as , cutting the hose to proper length, strapping to your leg, what about showering,sleeping etc. the big issue is how to reduce discomfort of long term use lasting over several days , im wearing one now and you can feel mild discomfort with sitting on hard surfaces or irritation from physical activity so i put a large square bandage over the entire area above the clamp because the stiff hose would cause irritation on the sensitive areas as i walked about so that nothing moves or rubs against your underwear or pants as for sitting i use a pillow to reduce the area where the hose goes underneath my bottom.. these things need more comments, ideas and suggestion from all sources so please share

    I keep watching this and trying to come to terms with it :( I’m in a psychiatric nursing program, but they’re forcing us to take a semester of medical nursing. I’m honestly not sure if I could ever do it, and will probably have to drop out, which is sad because psych is my love. Thanks for posting this and looking so stress-free about it.

    PLEASE know that seeing urine return during insertion is not a guarantee that it's reached the bladder. I had one inserted too short, and the balloon was expanded while inside my urethra / prostate (possibly the weak area of the membranous urethra.) This caused a lot of trauma and bleeding. LOTS of bleeding for several days. So much blood. Not to mention the excruciating pain for many days. I was sedated during & after the insertion procedure and no one even noticed for some time. If you insert a catheter, PLEASE be sure it is fully inserted and reaches the bladder before inflation. Other instructions state to insert until closer to the "Y" in the tube before inflation, then it can be pulled back out (gently) as needed.

      The syringe comes pre filled with sterile water, we connect it to the port and push in the fluid.

    Im with Lucy. As a 71 year old, Ive already had to endure what I would consider embarassing situations in the hospital. But the outstanding nurses helped ease my mind. I may have to go thru this so its good to know what to look forward to . Growing old aint for sissies.

    Eons ago when I worked in a hospital a urologist taught me how to do this. Well, it was a demonstration with explanation and then I was on my own. Fortunately there was an LPN that mentored me and she came with when I had to do one the first couple times. But you did just as the Dr told me. I like to talk so I chatted up a storm to distract the patient and calm me. When I had my bypass surgery I was told I would have one, which they did in the OR when I was out. Removal was nothing days later, didn’t even notice. Maybe it’s me, the chest tube removal was nothing either. My surgeon was awesome.

      So it sticks/grows into place? I hope they use Teflon if I had to have it, at least coextruded or some coextruded polyolefine.
      In any case a nice and skilled nurse like here is comforting.

      They are supposed to pull it out quickly . .same as a sticky plaster off a wound. Otherwise the patient cant stand to have it done! Painful but harmless!

    For me as quadriplegic it is routine. I do it by myself wich is better because you can slow down if you feel uncomfortable. Its pain free even without lidocaine, but to be honest it is uncomfortable when catheter passing prostate and bladder sphincter. Simply it looks more scary than it really is. Just calm down, don't panic and it will be way less scary experience. Deep breaths and relaxed abdominal muscles make the whole process easier because the bladder sphincter will not be blocked by spasm. As i say its pain free but when it's catheter in bladder you will feel weird... its like you constantly need to pee. :D This is due to irritation of the urinary sphincter. I also went through cystography and cystoscopy. Cystography feels pretty same. Only cystoscopy is more unpleasant but it is done under local anesthesia so again nothing terrible. It's about mindset. If you set yourself to agony... well it will be worse.

    When inserting the catheter you should go further once you have a urine stream before inflating the balloon. Once inflated it can be pulled back slightly to seal. Mine wasn't in quite far enough and caused minor damage where the urethra joins the bladder. Took a few visits to the urologist and insertion of a camera to diagnose, and a couple of months to heal up. Nothing serious, but could have been easily avoided. At least according to the doctor.

    Catheter insertion should be painless if done correctly. I have been catheterized at least 100 times for different reasons and it is no big deal at all. About 10 years ago I was severely injured in a car wreck and since then I have issues with urinary retention. Every so often I develop acute urinary retention requiring being catheterized for a day or two to allow my bladder to return to normal. The first few times I went to the emergency room and I was catheterized and sent home. After seeing a urologist it was determined my best course of treatment was to catheterize myself when retention happens and remove it myself a few days later. I have zero pain on insertion and zero discomfort while it is in or on removal. It is truly not a big deal being catheterized and if you are recovering from major surgery it is nice not having to move around to urinate.

    Pretty good video. I would recommend setting up your sterile field closer to the patient so that you don’t have to turn your back on it. Also, the wrapper from the sterile gloves and other unnecessary things like the plastic bag the kit comes in can make a great place to discard the betadine swabs so long as you are not crossing over the sterile field during discard. You want to avoid getting betadine anywhere it shouldn’t be since it stains like no one’s business. In this video the discard bag would go near the patients right hip. Great job connecting syringe prior to insertion also. Instead of putting the lube in the tray, you can put it directly on the catheter with the catheter remaining in the sterile field, bag still in paper like box and draped catheter into plastic tray then apply lube to not just the tip of catheter but as much as you can go using the whole packet. Also, changing direction of the urethra from straight up to more towards the feet at the point of resistance can help with the maneuver of anatomy. Your betadine technique and explanation was spot on. Also I loved how you put your gloves on as that is exactly how I learned. Just make sure your wrist area or the cuff or the glove doesn’t bump the supplies in the sterile field. If the patient can’t keep still or has involuntary movements, wouldn’t put sterile field on bed between thighs but that’s where I would normally. And if needed, have an extra set of hands to hold patient in place or position tray table at foot of bed so that field is above the lower legs. And no rings with gems please! Lots of bacteria can collect in there.

    So that's how they did it! I was out to it when they put it in,. For a week was wandering around the hospital with a bag of urine attached to my lower leg. Was a bit uncomfortable, but not painful. Having it out was not a problem at all. Interesting. Men are a bit uncomfortable about this sort of thing, and its good to show how easy it is.       Yea.. once out of the sleeve, the catheter is floppy and can touch a dirty field if not carefully handled.. I prefer to leave it in the sleeve until I'm ready to insert it.. then I remove the sleeve and carefully lubricate it before inserting.

    When did they start this new design with the balloon? I have had catheters several times over the years and not once did they ever pump up a balloon or even use a lubricant, they just shoved the thing in and let it go at that.

      There are different types of catheters, not all have balloons. This one is the most commonly used.

    You should put lidocaine on the tube and inside the patient. I keep going into the hospital with blood & blood clots in me, so I get a foley every time I go in. I asked the nurse if I could get lidocaine and it made it so less painful. I don’t know why they’ve never done it before.

    I saw some videos have lidocaine inserted into the urethra beforehand. Is this compulsory or not?. There are some people said the catheter should be inserted until it reaches the bifurcation. Then proceed to inflate the balloon and gently pull it back to confirm its functionality. After that push it slightly back in. Mind sharing your opinion on this?

    What is the use of a lidocaine gel if it does not reach the whole urethra ? What do you think is left of the lidocaine gel on the catheter when the tube has reached about 10 cm of the urethra, namely nothing!

    Well it hurt going in and it hurt being removed. And it is painful while in most of the time. Especially when moving and getting up and laying down. I believe there may be a better method for this purpose now a days. Correct?

      No , it dont hurt taking out since I had it done a week ago and a new one put in . They put it in slowly cuz too fast causes friction and till pee comes out.

      Some times. Usually it's due to the fact that when a new one is inserted not only is the catheter lubricated but the balloon is brand new. When removing the previous one the balloon has been inflated for awhile so it expands in size even when deflated just like a balloon you blow up then release the air out of.

      Males and females can both use these 14French Indwelling catheters, they are not different. The amount of sterile water depends on the manufacturer usually 5-10ml. It says this on the package and the catheter itself.

    I have been self-catherising for over two years now and can't think of ever having an indwelling again. (uncomfortable to wear) Always go for intermittent catheters!

    I have a bladder stone and currently in a waiting list for it’s removal. This is one of my main concerns. Not sure if they will still do the operation if I refuse consent for a catheter. If they will only do the operation if they insert one I’ll be cutting the valve off to drain the balloon and removing it myself as soon as I’m awake. No way am I keeping a three way catheter in.

      Sterile water is usually used. And you always check what the amount was used to inflate the balloon then make sure that is what you remove.
