APTA states that you must have documentation on file when using KX modifier. In case of an audit, if you don't have any documentation (updated plan of care) then how are you supposed to justify medical neccessity?
No additional doc required, but, you better make sure you're justifying physical therapy services in your notes and maybe an ABN at some point. That's my understanding anyway.
it certainly doesn't hurt! With 70% of healthcare fraud being committed by PROVIDERS (per the Association of Certified Fraud Examiners), we are already the target in these investigations. I have seen a horrible lack of supporting documentation by the physical therapists I have covered for in over the last 20 years. It's amazing they are getting paid at all. It doesn't take more than a sentence or two to go beyond the "requirement" which could later on implicate the therapist in not just fraud, but waste and abuse. Better to be safe, than sorry later. Just my .02.
