Sheldon understands right from wrong. What his problem is is that he had no grey center. He's either fully self-aware or completely oblivious. There are multiple examples in the show of Sheldon "knowing where the line is" but he announces then line then proceeds to cross it on purposes. His motivation is usually pure/brutal honesty or selfish. This show is not consistent with his personality type. They make him oblivious or self-aware depending on how they want to sell the joke or move the episode or arc forward. Sheldon is the shows pressure valve much like Spike was in Buffy the Vampire Slayer. He does or says the things that are socially inappropriate because deep down we've all thought it, thought about it before, or wished we could do it. If you're someone who is into character profiling you might find his development kind of disappointing in how they treat him like a high functioning autistic person one day and an emotionless narcissist the next.
