You get lung cancer, and go to get treatment. The insurance covers the initial diagnostics, your doctor recommends a very expensive but very successful treatment. The insurance (what you pay into, along with millions of others) doesn't want to cover it due to cheaper alternatives (regardless of success rate), but also the fact that you're doing something that is CLEARLY degenerative to your health. Insurance only works if more people pay into it than use it. If you have a heart transplant, you're going to have the insurance spending FAR more than you'll pay into it for 20 years. That means others contributions cover your procedures. Insurance doesn't work if you spend $600 a month on insurance but then get $2000 in services.
