Adjacent Tissue Transfer (ATT) does not include intermediate or complex repairs. According to CPT guidelines, ATT (CPT codes 14000-14302) encompasses the transfer or rearrangement of tissue to repair a defect, but it does not include the additional steps required for intermediate or complex repairs.

For example, if you have both intermediate and complex repairs, you would list the complex repair as the primary procedure and use modifier 59 for the intermediate repair.
Please refer your CPT manual page No. 89. The formula is mentioned. This formula takes into account the greatest clinical diameter of the lesion itself and adds "twice" the narrowest margin that is necessary for complete excision. The result gives you the total diameter of the lesion plus the margin, which is used to determine the correct CPT code.
Hi. Can you please confirm where in the guidelines is it mentioned that biopsy codes include simple repair. For example, 11102 has no mention of simple repair. Kindly please clarify. Its a bit confusing. Also thank you for the great videos and lots of effort..❤

The procedure involves removing a sample of the epidermal tissue, sometimes including a portion of the underlying dermis, using a sharp blade If there is any bleeding, it is usually controlled with ointment, a chemical agent, or cauterization. If Simple repair is needed,it is included in the payment for the biopsy and should not be coded separately.
