    I thought W,Z,V-Y plasty is a technique of ATT? In this video you said, if a traumatic wound is closed in z,y,w pattern - do not code ATT? im confused. Please enlighten me!

    Mam,as you said ATT along we code intermediate and complex repair code,but as per guidelines in the question ATT involved we can't code repair code right 👍▶️ plz reply my answer i have exam in this week

      Adjacent Tissue Transfer (ATT) does not include intermediate or complex repairs. According to CPT guidelines, ATT (CPT codes 14000-14302) encompasses the transfer or rearrangement of tissue to repair a defect, but it does not include the additional steps required for intermediate or complex repairs.

    Mam if only complex repair is given in the questions then what modifier we should use mam

      For complex repairs, you typically use modifier 59 when more than one classification of wounds is repaired. This modifier indicates that a distinct procedural service was performed.
      For example, if you have both intermediate and complex repairs, you would list the complex repair as the primary procedure and use modifier 59 for the intermediate repair.

      Please refer your CPT manual page No. 89.
      The formula is mentioned.
      This formula takes into account the greatest clinical diameter of the lesion itself and adds "twice" the narrowest margin that is necessary for complete excision. The result gives you the total diameter of the lesion plus the margin, which is used to determine the correct CPT code.

    Hi. Can you please confirm where in the guidelines is it mentioned that biopsy codes include simple repair. For example, 11102 has no mention of simple repair. Kindly please clarify. Its a bit confusing.

      Thank you very much for your support. In the CPT manual page 85 below title biopsy, you can find the guideline- Simple closure of the defect is included in the service.

      ​@@CodeMedMasteryhi. Thank you so much for your reply. Yes i see mentioned under punch biopsy and i also under incisional biopsy. However no mention of it under tangential biopsy. Or is it mentioned else where and i am not seeing it correctly? Please clarify.

      Tangential biopsies, which include techniques like shave, scoop, saucerize, or curette, typically do not require repair with stitches. The procedure involves removing a sample of the epidermal tissue, sometimes including a portion of the underlying dermis, using a sharp blade
      If there is any bleeding, it is usually controlled with ointment, a chemical agent, or cauterization. If Simple repair is needed,it is included in the payment for the biopsy and should not be coded separately.

      CPT 19120 code is used for the excision of a breast lesion without the use of imaging guidance.
      CPT 19125 code is used for the excision of a breast lesion that requires imaging guidance for localization.
