- What happens if you need a lasik touchup after cataract surgery? Is it even possible? Even though cataract surgery has gotten very modern and precise, there are still times when lasik can improve the results.
A lasik touchup after cataract surgery can be used to correct any residual prescription or astigmatism. A laser treatment can also be used in in some cases to improve reading vision after cataract surgery.

Thank you for this video. Five months post cataract surgery & 1 month after YAG I can still not see well. I saw my surgeon today & although he never used the word Lasik, his explanation of what he thinks our next option will be convinces me that this is where we are going. I have to wait another month as my prescription has not yet stabilized. It's very frustrating when you pay for the premium lenses (Panoptix, in my case) & not only is my far vision subpar but I now need readers.
I had the Vivity IOL then YAG and had the same situation. Two months later I had the Lasik Wavefront and my distance vision is very good now. Computer vison is OK and close up I need readers for fine print. I had starbursts and halos after the Lasik and those have diminished. My ophthalmologist told me that he had Lasik and also had starbursts and halos. He explained that the edges of the cornea are healing and would go away with time. Dry eyes are common afterward but that also improves. I paid for the toric lens and there was no additional charge for the Lasik. I am pleased with my results.
@@robertyoung9589 my wife, who had 20/10 vision before she got cataracts, had the panoptix lens put in, and she hated it.
Distance vision was not as sharp as she had expected, and she still needed readers to some degree.
We waited four months and my wife’s cornea doctor removed the panoptix and put a standard mono vision, lens and for distance and all as well.
We had heard about halos and starbursts, but we were told they were minimal. My wife had never seen anything like that in her life, so she was freaking out and decided to just have the Panoptix removed and all is well.
FYI, no warranty, so $3,500 out the window. 😩

Thanks for the video. I’m in that 1 in 20 who wind up with residual prescription after a multi-focal iol implant and I’ve been told LASIK will take care of the issue. Your video was reassuring!

I have just had my second cataract operation, and I am very pleased at the vast improvement of what was very bad Longsightedness, however I would love to have Lasik to make it so I don'''''''t need glasses anymore. Excellent video, I had cataract surgery on both my eyes about 5+ years ago and I've been having more problems than before. I have a black spot on both eyes that many times I can even see it. Also I have a lot of little pieces of lint that look like thin threads in both eyes. I've come to the conclusion of getting lasik surgery, first on my right eye and see if it makes a difference. If so I will have it on my left eye. I'd sincerely appreciate any response from you Dr.. Thank you very much.
Thanks Dr Eubanks for this informative video, I had cataract surgery 2 years ago and inquired about LASIK to fine turn my vision in my one eye but the doctor was very hesitant to go ahead with lasik for fear that I may be again disappointed or something else? What would make a lasik doctor in the same office group has my cataract doctor be so hesitant about using LASIK to enhance my vision as your video states is possibly? Thanks again

I had multi-focal lens implants in October then had the YAG laser and I’m due to have laser touch up in a few weeks. At this stage I wish I hadn’t started down this road but after spending £7,000 I need to see it through. Fingers crossed this works.

I’m seeing a new ophthalmologist this week and hopefully I’ll be a candidate for YAG procedure. Previous doctor never told me about PCO, I’ve been getting stronger and stronger prescriptions since about a year after cataract surgery. I’m kinda PO’d I had to learn about this on RUclips.
I just had a cataract surgery 2 mos. ago and still using the same prescription glasses before my cataract surgery, I was thinking that my cataract surgery was a failure.!! I asked my doctor if I can have lasik surgery instead since my vision still the same. He said yes, no problem but I have to wait at least 3 mos after my cataract surgery! I had a lasik surgery before like 20 years ago and my vision was perfect until I got older .. and my doctor told me I have a cataract. Please advise!! Thank you so much in advance!!!!
Can you please address Lasik after premium lens replacement? 5 months ago I received a Panoptic lens. Turns out the haloing was severe enough that it was exchanged with a Vivity lens about a month ago. I can read, I can see the computer, but my far vision is terrible. I can see the TV, street signs, or the screen in the conference room. Can Lasik fix that? Can you also talk about benefits of flapless Lasik?

Can you have LAZIK after having LAZIK in 1998, and Cataract surgeries in 2022? I just had the YAG POC laser yesterday due to "scar tissue forming". I never had astigmatism, that I noticed, in my eyes until after the cataract surgery. As for the astigmatism, my ophthalmologist said that he couldn't guarantee where or not I had astigmatism because I had LAZIK back in 1998 and the records for that surgery were not available for that far back from a different surgeon. He said that he could do the special lens for astigmatism but without the records before LAZIK, he'd be guessing. Does that make sense? Are there any other options at this point? For the record, I was a MINUS 16 in my right eye before LAZIK.
The latest and greatest IOLs are B+L Trulign Toric, with the non-dominant eye set slightly myopic to extend their accommodation range. Coupled with a laser to do LRIs to correct residual astigmatic error, this produces nearly perfect 20:15 vision at all distances without halos UNLESS something goes wrong. My question is whether LASIC can be performed after laser LRIs to address problems, and/or maybe for a touchup after decades of aging?
Hi Steve, If needed, lasik can be performed even when the original surgery used LRIs to correct astigmatism.

I have a question that maybe you can weigh in on.
I had cataract surgery 5 years ago & a 2.25D toric lens was used to improve my irregular astigmatism.
I see 20/25 without glasses, but I see ghosting & looking at headlights is rough.
Can LASIK smooth out my irregular astigmatism & match my toric lens & is PRK a better option.
My cornea doctor is happy with my 20/25 results and he is leary to try to improve it, but the ghosting and multiple images and halos at night is driving me nuts.
Where is your practice located?
If within a days drive, maybe I could get a consult.

How about prk laser about 3 months after Turic rotation was done and still have astigmatism and blurriness .. (Since I already had lasik and prk in bother eyes in the past I cannot get lasik correct?)
Can Lasik clear up near vision problems caused by a Clareon Panoptix implant?
Good morning. Thanks for the excellent explanation. 5 weeks ago I had a Synergy Multifocal IOL implanted in both eyes. it was an elective procedure as I didn't have cataract just wanted to discard the need for glasses. The near sigh vision is now good but the mid and far sight is quite blurry. Prior to the surgery I had excellent mid and far vision. Due to this conversation¡can I went to another Md who says eh IOL in left eye had shifted forward causing a -1 myopia. In addition he mentioned that there was still swelling that caused the blurriness. I understand the IOL swiftness can be addressed but I'm actually more concerned with the inflammation. after 5 weeks, inflammation causing the blurriness. Is this normal? Hi Javier. Glad you enjoyed the video. For most people, inflammation goes away after within the first month. However, some people may have inflammation that persists longer than a month and rarely even longer up to 6 months or more. This requires longer courses of steroid medication eye drops to treat.
Thank you Dr. Eubanks for this excellent video. 20 years ago, I had Lasik surgery and have experienced dry eye syndrome ever since, treated with OTC lubricants. Last week, I had a PanOptix IOL (non Toric) inserted in my non-dominant eye. I now have residual astigmatism which my doctor says can be treated with Lasik. I am rather concerned as I've already had Lasik in the past. I am aggressively using preservative free drops along with post surgery eye meds. How concerned should I be given I've already had Lasik? Hi MJ. Glad you enjoyed the video. So often times if one has had lasik in the past, an alternative procedure called PRK is performed which treats the surface of the cornea directly and avoids interacting with the old lasik flap. There are also good prescription eye drops and other treatments to help with dry eye. Ask your doctor about those

Also, I wish I had been told years ago that Lasik can cause dry eyes,and the consequences of not using preservative free eye drops. I appreciate the information you gave me about PRK - it is a relief to know that it's a common procedure for patients with a condition like mine. This is a very helpful video. I am 59 and scheduled for my first cataract surgery on 3/8 with the Vivity lens. I chose it due to the risk of halos and glare with Panoptix. I'm now told that it isn't available in the power needed to correct my vision to near perfect--only to a -4.0 so he's recommending Lasik (Countoura) enhancement 3 months after surgery. Does this sound right? The enhancement is provided at a minimal cost within one year of initial cataract surgery. (And now I'm freaking out about all the recent revelations about Lasik surgery and its long-term negative effects.) I don't know what to do. If I could avoid all of this I would but my vision has deteriorated so much due to the cataracts.
Glad you found the video helpful. I think you may be referring to the FDA's draft guidance on refractive surgery. While I admit I have a certain perspective on lasik since that is a primary procedure I perform, there are issues with how that report was created and the data it was based upon and multiple medical organizations including the American Society of Cataract and Refractive Surgery have asked the FDA to withdraw that draft because of that. But ultimately, you can only do what you are most comfortable with. And if that is a fear, perhaps going with a monofocal or toric lens that can correct your full prescription may be the optimal way for you to correct vision.
I had cataract surgery for high miopía and my doctor advice was to use monofocal lense, I feel grateful now, only use readers but it was life changing, willing to do lasik in the future

I am 45, had IOL replacement, but not all astigmatism was fixed with IOL, so after IOL surgery I had LASIK on the right eye. It definitely fixed residual astigmatism, but side effects are halos, glare and chromatic aberrations after LASIK. Doctor told me, it takes a while for cornea to heal.. so currently waiting....
That is correct. It does take some time to heal up after lasik. It is also important after lasik to use preservative free artificial tears aggressively as everything heals up as dry eye can also be a cause of fluctuation of vision and glare.

I see your post was 5 months ago. I am going to have post-cataract surger Lasik in about one month to resolve astigmatism in my right eye. How are your eyes doing now? Are you still have the same side effects? Thx!
Have monocular vertical diplopia in right eye from ptosis induced astigmatism (in my opinion) and cataracts (to be removed shortly). Will the preliminary exam for the new implanted lenses be able to get the Rx correctly going thru the ptosis induced 'wrinkle' of the astigmatism? Or will that astigmatism go away after ptosis corrective eyelid surgery no longer has the eyelid drooping across the field of vision causing the wrinkle? And if the implanted lenses were compensating for the "temporary" astigmatism that fades away after the ptosis surgery, does that leave me with wrong Rx for the lenses? That gets fixed with the laser?
A droopy eyelid (ptosis) can cause extra astigmatism. If or when the droopy eyelid is corrected, the amount of astigmatism can change. Lasik would be able to correct a change in prescription if the astigmatism changes after cataract surgery

I dont get it Southwest eye inst El paso east said during surgery that cataract surgery was Lasik surgery with cataract removal If patient choose premium lens like a toric lens.. Are you saying although patients pay thousands for correction that they will still perhaps need Lasik surgery? Please explain because why would people pay thousands for toric lens to see then not be able to see right and then have to pay more to get surgery again.. im confused?
Cataract surgery does correct prescription and most people do great afterwards. However, the accuracy of cataract surgery isn't 100%. About 1 out of every 20 patients may be left with some residual prescription and benefit from a lasik touch up procedure to correct that remaining prescription. This applies even when using the premium lenses as well. But frequently a lasik touch up may be included in the price of the premium lenses.
I had detached retina surgery, with silicone oil placed in the eye, subsequently cataract surgery was performed, but post surgery vision is blurry. Will Lasik correct?
Great question. If there is damage to the retina and a pair of glasses can't sharpen things up, than chances are unlikely that lasik will be able to improve things. But if glasses can sharpen things, lasik may work. However, silicone oil can change the prescription of the eye. And so lasik wouldn't be recommended with silicone oil in the eye because when that silicone oil is removed, the prescription will be different.
I have +0.5 of hyperopic residual on my domiannt and 0.25 on my non-dominant. The non-dominant is tolerable, but +0.5 is blurry. Can a refractive error this small be safely fixed with lasik?
+0.50 is usually the minimum that surgeons will consider for operating. Especially if its bothersome and correcting it will allow for a significant improvement.
@@eubanksmd my surgeon suggests a sulcoflex, but i'm not liking my options. 0.5 is just enough to cause diplopia significant enough to affect binocular vision.
What do you mean by 'residual prescription'?
If you end up off target after cataract surgery, then instead of having zero prescription with perfect vision, you may have some nearsightedness or farsightedness blurring your vision. This unplanned extra prescription is 'residual prescription'.
@@eubanksmd Um - sorry, 'off target'? Do you mean the replacement lens being not quite where it should be? Thank you for your reply anyway. Is there anything to be done about it? I live in the UK and was told the most common complaint from patients after cataract surgery is having to wear glasses for reading. (Ha ha - I now need 3 pairs, one for reading, one for computer and one for middle distance, whereas before I had one pair of bifocals for everything). On the NHS you get a monofocal distance replacement lens, and having to wear glasses for reading is expected and routine, apparently. I had one eye done (cataract) almost 4 months ago and am now getting some blurring. Some days worse than others. They are talking about 'fixing it with a laser', but from what I've seen that just means basically shooting a hole through the back of the capsule so that more light can get to the retina. Having read about the possible drawbacks, which nobody tells you about, like lots of floaters, the possibility of detached retina, possibility of the lens falling out of the back of the capsule, the possibility of having to have 'stray bits' lasered again, I'm not sure whether to put up with a little blurring and hope it stabilises, or go for the laser. I get that it's very quick and painless, but nobody mentions the possible drawbacks. It has taken much longer than I expected to recover from the initial cataract and to be honest it still doesn't feel right, and gets sore and tired very quickly. Also my dry eye is ten times worse than it was before. All I was told was 'it'll be fantastic', and although I asked a lot of questions I wasn't prepared for this. Too late to change my mind, and I more or less had to have it done for driving anyway. Sorry for the long post, it's just so difficult to get any info on medical procedures here. Many thanks again.

What about negative disphotopsia after eye surgery?
Unfortunately lasik is unable to treat negative dysphotopsias. It can only treat prescription that is also correctable with glasses.

For most people, dysphotopsias improve through a process called visual neuroadaptation. I talk more about that process in this article: eyemountain.com/training-eyes-after-cataract-surgery/
For persistent negative dysphotopsias, reverse optic capture is one way that it can be improved. Another way is to have a second lens placed in front of the first lens (known as a piggy-back lens)
@@eubanksmd I’m at 18 weeks post cataract surgery with a mono focal lens. I still have negative dysphotopsia and flickering on the temporal side. It has improved a little, I suppose through neuro adaptation, but there is still room for improvement. How long should I wait before exploring one of the solutions you mentioned? Thank you.
