    ACL (anterior cruciate ligament) injuries are all too often sent for surgery when most have a really good chance of healing 100% naturally.
    They can even get back to being active with sports and running if they choose!
    In this video, we cover 3 things to check on your knee for an ACL tear, then we explain all the different types of ACL tears and how to begin to treat an ACL injury naturally.
    A scary statistic you need to be aware of if you’re planning to get an MRI (or if you already have), is that one study reported over 40% of ACL tears found on an MRI can be wrong!
    That means the MRI showed a torn ACL but when the surgeon went in to repair it, there was no tear. It might have healed or it might have never been there...either way the surgery was not needed in those poor patients.
    There are endless ACL rehab exercises out there and nothing special about a single one that will help you most. What is most important is getting the right kind of stability so that your ACL can heal.
    That’s why we recommend getting a hard hinged knee brace on as soon as possible so that you can give your ACL some relief.

    Hi Doc, Thanks for the video it's really helpful in understanding the problem clearly
    I had a knee injury 6 months back and decided to take MRI now as there is a constant pain, and here is my results.
    *Minimal knee joint effusion.*
    *Grade I tear - Posterior horn of medial meniscus and lateral meniscus.*
    *Posterior horn of medial meniscus and lateral meniscus.*
    *Anterior cruciate ligament-Partial Tear.*
    *Posterior cruciate ligament - Strain.*
    I shown these results to an Ortho Doctor and he suggested for Arthroscopy as next step..
    How severe is my injury? and also trying to get a knee brace for myself after seeing this video.
    Once again thanks for the Information and support 🙂

    My husband was clipped on December 25 by a wild snowboarder while skiing. He could barely walk for months with the swelling but now there is less clicking/popping and now just more achy from time to time. Walking for extended periods was hard. I’ve given him glut strengthening exercises and gentle shallow squats. He had a McDavid hinged knee brace in the early months but stopped wearing after a few months as he started feeling much better. He is much better but his MRI at the end of May was read as a completely torn ACL and both meniscal torn and MCL partial tear. But I was the only one as a chiro who did the basic orthopedic tests and the anterior drawer test was NOT positive. However, medial gapping test was hugely unstable. So I was stunned when the results in MRI complete ACL tear were read. Up to 40% are read wrong? I think it is MCL because the goalie squat was painful for him but is not anymore.
    So are you saying he should wear this other brace you recommend? Or just continue with the McDavid brace? All the time? He is much better as the knee only aches from time to time. I’ve been advising him to fix the muscle imbalance and strengthen the gluts. Standing in sand on the beach makes him feel unstable. They recommended orthopedic consult for ACL reconstruction and a custom ACL brace but so far two Orthopedics have turned him down.
    Should we just get this brace to recommend at this point? How long should he wear it for? I talked to him about doing your online knee rehab plan since it would clearly give him a guided program to help him fix his muscle imbalance. He has a history of loving biking and leg press and always had strong quads. That’s why he likely injured a meniscus many years ago. Sadly this was a freak injury on the ski hill.

    I fell out of a trampoline at defy 10 feet up in the air . I looked down and the trampoline was gone and I stopped my fall with my right leg. My leg completely came out of my knee. I was lucky when I wiggled my leg for it to go Back in place. I was in horrible pain. My 2 year-old asked me if I was OK. Never have I heard her ask me that. My heart was broken. My first thought was how am I going to feed her, how am I going to go grocery shopping ,cook , wipe her butt etc.. my life is over. . Anyways, so finally, the pain was much better so I figured I’d get up and walk. Unfortunately my leg completely gave out and I fell to the ground. Instantly I knew my life has completely changed forever. I’m waiting to see my Orthopedic in a few days. In the meantime, I want to see my options. I really enjoyed this video , I very much appreciate you taking the time to make this . According to the ER, they think that my ACL and my PCL is gone. I don’t have any pain with my leg gives out but I feel the bones hitting each other and that feels crazy and I just feel like someone kicked me in the knee but the pain goes away. My foot also turns blue when I am on my crutches, so I remain with my leg up on my wheelchair. Would there be a chance for me to give this a try you think? I cannot put my foot down on the floor or else my leg will give out and he goes sideways not front ways..

      There’s still a possibility your acl is torn even if your leg is improving (something else could’ve caused that pop though) because my acl was fully torn for 10 months and I didn’t notice because after the incident that tore it (I didn’t know it was torn at the time) my leg improved back to the point where I could run full speed, so I’d get an mri if I were you im saying this so incase it’s torn you can catch it early

    Appreciate this informative vid, thanks for your time and sharing your expertise 👍🏽
    I have a recent full ACL tear possible meniscus damage as well. This sounds great at least worth a try and the orthopedists I spoke with did advocate at least for a middle ager a wait and see approach as you mentioned, at least 2 months (which thanks to your vid is likely not enough time to know if it will heal on its own) then consider surgical reconstruction with hamstring tendon. However my concern, there was a study done by HSS in NY, that showed those who did not get ACL reconstructive surgery were much more likely to suffer from osteoarthritis 7 years after injury than those that got the surgery. Is this something you have seen at your practice or are aware of?

    • @epmanualphysicaltherapy
      @epmanualphysicaltherapy  2 года назад +9

      Hey cardo, sorry to hear that you've got an ACL tear :-(
      Yes, we've heard about that study. There are more like it. We have to question what kind of health and strength the people from the study had. We don't know what they did for all those years leading up to getting knee arthritis. The assumption we work under is that they kept doing exercise or activity that fed into a muscle imbalance. Muscle imbalances around the knee (over-dominant quads and under-dominant glutes) is what sets up many chronic knee problems, including knee arthritis. That coupled with the loss of stability from the torn ACL can definitely expedite knee arthritis.
      But what if you correct your muscle imbalance and maintain it?
      That has not been studied. In fact muscle imbalances have not been studied. We see people here in the clinic do very well after an ACL tear. It's up to them to keep up their strength for the long term.

    Hello Dr.
    I sustained an injury from jumping off the top of a car during an accident three years ago. Initially, I visited a hospital where they diagnosed me with a torn ACL and recomended surgery which i didnt take. Despite this, I could bend my knee fully backward without pain, but I couldn't fully stretch it as before.
    Later, i visited another hospital for a second opion and noted that my walking pattern was almost normal despite the injury, which led them to believe that my ACL was not torn based on the mri result of the previous hospital. They advised me to undergo physiotherapy exercises. This incident happened about three years ago.
    Since then, I’ve done some light exercises and have been able to run and perform push exercises, but I still can't fully stretch my leg.
    Last week, I collided with another player while playing football, and it was quite painful. After undergoing an MRI, the hospital confirmed my ACL is torn and suggested surgery. Currently, I can walk without limping as a week has passed since the incident.
    I'm concerned about making the wrong decision regarding surgery. I am not a professional football player; I play for fun. I am currently in a foreign country with better medical facilities. Today, I came across your video, and I am considering not having surgery based on your advice.
    Could you please provide your guidance on this matter?
    Thank you

    I was not referred to getting a knee brace after I went to the hospital for the doctors to check for more than 4 times, and I really appreciate your videos Dr. it’s now been 6 months that I have had an acl grade 2 but I feel it is healing better than ever, and it’s honestly sad that the head orthopaedic doctor from Fiona Stanley hospital today told me I have grade 3 injury and said that I needed surgery without looking at my previous mri scan. Any suggestions…

    Thank you. I'm currently at the point you described at the end of the video, where im rapidly improving, however i feel i should truly exercise my knee as much as possible. I had a ACL surgery 5 years ago, but i have likely injured a ligament in my knee recently. I really dont want to waste another year doing rehab, so this video was truly helpful, now i have a new insight! One question tough, would you recomm3nt wearing atleast a lighter brace when exercising my legs, for exampling when using a rowing machine? I do not feel much discomfort when doing this exercise without it, but feels a lot safer with it.

    Hello doctor! I injured my leg almost 9 months ago in the gym and it turned out to be a partial ACL tear (in the interstitial fibers) and the ACL was not stretched. I did physical therapy for 6 weeks and the swelling and pain got way better. But recently I got a job in which I would be sitting for 8 hours straight and I noticed that my knees started getting swollen, it hurts and it feels like there are needles inside my knee (couldn’t find a way to better describe how it feels)
    My question is, can I get back to the gym with a swollen knee in order to strengthen my quadriceps and my hamstrings? And if yes, are there any exercises at the gym I need to avoid? Thank you

    Thanks for such a wonderful explanation. I met with a bike accident and my acl got tared .Then I underwent ACL reconstruction surgery 6 years back. A month ago I fell down and my current MRI states that " diffused thickening and increased signal in the ACL" - diffuse increased signal intensity secondary to mucoid degeneration. One of the doctor suggested for the surgery again. But another suggested to proceed without surgery. Does this MRI statement meant partial tear in ACL?

    • @epmanualphysicaltherapy
      @epmanualphysicaltherapy  Год назад

      Hey Subha, sorry to hear that you're dealing with this knee ACL problem :-(
      If the MRI report did not explicitly read that there is a tear, even a partial tear, then there is probably not a tear. "Diffuse thickening," "Increased signal," and "mucoid degeneration" are not the same thing as a tear. Those problems are things that could eventually become a tear, but you probably don't have a tear yet.
      This sounds like you do not need surgery and you need to address the root problem so that you don't end up having a tear. You should check out our library of ACL videos if you haven't already. You can see it here:

      You've got to get in a knee brace fast and rest a bit so you can let your ligament and your meniscus heal in your knee. Once you can get your knee swelling down, then easy exercise is next. Thanks for the information, this made me understand alot of things. I had a knee injury 3 weeks ago while playing football and the MRI is saying torn ACL, I was able to walk right away, I got swelling the next day but the situation is getting better since and I don’t feel pain, I did the test and I don’t feel pain! So I assume its minor or partial or stretched I’m not sure.
    The only this is I can’t fully engage my quads when my leg is straight compared to my non-injured leg! And I feel this is weird and I saw that I have to exercise my quads as a way of recovery. I don’t feel pain so hopefully I’m recovering, I’ll get the brace and gradually exercise all my muscles around the knee as I heard this is the advice! Please let me know if there is anything else I can do.

    Very informative video. I recently injured my left knee & got my results from my MRI...complete rupture of my ACL. I'm confused because I'm not having pain & for the most part I've gain most of my mobility not 100% but I'm very close to it (injured 02/09/2022). Is this something that is considered normal for a complete ruptured ACL? Keep in mind right after I injured my knee I iced it, put a brace(still wearing it) & have continued to elevate it.

    • @epmanualphysicaltherapy
      @epmanualphysicaltherapy  2 года назад

      Hey Haydee! Sorry to hear that you've got a complete ACL rupture :-(
      But that is great news that you've got nearly all your motion and no pain!
      That means that you have a chance at either healing the ACL or at least compensating with strength to go on without surgery. This does happen to a lot of people with ACL tears, but usually surgeons push patients right into surgery. Keep going with the brace and start on the recommended exercises. You might even find a local specialist to guide you on healing without surgery.
      The best case scenario is that you recover adequately and get back to normal again. Worse case scenario is that you get strong and put yourself in the best position to have surgery and recover as quickly as possible after you have surgery. There is no rush if you're feeling good and improving.

    Thanks for this video! I am in category 3 and torn my front ligament completely, along with both meniscus getting injured and a bone bruise condition in my thigh bone. I started early rehabilitation training in form of weight lifting etc. with great success and I can run and lift fairly in comparison of before my injury, even playing some soccer 4 times after the injury. My biggest issue is that my knee has been sliding out of position about 12 times in 3 months. But it does not happen while I am doing something that requires more force like training and so forth, but when taking a quick step in the opposite direction boom there we have it. So I find it difficult to make a decision about wether surgery or no surgery is right. My concern with no surgery is that my knee might get new injury or get worn out by continues slides of the bones. But I have not been wearing any form of knee support as of so far. I will try that and give my knee more time to recover before I make a decision. Thank you for making me realize this just now. 🎉

      Ive had acl reconstruction on both knees, most recently 2 months ago with a partial meniscectomy. Its not easy and the recovery takes time and dedication. Based on your comment if you're knee is buckling regularly you are risking further damage to the other ligaments and meniscus. Living with a torn acl requires a lifestyle change for most people, soccer and other pivoting activities are risking more serious damage. Further damage can cause osteoarthritis down the road and result in a knee replacement. I opted to have surgery both times because I do not plan on living my life in a straight line. Take some time for your decision but please be careful in the meantime.

    Hey Doctor. Thank you for this video. I twisted my knee playing basketball. Did not hear a pop. I was able to walk on it after but stopped playing. I’m 48 years old and stay fit. I went to the Emergency ortho near my house - they performed an x-ray and tests. It was a PA but she said I sprained my knee and to wear a brace. I told her my knee buckled walking into the garage step about an hour after injury. I injured my knee at 8am and then went to the orthopedic PA emergency clinic at 2pm. The instability happened at 9am after the injury. Question…she said for me to come back in 6 weeks if not feeling better. She had me jog in place diluting examination and then did some tests. But it’s been 2 weeks and I still don’t feel 100% confidence in it. Wondering if it is an acl or like a grade 2 mcl. I definitely feel pain on the inside portion of my right knee. I wear my brace every where. Only take it off to shower.

    Thank you for this informative video. I’m a ski instructor who had a bad crash 6 week ago with ACL tear and radial tear of the meniscus. My orthopaedic says I should get surgery, I personally would rather not get that. I have to say I’m almost back to my previous physical prowess. I can deep squat, run or even jump (though I do it carefully). I have worked on my quads and flutes and my body is responding well. Of course I still haven’t tried skiing, which is my job. According to the MRI my ACL is torn, but there is no popping or anything of that sort.
    Do you think I should really do surgery? Consider that the doctor said he would take a whole tendon from my thigh to reconstruct my ACL, not excited about that.

    2. Complex horizontal and radial tear involving the inferior margin of the posterior horn and root of the medial meniscus.
    3. Mild joint effusion with synovitis.
    4. Mild marrow edema in the tibial plateau.
    With this the ortho told me to just apply for a rehabilitation for the strengthening and after that, i told my therapist that i want to play again and he said that i should have my acl to be operated to have my life back.
    So as i watch your video, and read a comment i want to ask for a opinion to what should i do for my knee.
    Should i operate or opted for the natural healing method?

    Hello, Don't you think persons with torn ACL can develop osteoarthritis if surgery was not performed?

    • @epmanualphysicaltherapy
      @epmanualphysicaltherapy  2 года назад

      Hey Jason!!! Great question. Everyone develops osteoarthritis to some degree as they age. The problem is when arthritis develops faster than it should. Arthritis progresses too fast when there is too much pressure in the knee joint. This happens when the quads are overused. People with ACL tears are more susceptible to getting faster progressing osteoarthritis if they don't fix the underlying root cause of the fast progressing arthritis - over-dominant quads.
      Even for people that do get ACL surgery, they are more susceptible to getting faster progressing osteoarthritis if they don't address the quad dominance.
      Does that answer your question Jason?

    I have a torn partial ACL graft since 2020. I am still able to play with it but not in a really competitive setting. Hardwork and leg exercises will only help you until it does not. An unstable knee is really hard to risk playing on

    I had an acl injury 6 months ago but the acl test(when we bend feet inwards and squat ) is painless, does that mean i am fit?

    I think I might’ve hurt my acl about a week ago, less than 2 days later I was able to put full weight on my leg, and I’m walking fairly normal a week to the day later, the test you gave here gave me 0 pain or discomfort whatsoever, I was told yesterday by a physio therapist that I may have torn my acl but when I first injured it I felt no pain at all and got up and walked off the field (with somewhat of a limp) but it is just hard to believe I tore my acl when I can put full weight on it, do multiple tests with 0 pain or discomfort, bend my knee completely, straighten it 99%, the other 1% I get discomfort and feel a pain of maybe 1/10, so I would believe if I injured my acl in some way, I just can’t believe a full tear, I can also pivot and cut directions while walking and light jog 7 days later, what are your thoughts if you can respond to this that would be great, I get a second opinion from an actual doctor in a few days to see what’s going on.

      Hey Anonymous, sorry to hear about your knee injury. Yes, it sounds like you're doing well despite what you were told. If you had a full ACL tear, you would not be able to do any of that stuff. Hopefully you can get back to 100% as soon as possible!!!
