I use this route (Mariposa Road to Forestry Road to 3n45 to 138) all the time in both my 2017 Outback or 2009 LX570. Southbound is easy, just pick your line. Northbound your better with an AWD SUV - no problem. 138 is a kick in the face if the pass is really backed up, and if the 15 is stopped south of 138 you're still stuck.. Have you ever gone left/east on 138 to 3n44 and continued south? It looks like you can pick up Cleghorn Road (dirt to the 15, then paved) and then under the 15 onto old Cajon Boulevard and continue south. Oh, thanks for the video.
I've been on the dirt part of Cleghorn Rd, yes. It was also just for fun. That way takes a decent amount of time to cover though, plus there are always off-road enthusiasts from anywhere in SoCal driving/riding in the area. If the Pass south of 138 is that bad, I'm more apt to just take 138 over to Crestline, then come back down 18 into San Bernardino. Another option to get around the delays that 138 @ 15 often sees is to just continue west on 138, then south on Lone Pine. Right around where Lone Pine curves to the right from southbound to westbound is a dirt road you can use that will eventually get you to Swarthout Canyon Rd @ Cajon Blvd. I've had some fun on that road more than a few times in the past as well. There are some other forest service roads that can be used between Lytle Creek Rd and Swarthout Canyon Rd/Cajon Blvd too. Lots of fun to be had, though a fair amount of that is better left for higher clearance vehicles, and maybe also with 4WD too.
I considered adding in another dirt road that I like to take that can be accessed from another road off the other side of I-15, but we were wanting to hurry down to Dora's mom's pad in Pomona when we did this. :-D I'll have to record another one of the other dirt road that I'm talking about. You can find it on the map. It's called Swarthout Canyon Rd and it connects to Lone Pine Canyon Rd on one end and the same stretch of Cajon Blvd that I was on in this video on the other end.
So at 5x speed does that mean that trip took about 15 min total from oak hills to the 138? I traveled that road in a little Chevy spark like that little yellow one in front of you when you got on the 15, and it was a bit rough. I just don't recall how long it took. Days like today (bronco fire) it's Good route for people to take to bypass some traffic

Can a regular 4x4 truck make it through no problem? without really high clearance and off road tires
This video was made with a Nissan Sentra, so any vehicle with higher clearance will absolutely do just fine with this route.
