The nurse, Jones, may have physically been responsible for those deaths, but what action, if any, was taken against those who were in positions of power and let her get away with it - so long as they didn't have to deal with the "problem". Then, gave her a reference and sent her off for someone else to deal with. What she did was just awful, but I think the case can be made that those in authority did just as bad, if not worse.
