If we are talking about condition and medication, we can adopt a methode like
Htn managed with quinapril etc
While explaing about the condition has to mention the context also.
Who become unwell
When she admitted
What was the diagnosis
What are the lab investigation help for the diagnosis?
What are the medications?
When she is discharged and what are the follow-up instructions?
Best methode to write a letter we have to visulaise the condition of the patient and to speak out or write a story?
We have to follow al the steps
In case of emergency letter
Today's informations has to write in the first paragraph.

Eg-Ms styles has hypertension, diabetes type 2 and depression.Managed with quinpril 40 mg,metformin 500 mg and glaclazide 30 mg daily.Her depression is we'll managed with regular counseling.

What happened to him?
When he was treated?
At what context he need assistance?
Whole case note we have to viualize🔥

Irrelavent point should remove and consider relevant points.
