Plugin Used : Follow me
It is very simple to make :
* Draw a rectangle and pushpull it as much as you want
* Draw an arch shape on the front face of the rectangle
* Draw a line from the center of the arch
* Draw a circle on the end of the line
* select the circumference if the circle and the click on Follow me plugin then click on the face
* finally delete the rest of model
