The attachment with 3/8 drive in is for Left threads pistons. You just use a 3/8" in ratchet and a 6"long extension or you can use a small T handlebar.
I have the older Pittsburgh set that includes two handles---one with right-handed thread and the other with left-handed. My car's passenger side caliper piston has to be rotated in counter-clockwise as opposed to the driver's side right-handed. I'm wondering how this tool does this with only one handle. I think someone else was wondering about this too.
I've had and used this kit for over 10 years. For the price, it's one of those HF no brainers.

That's a good question I'll have to take a look at it
I'm thinking that the piece that fits in the ratchet drive could be used in lieu of the T-handle, since a ratchet turns both ways.

I wish it came with the left threaded t handle. Some pistons screw in the opposite way.
