The setup if the interface as explained in the video is the same. Essentially everything you need to do for the exam is the same. The design changes a little but everything is in the same place.

Yes, you are able to use any software package the exam center (your school normally) has available, providing the device is not connected to the internet. The examiner will provide a report for the exam board stating all the software used and the version of the software.
This is the setup i use for my students. If you are using Dreamweaver you can use any interface setup you like. I get my students to use the same one as it is easier for them to follow my steps.

A it hard to figure out your issue without knowing exactly what software you have. Click on help menu and then about. Send me the exact name of the software and the version. Also is this a licensed version?

Again a bit hard to trouble shoot without seeing your screen but, what happens if you create a style sheet from File new CSS? that should enable the designer on the right. If not you will need to send me a screen shot so that i can see the interface. You can email me with screen shots.

Whatever works best for you. If you understand what you have to do and you have a good understanding of the code, it makes no difference what you use.
