Same happened with me while beating a campaign. From my researchers, this glitch happens when player failes qte. Game loads last checkpoint with player models when not every model properly loads in. All the campaign glitches in general triggering when player dies in a cutscene or while failing the qte prompt. The main reason for all of that problems is because of how bad the campaign scripted itself, and mostly because of the Frostbite 2 spaghetti code.
