It seems to me that "How to Read a Book" has not changed the way that books are written. I see still see indices at the end, instead of after the tables of contents, where a skimmer would want them. Also, it seems to me than an index is rather like a list of tags in a PKM, with each tag having a list of pages that contain the tag.
