Addressing Orthostatic Intolerance in ME/CFS

  • Lucinda Bateman, MD, delivered a presentation to the ME/CFS International Conference 2021: RID - Research Innovation and Discovery. Orthostatic intolerance (OI) is the development of symptoms in an upright posture that is relieved or partially relieved by reclining.
    In this presentation, Dr. Bateman reviews OI:
    - symptoms
    - assessment
    - research data
    - hours of upright activity (HUA) as a measure of function
    - triggers
    - interventions (pharmacological and non-pharmacological)
    "Treatment can improve symptoms and function and may lead to insight about the underlying disease process."

    Thank you! I finally have mine under control. I first experienced the problem standing at the deli or waiting in the checkout line. I was fine as long as I kept walking. Had to abandon many a shopping cart to go lay down in my car and learned to sit on the bench near the front of the store until a lane had no people. I was dx with neurocardiogenic syncope now known as vasovagal syncope I guess. Was put on atenolol which helped a lot but after a few years I ended up with HBP so was switched to Benicar. I wear compression socks anytime I know I am going to be upright for an extended period of time and concrete floors are the worst (grocery stores, Home Depot) not only standing but also walking from one side of the store to the other. I have lost weight so hoping to get off the Benicar and am also treating with my Naturopath with herbs for circulation
    One question I have is, I was initially told by my Rheumatologist that CFS patients have abnormal red blood cells, they are sticky so they stick together and are mis-shapen so we don't get good blood flow into the smaller capillaries so she recommended taking grape seed extract. Do the abnormal red blood cells have any indication in the OI?

    Thank you for all you do. I particularly liked the shorter length and limited focus of this video. It made it easier to take in as someone with mod/severe mecfs.

    I'd heard saline transfusions could help ME cfs but didn't understand why, this sheds some light on a lot of things

    I definitely need to do the NASA Lean Test after watching/listening to your video as well as a fellow ME/CFS sufferer advising me too.
    I’ve had ME for well over 30 years and have all the symptoms of pots or OI but finding it is much worse as I get older (61) I have seen my doctor many times expressing concern about very high HR (60 - 130 and up to 170 bpm) but can’t get a referral or any support yet. So I hope my husband can help me do the test.
    If I’m wearing a home BP testing kit do I have to press the start button every minute for result for 10 minutes? Seems hard to do in short time period? I have a finger pulse/Ox so will keep that on permanently and ask husband to write all numbers down every minute? Hope I’ve got this correct?
    I have been very worried that there’s something seriously wrong with my heart and just want to know what it isn’t so I can at least work on getting correct treatment and help myself. I believe I was experiencing OI or pots from my late teens/early twenties. It explains so much.
    Your channel and online content is fairly new to me in the U.K. and wonder if there is an equivalent here in the U.K.?

    I have had CFS for about 8-10 years now. I was finally diagnosed with this disease about 7 years ago. I don't know anything about the ME part of this, but I have all the symptoms of that too.
    My question is how do I get help for these diseases. This is the first I have heard about orthostatic treatment. I live in the Macon, Georgia area. Will you please advise me about this treatment. I seem to be getting worse each month now.
