    I'm a physical therapist who had his right hip replaced almost 12 years ago at the age of 19 secondary to a diagnosis of Avascular Necrosis. In this video, I want to show you how I train to remain fit, active, and healthy while achieving my goals. You should not do the exact same routine as me as we will have different training histories, injury histories, goals, etc. Instead, I just want to show you that you can still be highly capable and strong after a major surgery like this. I always recommend working with your surgeon and physical therapist to tailor a program to your needs.

      6 years ago I got a left hip total replacement. Two questions. #1, how far can you bring your replacement side knee up to your chest? I can bring my non-surgery side up to my chest but the THR side is a good 6 to 8 inches away. #2, on your single leg squats, was that 8 reps at 155lbs "per-leg"? Thank you.

    I am 57 years old and had my Total Hip done 6 months ago using the Zimmer G7 system. I have played ice hockey since I was a young boy and it pained me to stop skating. The news is that there are many of us out there that can return to sport! My implant feels amazing and I have been able to far surpass my old restrictions that were hampering me. Just wanted to give folks some positive news and encourage you to seek good after surgery care, solid nutrition and a maintain a positive attitude. (oh.... get a great surgeon too!) Best Wishes!

    thank you. I have hip replacement scheduled soon. You are helpful and encouraging! I have a long history of doing weight bearing exercises and calistentics, but realize I may have to learn how to train all over again.     I’m always encouraged when I refer to your page. My hip replacement is eminent. The surgeon thought that 300 pound squats might be risky after the surgery. So I’ll have to change my approach. Your instruction will help me with this. I’ll be 65 when I have my THR.

    Wow, thanks. I'd looked forever to get some info on this from someone that's actually active. I had posterior THR roughly 2008 on one hip and 20013 the other (AVN as well). I'd always been active with moderate level skiing, Martial Arts and gym workouts. I had no complications and was walking without issues after a few weeks and was back to horseback riding (hunter/jumpers) within 3 months each time. I was also going to the gym but was always being warned about compromising my hips with any weight training except maybe light lunges. Medical professionals are very concerned about liabilities first and foremost. I'm 72 now and a while ago decided to forego that conservative recommendation since I lost a good deal of mobility and strength in my legs. I've added several of the ideas mentioned here as well as light squats. I figured low repetition and modest weight was lower wear over time in contrast to say, frequent 10K runs or walks. Calisthenics/mobility exercises have helped as well. Things are much better as a result with no adverse affects I can tell. I will say that I adjusted my expectations, progress and weight loads/stress to allow my body to adjust and accommodate to the loads and routines. Each individual needs to assess the benefits and risks/rewards they are willing to assume and live accordingly. It should go without saying (but I will anyway) don't press limits, work through pain or disfunction and make sure you're doing the exercises in correct strict form. Thanks again for the posting.

    This is awesome. Just had a total hip replacement (left, anterior) done 4 days ago. I'm only 53, and normally very athletic, but I had a parachute accident in the Army a couple decades ago which finally caught up with me.

      Sorry to hear about the parachute accident, I had a speed skate accident broke my left hip joint 12 years ago turned arthritic now bone on bone joint facing full hip replacement. Very fit before marathons fast cycling weights never imagined a bike crash would show my hip joint. So obviously devastated and just really appreciate some important pointers regarding the operation if wanting to go back to sport. And can you run forefoot no heel banging? some surgeons say yes & have patients doing the London marathon others say no?.. ok. Appreciate some feed back, I am in need of the operation asap now.. thanks,

    I had a total hip replacement when I was 19 years old I’m now 26 years old, I’m still incredibly active and still train as well but within the past year I’ve been noticing some pain that hasn’t been there before I’m glad I’ve found these videos. You’ve shown me some great exercises and I’m hopeful they will improve the pain from here!

    Thanks for the information regarding weight training with a total hip replacement. Very helpful since there is not much information available. I climbed a 9100’ peak 11 weeks after a right total hip replacement. I’m planning a backpacking trip, so I wanted to add some strengthening exercises.

      @@henaafreen4767 Hi, I had my right hip done 9 days ago. Due to Avn. I have alot of numbness along with pain and weakness in my new hip joint to just above my knee. Did you experience anything like that? Also how are you coming along at this point? Any milestones that you can share?

      @@justina2333 yes I also face lots of pain weeknes.. now my 45 days complete but still I have pain I m taken paracetamol 1000 power twice a day and walk every day 5to 6 min...

    While I applaud ur efforts it would be hard to believe this effort applies to everyone. I’m 64 and am in week 6 of posterior THR. Just met the surgeon today and I’m starting PT asap.
    Although the surgeon cleared me for everything the fear is still a big factor. I’ve always been active but mentally I’m not ready to split squat etc.
    The information is all over the place.
    My advice is to find a surgeon and PT who unfollowed what u want to accomplish post operation then work through that reality.

    Hi, thank you for great content. What to do if I have crunches in the knee and VMO pain during knee flection? It increases after walking sometimes after lifting. Or could you make a video about knee crunches, please?

      Yeah corticosteroids are aweful. Dr. Gave me for my groin injury now I'm suffering from Avascular Necrosis of the hip. It's so painful.

    I had a hip replacement ten weeks ago. I am planning on reffing some games (soccer) in two weeks. Should be fine. Overall recovery was pretty easy. First two weeks is when you really need to take it veeery easy. Short walks, nothing more

    I had a hip replacement surgery 6 months ago… I was told that there was a restriction, in movement bcuz you can pop out your hip & will have to go back into surgery to pop it, back in. So I have been so worried & afraid to do to much to prevent any injury to myself. So I am shocked to see you moving so freely & with ease that is so awesome 👏 How long does someone have to wait, to be able to function, normally after hip replacement, surgery? I would appreciate any advice, thanks!

    Thanks for the information, I am 53 contemplating hip replacement. I walk and work and sometimes I will still jog on a treadmill very short distances but can easily walk 2.5 miles and then to lifting machines but I have significant decreased range of motion and cannot cross my legs and rest my feet on the other knee while sitting. Will I never be able to do that even after a hip replacement?

      Do not follow this video, THR patients can't lay down or sit on the floor forever. Better check with your surgeon. They make videos just to get high no of views. I recently had THR done on left side and i have been asked not to sit or lay down on the floor.

    I am 56 years old and 3 weeks out from a total hip replacement on my left side. I am using a cane to walk. I can walk a very short distance without a cane but there is discomfort and tightness.
    I used to play a lot of sports but I'm told that I will no longer be able to run and play sports like volleyball and softball anymore.
    I want to use this as an opportunity to get back in shape and be as healthy and active as possible.
    I take one day at a time.

    Hi Sir, My dad is 45 and I have a strange question on his physique. He underwent a successful left hip implant surgery 6 years back. Now he is fine and doing gym with less weights, he avoids inclined bench press and most of lower back workouts like squats, as the body weight comes on hips. So, my question is can he do v cut abs workout and abs workout i.e. cardio exercises. Will there be any harm on his hip joints. He also do leg press in outdoor park, he sits on a leg press machine in a park and moves backward and then front direction with legs on pads on a machine.Basically he avoids squats and inclined bench press as well. Please advise can he do v cut abs turning body right and left standing straight and also cardio abs workout which includes little bit leg raise and seated crunches and sit ups Crunches. Will it be possible for his hip or it will be harmful?

    Thank you so much for sharing you give me hope. I am 27 year old male and I just found out that I have arthritis in both hips. Left is moderate to severe and eighth is mild. Doctor said he can see me needing a replacement by the time I’m 30. Absolutely terrifies me…. How are you so calm about the need for revisions? I imagine I will need 3 revisions if the hip only last 15 years and that still would only put me at 75. Do you know how many revisions someone can have?

      I'm 32, had both sides replaced when I was 29, I developed AVN when I was 27, left hip first, slowly the right side, by the time I was 28 I was walking like a penguin, had no range of motion, got my 1st side done 9/11/19, then got the right side done 3/11/20, if you take therapy seriously, you will recover quicker, my left side was worse than my right side, I still have some pain here and there, and I imagine a revision will be in my future, but I'm going to correct my lifestyle and get stronger, it'll buy you some time. Trust me bro, I know the feeling of feeling like life is over, but it isn't my friend. It made me appreciate everything more, and based on the research I've done, if you stay in shape, basically find a routine that works, it takes the weight off that joint, plus you will recover quickly whenever you do need a revision. I was told by my doctor that I would need another one when I was in my 50s. This is my personal goal to get in shape, there are ways you can get stronger.

      @@kervet6046 turned a real corner at around the 3 weeks mark. Still on the crutches for the longer walks (walked 5km the other day) and can get around the house on a cane.
      Doing the Physio etc I’d say I was a real wreck for about 2 weeks. Not in pain really just the exhaustion was crazy.
      Do you need a new hip?

    You said you’re limited still in internal and external rotation, but have great stability which is great! My question is: Is regaining internal rotation possible? Is it related to certain exercises that yours is still limited or does the surgery limit that range indefinitely?

      I have more internal rotation than I did prior to the surgery, and more than my other side which was also diagnosed with AVN. But yes, the surgery will constrain the limits of your range of motion indefinitely. I won't be doing the splits any time soon!

    I broke my hip femoral neck it didn’t heal so I got a osteotomy. My range of motion is restricted and it’s uncomfortable. I broke both femurs and arm in accident. I think I want to get the hip replacement to relieve the uncomfortableness of my hip. It’s tight painful etc. How is your range of motion and does it feel uncomfortable and/or strained? Does it bother you in any type of way after it has been generally fully healed? I think if I just broke my hip it wouldn’t be a big deal for me but it’s taking a toll to have 3 limbs bothering me and hurting

    Hi so I have a question about flexibility. I had a hip replacement 10 months and was wondering how much flexibility in my new hip is safe I’ve started working out a few months now just the bike and a few med ball slams and body weight squats not too deep. No body seems to say how deep you can go what’s safe and what’s not to cause a dislocation if any body knows of a group or something to post questions on instagram please reply thanks. I have quite a few questions 🙂👍

    Hello "Mr. E3 Rhab ", where was your body's ability to perform these exercises...before ...your right hip surgery?.... I assume only like if even just 90 percent less able too?? Also how long did you suffer with lack of being able to perform physical activities before you let the surgeons do surgery,... was it weeks or months or years..? Thank you.

      Hi same here 2 n half weeks ago , an automatic car is easiest but I can drive a manual car too England right hand drive was my left hip I haven't driven far getting in and out is the thing u have to be careful and slow getting in and out the car seat ,,also it feels like I have a spanner in my back pocket when sitting in the car

    Hey there, I'm now 5 weeks post op & feeling great, but still following Dr orders of movement restrictions. Wondering if you have any videos that have workouts for this phase of recovery. Not necessarily hip-related, but general workout that stays within restrictions. I'm just feeling like a slug & ready to get back to some movement. Any insight would be great. thank you

    Hey Marc! Just had a total right hip replacement myself. It was a TFL more lateral approach. I am 35 days post op and continue to have pain that radiates through the superior patellar tendon through the knee and down the tibia. I was told the pain should stop at the knee but when looking at dermatomes I see some extending to the lower leg. Any suggestions would be appreciated. 57 year old very active male who played ice hockey 3x a week and actively weight trained.

      Hormones matter; I’m with you Bruh! I’m three weeks post op and still experiencing significant pain. I’m still taking narcotics. It radiates down my leg. Mid thigh to ankle sometimes. I was incredibly active prior to the procedure. Worked in exercise science most of my career. My poor right hip said ‘I’m outta here’ awhile back. Hope you’re doing better ✌

      @@hoosierbaddy3052 ok good news is that the pain is completely gone. Started following knees over toes guy and Athlean x. The knees over toes work has been life-changing and the medial glute work transformed the hip to knee issues. Simply amazing. Try them out

    Things were going great until I pushed it too hard. Now, am dealing with hip flexor irritation, adductor irritation, and a quad issue. I'm almost three months out from surgery. I'm pulling way back, more rest and some ice, and slowly trying to get back to stability. Is this common?

    Has anyone here had a hip pinning ? (Dynamic hip screw fixation) put in? Just had this done at age 22 7 weeks ago and wondering how soon I'd be able to start working out lower body.

    Hello I have a question I had a hip replacement in 2017 and I have muscle atrophy and it’s still there after all this time honestly I didn’t or haven’t been doing anything to help the muscle come back (I did at the beginning with my physical therapy) my question is is it too late to start to try to build that muscle again? Also can I do leg press? Will be do anything to the hip replacement?

      Never too late. I would go to a P/T for a few sessions to get the exercises & use those. Then follow an exercise program that strengthens the glutes (max, medius and minimus), hamstrings, quads and core.

    Got both hips replaced when I was 19 and I’m 20 now. I’ve been struggling to make or find a workout plan that hits whole body because I feel like I’m limited with my hips. I only workout upper body and I feel like I’m overworking it. It would be awesome to get in contact with and talk more about this

      Did the doctor talk to you about how many revisions you can have? I found out I’m very close to needing my left hip done and my right is showing mild sign of arthritis. I have been going back and forth wondering. How many revisions you can have since they only last 15 years.

    I'm fairly young (49) headed towards THR soon. Wanted to know if you have anyone in your orbit that has experience with getting back to surfing? Doing research & I've now become more nervous of dislocation than the actual procedure. Any info wold be great, glad I found your page

      I've snowboarded and had a client in her 50s recently go skiing. The risk of dislocation is relatively low. I'd definitely recommend communicating your concerns to your surgeon and working with a physical therapist.

    How these exercises are posible .my Dr said not to breach 90s angle ever don't bend etc ..I m confused now ..I got my hip surgery 2 years back ..everything is fine but I want to do some workout but not sure what to do.

    Great video Marc, I am 6 year post left THR, I am a runner and it was great to get back to it 5 months after surgery, I am thinking about adding some core workouts (stomach) to my daily strength training routine which I got from my hospital physio, Is it a good idea to do the plank? or would you drop that exercise? thanks

    How old are you if you don't mind me asking, I had a right hip replacement when I was 35 I'm now 39 and since the op I've shied away from the gym as I'm very anxious about possible damage. This has resulted in me puttin on alot of weight and my confidence is rock bottom

      I am in the same situation, keep this in mind my friend, go to the gym, find a good stationary bike, and ride a mile. Every day or other day and you'll feel your hip and leg getting better.

    you are doing basicaly a one legged squat with 155lbs.....seems like a lot of weight for one hip......would this be the same as 310 for a squat....also i believe you do this twice a week.....considering wear and tear it might be too much. Iam on my 4th year with both hips done....I started squating after the first month with 5 lbs and added 5 lbs every month, after 2 years i am at 125lbs....3 sets 10 reps.....stopped at 125lbs....been doing this weight for 2 yrs....once per week....it is very light but i worry about wear and tear.....dont want a revision....i am 65.....i realize that you being so young you will need at least one revision....in a way this is great for you because you will know how long your first one lasts....then you can structure your workouts accordingly to get the best from your revision. Anyway do you think i can increase my squat weight or frequency to twice a week. I was a powerlifter, ergo the hip replacement....i belive....i was very small boned squating over 500lbs for reps.....this destroyed my hips...no steriods.....H.I.T

    How long did it take before you could stand for hours and walk miles? Does it feel uncomfortable to walk miles and stand hours?

      I don't recall. I walk about 5-6 miles per day without any issues.

    Hi your flexion is well past 90 degrees isn't that a risk of dislocation? Or is that just the first 6 months ?

      Some doctors give temporary precautions early on, but they aren't meant to be permanent. No risk of dislocation for me here.
