Straitjackets originally appeared as a means of fixation in prisons. In the museum-prison of the Decembrists in the Peter and Paul Fortress in St. Petersburg there is an exhibit - a prison straitjacket.
In the second half of the 18th century, shirts began to be used in medicine as an alternative to metal retention devices, rightfully considered more humane. The removal of chains from patients in psychiatry is associated with the beginning of the era of the great French physician and humanist Philippe Pinel, who, refusing to use traumatic means of fixation, did not deny the need for mechanical retention of patients. Abandoning chains and switching to straitjackets led to a decrease in the number of injuries not only among patients, but also among medical staff, as violent patients often used chains that bound them as weapons.
Until the beginning of the 20th century, shirts were a mandatory attribute of a psychiatric hospital. With the rise of the level of medicine and the discovery of more and more effective medicines, the need for means of physical restraint decreased significantly, and most progressive psychiatric schools began to preach a policy of non-harassment, which in many cases was more effective. But it has not been possible to completely eliminate the need for the immobilization of psychiatric patients until our time.
There are two main types of straitjackets in shape: with and without sleeves. In shirts with sleeves, the sleeves can be from 1 meter to 3 meters long, or belts or ropes from 0.5 to 2 meters long are sewn to the sleeves. In sleeveless models, there are special pockets for hands on the inside of the shirt. Sleeveless versions are rarely used in medical practice, more often such shirts are used in prisons.
There are special shirts for patients who need voluntary fixation, for example, for epileptics. The patient is able to fix himself with such a shirt on his own and also independently, being in the mind, get rid of it. Being in a twilight state, it is almost impossible to take off such a shirt.
Straitjackets can be short (up to the waist), medium (up to the knees), long (up to the feet) and elongated, the length of which exceeds the height of a person. In the latter case, the shirt is usually sewn from the bottom. Shirts of this type are sometimes called mummifiers.
There are also straitjackets of increased reliability. They must have one or two crotch (sometimes called a scratch) belts. The crotch straps pass through the crotch and do not allow you to remove the shirt over your head. There are also vertical loops on the shirts of increased reliability. One on the chest and two under the armpits. On some models, an additional belt is added that tightens the elbows.
The variety of models of straitjackets is associated with the search for a compromise between physical fixation, patient comfort, and how convenient it is for medical workers to put a resisting patient in such a shirt. Straitjackets manufactured by Posey can be considered the most successful. Shirts produced by this company can be attributed to shirts of increased reliability, with crotch and side loops, but without fixing the elbows. If desired, the patient can be fixed with such a shirt very tightly, but it is possible to be as comfortable as possible, that is, to leave almost the full range of movements, while the patient will still not be able to take off the shirt.
To increase the reliability of fixation, there are transportation straitjackets. They look like sleeveless shirts and are put on the patient over a regular straitjacket.
