The series used to have a concept of dying in Hell and being "erased". Being killed in Hell is just a severe injury, such as the model in episode 2 who will eventually "pull herself back together". Being erased is when the soul itself is killed, such as by an angel blade. The eyes on the walls and floors are alleged to be markers where souls were erased in the past, but that might just be widespread fan fiction.
