Omg, just cannot sleep so rolled over and took another Hydrocodone. Chronic lower back S5-L1 herniated disc. Have had steroid injections (15+) & microdisectomy. Put off surgery, but I'm ready - have referral to surgeon. I'm sure I have neuropathic pain, and compression of nerve. Wish me luck - I need relief 😮‍💨. Thk you Dr. Furlane, feel like you were reaching out to me!

I am wondering why I’m still having pain on both shoulders, similar to the pain I had prior to my 2-level ACDF surgery last November. My neurosurgeon prescribed Diclofenac 50mg once a day. My GP prescribed Diazepam 5mg to help me get to sleep; and physiotherapy once a week for the last 3 months. But doesn’t seem to help much. What should I do?

Since children who experience adverse childhood experiences are not always treated for those experiences in health care facilities which record the cause of physical injuries; the proper treatment and recovery period etc. later when they become an adult wouldn't it be harder for physicians to diagnose what kind of pain they are suffering from?

If it's for education purposes, why are you naming treatment drugs here?

Seems to me that a person could be experiencing both neuropathic and nociplastic paiin. Could nociplastic pain be caused by the over prescription of pain medication while the patient is not recieving proper treatment for their injuries past a certain length of time after severe injury at work on top of them already having to cope with old scars which they ended up with during childhood too which makes their gait and body mechanics different? Another thiing. Why do some physicians say nothing when a child ends up with a broken bone during regular recreational activities when that bone may have already been compromised before then while not healing properly during both neglect in addition to physical abuse too. Instead the physican only calls it a freak accident instead instead of keeping more detailed records until collecting enough evidence for then reporting to police.

Thank you Dr. Furlan as always, the one who has helped me more than anyone to understand my own experience.
I have nociplastic pain mostly combined with others. For the nociplastic pain I use Gabapentin now (200mg am and pm), also Propranolol (40mg am and pm). The Gabapentin dulls the pain. The Propranolol takes the burning pain out of my face). I also have migraine medication. When I have really bad body pain that I can't reduce I will sometimes use my migraine med as it seems to work for my body inflammation. I also use Ibuprofen and I can't imagine not using it. I tried many times and became extremely depressed as the pain was too bad. I would like to find out more about low dose naltrexone and if this might work for me, and if so maybe I can finally reduce or stop using Ibuprofen.
I have tried the different antidepressants that you recommended but my body reacted very badly to them. I have heard this is a symptom of having nociplastic pain as well, having trouble adapting to medications. Thank you.
