  • Should I Prescribe Ivermectin for COVID? | Ivermectin Update 2021
Ivermectin is a cheap and widely available drug that is getting many buzzes right now, with its potential to prevent and treat COVID, which I’m cautiously optimistic about. We’ve seen the hype before with different drugs that ultimately did not pan out. For Ivermectin, the buzz started when researchers in Australia showed that in vitro, meaning in a test tube, SARS-CoV-2 replication was reduced about ~5000-fold over a 48 h time span.
In April, I made a video about this, and I explained how this drug is thought to inhibit viral replication. And is also widely believed to have anti-inflammatory properties.

    ---------------------------------------------------------
Since April, I’ve combed thru multiple studies, as scientists worldwide have been looking at ivermectin, and so far, 14 studies have been completed. But these studies are not without their flaws. There are retrospective studies that introduce bias, and most of the studies have small numbers, and some are published as preprints before a thorough peer-review process can be completed. The benefit of pre-print studies being published is that knowledge can be quickly shared worldwide.
The hope is that prospective randomized studies will follow these observational or anecdotal studies. The drawback is that studies are frequently published and retracted once they are more critically reviewed. For example, I was able to find 3 ivermectin pre-print studies that have since been retracted. But I need to know, should I be prescribing ivermectin for COVID? If so, at what dose and for how long? Well, in this video, I’m going to objectively break these studies down.
The gold standard for a scientific trial is a prospective study that is double-blind and randomized with a treatment and control group, and it should be well-matched in as many variables as possible to reduce the risk that unforeseen factors, and not the medication's action, are causing the effect that researchers are studying. A prospective study enrolls participants to evaluate a treatment protocol, follows them for side effects, and evaluates the results.
A double-blind study is when neither the investigators nor the treatment recipients know whether they have received the treatment or a placebo. Amid a pandemic, decision-makers have been trying to balance the need for evidence-based medicine and good clinical practice with the urgent need for safe and effective treatments for COVID.
Advocates who are on the side of pushing more treatment protocols through this process, even on sub-standard clinical evidence, will argue that the death rate continues to increase and that there is not enough time to justify a full prospective study that can take months. They also note that the medications they are choosing to treat COVID have been FDA approved for other uses and are considered safe, even if they have not been tested to verify safety and efficacy for COVID.
Ivermectin is sold over-the-counter in many countries to treat people and animals for parasitic worm infections. In Latin America, such as Peru, Guatemala, and Bolivia, its been in great demand as people believe that it protects them from COVID. Researchers in Latin America point out that the evidence is scant that ivermectin protects against COVID. However, they have difficulty recruiting participants to run a clinical trial and test their hypothesis because nearly everyone has taken the drug.
Bias can lead to beliefs that become difficult to dislodge and question. For example, if people believe medication will work, it can become difficult to challenge this belief. Many will justify using unproven remedies by saying that it has a good safety profile and will not hurt even if it does not work. A zealous belief that medication will cure can lead to users ignoring side effects, a confirmation bias.

Mike, as a retired urologist and assistant professor of surgery and someone who appreciates the efforts you make to bring rational discussion about Covid to the public, thank you for your RUclips essays. They are instructive and valuable information for all of us. I would, however, ask you to rethink your decision on prescribing Ivermectin for early Covid patients. All the studies you presented had positive results. Each study clearly had limitations as no exacting double blind trials have been completed. However, in view of the fact that these trials each had positive results with the use of Ivermectin in early Covid cases, I would encourage you to at least offer the medication to your early patients while letting them know that you don't know how effective it may be. At least short term Ivermectin is unlikely to be harmful to the patients and may, indeed prevent more serious complications from the disease. I personally would take it if I were afflicted and I could not do that if I weren't willing to at least offer it to my patients. Give them the facts, but give them the choice.

@Ace Ventura I don't know Dr. Hansen, but I suspect that he simply respects the guidance he receives from the FDA and CDC. Additionally, he deals with patients in the ICU who don't respond to Hydroxychloroquine or Ivermectin - at least not well. He therefore doesn't have experience in preventive medicine where Ivermectin would shine. We shouldn't slam someone for their opinions if they disagree with us, but try to understand where they are coming from. Let's raise a glass to healthy, respectful dialogue and maintain our respect for other's views. Thank you, Dr. Hansen, for opening a place on your platform for a reasonable discussion of views.

I am a physician in the US and have been involved in early treatment of COVID with ivermectin in 18 different people. One of them was my best friend from high school (our valedictorian) who is an emergency room physician. He thanked me for the tip on ivermectin - he and his daughter were both asymptomatic within 48 hours of first dose. A number of my physician colleagues always carry their supply of ivermectin so they can treat at first symptoms.
My experience with 18 people (treated on first recognition of COVID symptoms) is that all have been significantly improved 24 hours after first dose, and all were virtually asymptomatic at 48 hours after first dose. Four people in their 50s and 60s had some residual loss of taste/smell beyond 48 hours, but no fever, cough, difficulty breathing, muscle weakness, fatigue, clotting, etc. Early treatment is very helpful as the virus generally only replicates for about a week after symptoms present. After that you are dealing with the post viral inflammatory phase, which is proportional to the amount of viral debris left from the replication phase. Keep in mind that the potential side benefit of reducing viral replication is that you should be less infectious for the people around you - and if they get infected, they should have significantly less severe illness.
Dosing for humans is 200 micrograms per kilogram, so for a 75 kg human (about 170 pounds), I use 15 mg in two daily doses - the first given when symptoms are recognized, the next the following morning. I have not yet needed to give a third dose.
Note that the recommended dose for horses is also 200 micrograms per kilogram (of horse), identical to the 200 micrograms per kilogram (of human). Of course, a human should NEVER EVER take a medicine intended for a horse, even for treatment of a potentially fatal disease. I certainly wish the FDA would step up and encourage physicians to use their own judgment to prescribe ivermectin for their patients, rather than discouraging it with their statement issued about 4 months ago. They have not updated their guidance to reflect the emerging medical studies supporting ivermectin.
I have observed that the 1.87% ivermectin paste preparations such as Duramectin have just about doubled in price over the past few months. Guess a lot of horse lovers are taking better care of their horses than their families in times of COVID. (Note that some ivermectin parasite medicines for horses contain additional parasite medicines which are listed on the package, such as pyrantel. Don't buy one of these if you want your horse to only receive ivermectin. I have not yet seen any other medicines added to the 1.87% ivermectin paste preparations).
For completeness, I should also mention that almost everyone is deficient in vitamin D. Prior to COVID I had been taking 3K units per day (much more than recommended dose), but increased that to 10K units per day for 4 months (120 days), after which I tested my blood levels (since you can theoretically overdose on the fat soluble vitamins (D, E, A and K)). My blood level was 68 ng, which was perfectly in the "normal" range of 30 to 100. And my serum calcium level was exactly in the middle of the normal range. Of course, everybody is theoretically different, but if you haven't been taking significant vitamin D, think about doing it. I buy my 10K unit gelcaps of vitamin D from Amazon for 4c each. If you wish to take a lesser amount on a daily basis, take a single 10K capsule every two days for a daily dose of 5K, at a daily cost of 2 cents. Interesting study at a hospital in Spain - about 80 sick COVID patients all got the "best available care" but were split into two groups, and one group also got high dose vitamin D (actually, activated calcifediol, which is the activated form, allowing more rapid effect). In the group without vitamin D, 50% went to ICU. In the group with high dose vitamin D, 2%. Decide for yourself whether you wish to overdose on vitamin D, or overdose on COVID. (Don't take more than 10K units vitamin D per day, though, without consulting your doctor.)

If he applied the reasoning he uses not to prescribe ivermectin to the vaccines he wouldn’t endorse or use the vaccines either now would he?
Wasnt the emergency release of the vaccines contingent upon there not being a treatment?
Also being a doctor you might understand what is meant by “leaky vaccine” which is caused by mass vaccinating people during a pandemic which causes the virus to evolve variants on behalf of its survival. A virologist and epidemiologist should know that you don’t mass vaccinate during a pandemic. You mass vaccinate to prevent a pandemic. They know this but they are doing things as they are? It goes against the science.
So here we have the suppression of a long used treatment safe and cheap while on the other hand a coerced and forced global mass vaccination campaign that is making things much worse. Huh.

A good nurse would know to go to the Infectious Disease Society of America, FDA, NIH, CDC, British Medical Journal, New England Journal of Medicine, Johns Hopkins and the Mayo Clinic, for starters, to see what the best primary medical sources that examine ALL the studies worth examining have to say.
      Did you do so?
Numerous primary sources eliminates cherry-picking, outliers and conformation bias.
Had you gone to those and other primary sources (see my post, below) you would have discovered that to have any positive effect, Ivermectin has to be taken at doses 100 times that approved of in humans, and is therefore not recommended because it can harm or kill at those dosages.
We have far safer drugs that have a far more beneficial effect because, for one thing, they are designed to attack viruses and not lice or intestinal worms.

Also when our cats and dogs are sick, they just went outside to sun bath. And then when they return home they were okay. Ive notice that i cough alot and phlegm were release and I have back pain. The positive thing that ive got from it is that i was not that weak and i had a appetite to eat. I was not eating well for like 3 days. Then i tried ivermectin in my 3rd day, i took 3 and the other 3 is for boost, its more like vit d. After that i was shock that in day 4 i was able to eat and my back pain was gone. My cough was reduce and i can cook and wash my clothes and clean the house. So i stop taking meds for cough, took ivermectin, went outside, took vit d and c. Eat fruits and drink water. In day 5 i was like 95 percent recovered with super mild cough like i cough every 4 hours or 5 and only ones or twice. So i am a believer of ivermectin and also vit d.

The answer is no, but there's too much money to be made by big pharma, so our regulatory agencies will push the experimental therapies that cost a fortune, and try to snuff out existing and known to be safe drugs like ivermectin because the patent is public, and there isn't any money to be made. this is disgusting, for-profit health care at its worst.

Your rational is flawed. What is the cost benefit ratio. What do you lose by prescribing a drug with a well know high safety profile. While at the same time included in the standard of care is Redesiver and monoclonal anti bodies that show less effectiveness than ivermectin and more severe side effects. There are no outpatient treatments that meet the standards you propose. So you would deny a potential life saving drug, knowing the risk is low and the benefit potentially high?

We need more studies? We already have a valid study. It ran for the last year and involved the whole world population - and we were all the members of the control group who got nothing - not even a placebo. It is now a scientifically proven fact that if you don’t do anything to prevent infection or treat early symptoms, hundreds of thousands of people die.
We got that part of the study down pretty good.
It is unethical to have a study where denying medicine to the control group may (will) cause harm or death. Continuing to demand further study which must be double blind is now not only ethically unacceptable, it is immoral.
Anywhere it has been adopted in large populations has been shown to decrease the death rate dramatically in absolute correlation with the timing of its widespread use. (And, apparently, the dosage they are using seems to work - we could just ask them).
That information alone should be enough to not only unlock the restrictions to its use, but to promote its wide and immediate distribution.

We have a drug that has as been proven safe and effective in stopping Covid yet the government, and it's minions like You Tube, have declared war on it in favor of an experimental vaccine. The question is WHY?. Why are governments so hell bent that this vaccine be the mainstay, the front line of defense against a virus with a 98% survivability rate?

Once you are satisfied that something works it becomes impossible to run trials in which vulnerable patients might die for lack of a medicine you know works.
