  • Chronic non-bacterial (aka abacterial) prostatitis is like a ghost. It poses a major challenge to patients and doctors because it is very hard to treat and like with ghosts, the question remains: does it exist? Welcome to yet another fascinating video on UroChannel where Dr. med. Dr. phil. Stefan Buntrock, urolgoist and sexologist from Göttingen, takes on one of the most challenging diseases in urology. Chronic abacterial prostatitis is a perplexing condition where no bacteria can be found as the cause of inflammation, making it a puzzling challenge to diagnose and treat. The problem is, that this form of chronic pelvic pain is likely not to be an exclusive disease of the prostate. Most likely, several different conditions mimic the symptoms of prostate inflammation, something that is called by the term "referred pain". Muscular overactivity from a tight pelvic floor is definitely one of the reasons. Dr. Buntrock describes how treating pelvic floor overactivity with low intensity shockwaves (ESWT) is able to cure these men. This finding challenges the whole concept of chronic non-bacterial prostatitis because it may not be the prostate after all. However, not all men respond to that kind of treatment. So it is most likely, that a number of health conditions will cause the same kind of symptoms. In summary, multimodal therapy with cognitive behavioral therapy, non-steroidal anti-inflammatory medication, muscle relaxants, accupuncture, etc. will be the treatment of choice.

    I have only “constant urge to pee but nothing comes out even after fully emptying” , two urologists diagnosed me differently one OAB and the other Abacterial prostatitis! NOTHING HELPED ME It is like living on my own private hell

    In my opinion it is a problem of the Nervous system!
    Stress is the big problem.
    The Body and the System is to long in the fight or flight modus.
    The muscles in the pelvic floor can't relax!
    Then comes the pain and all the others problem.
    Stress is the biggest problem!!!

    Great video :) I have had chronic non-bacterial prostatitis (at least that's what I think it is, after two urologist visits) for the last 1.5 years (43 years old, fit and sportive), my only symptom is blood in semen. It used to come and go, but for the last 2-3 months or so has been constant. I have an irregular sex life, long periods of inactivity but I do masturbate. I took saw palmetto extract, but it did not go away. Can it be latent stress? I'm beginning to worry about the consequences. Thanks in advance :)

    I was infected with a sexually transmitted bacteria. I did not receive a sufficient amount of antibiotics at first, and after that I took a sufficient amount. I did a culture and the culture was negative, but I began to feel pain in the perineal area and it became burning when cleaning the anus.Cool, is this the solution to my condition as you said?

      I am answering to your comment because I think many people think so. But masturbation has nothing to do with prostatitis.

      Pelvic floor overactivity has no influence on PSA, I am afraid…
