Maybe Black Mesa also had a bit of an understanding what guard duty actually entails. Case in point, they have about a billion Barney's where they didn't prioritize attention spans, and the facility is a complete warzone now. They clearly had massive issues even before that.
Don't forget smoking and drinking coffee. in all seriousness, a really good security guard is one that doesn't get bored and can keep themselves awake, observant and ready for anything even when nothing happens all day. I'm sure if they had more of those types of guards at Black Mesa, this situation would have happened differently.

I have a theory that the reason why we never see the scientists poop other than those 2 is because they all wear adult diapers due to most of them not getting bathroom breaks.
Even when there are more enemies. The inside _could_ be used for CQC (Close Quarter Combat), but the outside is either flat or full of hills. You can tell the difference in urban combat as well. Look at the USA invasion of Panama, for example.
