    Stephanie asks:
Apparently I broke my scaphoid months ago and the doctor just now caught it because I was having a hand xray on a finger. I’ve never had any pain in my wrist or scaphoid area, none at all. They have me in a cast, but my take on it is, if I didn’t even know it was broken, why bother? There’s no pain and no swelling at all, and I wouldn’t have even known had I not had an xray for something unrelated. What is the risk, with no pain and having not even known I broke it, of taking the cast off and going on with life?
The scaphoid is the bone located at the base of the wrist on the side of the thumb. You can injure it by falling and landing on your hand. Often a patient thinks that they just sprained the wrist because there is no obvious deformity. X-rays, though, can show the fracture.
One major challenge with scaphoid fractures is getting them to heal properly. The blood supply to this bone is tenuous, so even nondisplaced fractures can fail to heal. Displaced scaphoid fractures almost always require surgery to line up the fracture and compress it with a screw. Hand surgeons often operate on some nondisplaced fractures in athletes. If nonoperative treatment is possible, the surgeon will likely still treat the patient by placing him in a long-arm cast.
If the fracture doesn’t heal properly, the bone can collapse, and the patient can develop degenerative changes in the wrist. If a scaphoid nonunion is seen on x-rays, the surgeon might fix the fracture surgically and add bone graft to help the bone heal. The key is doing what needs to be done to get the fracture to heal.
Please remember, while I appreciate your questions, I cannot and will not offer specific medical advice by email, online, on my show, or in the comments at the end of these posts. My responses are meant to provide general medical information and education. Please consult your physician or health care provider for your specific medical concerns.

Dr. David Geier I fractured my scaphoid and I’ve been in the cast for 3 months. Now the surgeon told me I have 2 options which are; 1: try let it heal by itself and I’m going to get arthritis or 2: they have to take a bone from my knee and replace the fracture which will then heal. But for my knee there is a risk that I will get pain, clicking, and sometimes can’t walk. If u can give me any suggestions that will be greatly appreciated

I am 37 years old, I have had my left wrist in a cast for 6 months in row after surgery that involved K-wires to better the alignment for the scaphoid bone that was fractured at the waist. The bone has healed almost 100%, yet the wrist became stiff. I have been doing physical therapy on it for almost 2 years thus far. Now, when I bend it downwards it moves, but not 100% except under pressure which cause pain as it gets to closer to the natural bending range. Likeswise when moving it sideways towards the thumb, it moves, but gets painful as it comes closer to natural end of this move.I am too frustrated yet I do not lose hope, hence, would you mind recommending to me any recent technique, surgery or therapy that heals the remanants of this stiffness and recovers it 100% as it used to be?. Thanks a million for consideration.

I had what I thought was a sprain in my wrist in my early 30s. I didn’t seek treatment. Turns out I had an undiagnosed and untreated scaphoid fracture. In my mid forties, I developed severe pain in my wrist. I had developed SLAC wrist. Now 10 years of pain later, I finally had a a Scaphoid Excision with Four Point Fusion. I am 11 weeks post op and still in my splint. Great advice from this video. Get any wrist injury examined and treated.

I had what was termed a violent scaphoid fracture years ago falling off my scooter - I was offered a surgery option with a small screw straight away and took that option. Was done two days later - only needed a splint not a cast, very little discomfort at all and healed perfectly well in about 4 - 5 weeks with no loss of function / motion. Would highly recommend the surgery.

Somehow 15 years later I rebroke it in my sleep it was nice an calcified spent a year in a cast Dr said I probably didn't need surgery. One year later I got surgery and then my Dr retired. No follow ups or rehab were scheduled. It's now been 2 years and it still hurts and now I can barely do anything with it. I was a juggler, exteme sports enthuisiasts, and acrobat. Now I sit on the couch and wish I was dead. Everyone at work and in my social circle acts like I'm a faker and the only thing I have to look forward to is the off chance I have to get surgery again which will be a bone graph from my hip and another year of inactivity. I think a Bullet would be a better cure at this point my quality of life is absolute garbage, my incomes been cut in half cause I'm a manual laborer that can't work. I broke my scaphoid in early December 2015 and am still in a cast now (September 2016). Totally sucks. when I broke it I was told 3-6 months to heal (sounded long at the time), and now I'm on 9 months. My next appointment is in mid November, which will make it 11 months in a cast (assuming I get it off). In all honesty next time I fall I'd rather land on my face than break this POS again. I'm a young, healthy, active male, which makes the time to heal even more frustrating. Also my doctor said physical therapy probably won't be necessary which I find hard to believe.

Had a scaphoid fracture on my shooting hand my senior year in basketball. I taped it up and played with it for 5 games because I thought it was a sprain. When I realized it wasn’t getting any better after a month I got it checked out. It split directly in 2 and the bottom half of the bone died so they had to use bone grafts to fix it. I got extremely lucky that it healed. Despite a successful surgery, 7 years later my wrist is now at about 60% of what it was when healthy. I would say. Being “tough” isn’t worth it. If I would’ve gotten it checked out ASAP i would’ve got a dissolving pin put in. Now I have a permanent screw.

My fracture was missed on the initial evaluation. After several months of physical therapy another x-ray was taken and the fracture was discovered and was displaced. My surgeon had to take a bone graft off my hip and inserted a screw. The end result is that I have lost range of motion and now after 5 years the pain is getting to the point where I will be going back in to have it re-evaluated. X-rays after the surgery showed a small loose piece that the doctor acknowledged but said that it shouldn't be an issue. I'm not sure if that is what is causing the pain now or if it's just arthritis.

The ortho asked me to proceed to do exercises to increase mobility. 1 year later, I still do not have full mobility and if I extend it beyond a certain , pain creeps in. If I keep extending it, the pain disappears and I get increased mobility for a while. Hours later, I lose that mobility again.

I had great mobility before with it broken and had to have surgery with a bone graft it's been 2.5 months since. I'm in a brace and mobility has gotten better since the 2 week cast came off but I can't move my wrist nearly as much as I thought I would be able to. I hope it heals. I don't want to be out of the game again. this isn't a respectable injury. I think people feel like I'm milking it. breaking my leg would've healed quicker. ughhh

I was in a cast. Then it was taken off but NOT re scanned. So they didn’t check that it was fixed. It turned out that it wasn’t fixed. And I now have a non union scaphoid bone and a vastly reduced range of motion. If there a way of increasing my range of motion?

I had an accident more than 5 years ago and still live with the pain in that area.
It first happened 12 years ago and it healed by itself which is the reason i didn't went to the doctor until now.
The pain and the limited mobility of the wrist that i learned to live with until hiling doesn't seems to work now like before.
Is possible that the bone to be broken and dead? I work in industry so i need my hand very much. Thanks!

I broke my scaphoid in the exact reason you talked about playing football and falling back after getting hit and landing on my hand and having my wrist bend back. I went to the doctor about 3-4 weeks later because the pain wasn't awful and it was mostly when i applied pressure to the wrist mostly because i wanted to finish the football season so the first time i went to the doctor i got an x ray and the doctor said they couldnt find anything wrong. So after 2 months went by and i couldnt walk my dog without getting pain in my wrist i went to the doctors and got another x ray and they told me it was broken and had been broken for a while and that it had started to heal improperly so they would put me in a cast for 3 months and see how it was after the time. After the 3 months the doctor told me they liked the progress of the cast and would put me in a cast for another 3 months when i got my second cast off they told me my wrist looked 100% better and that i shouldnt have any problems but as soon as i got the second cast off i could feel pain in the same area i always had and till this day my wrist hasnt felt the same and i still get theses pains espcially on cloudy rainy days any tips ?

Thanks for the video Dr Geier!
I broke my scaphoid 6 years ago on my dominant hand. My experience was similar to what you mentioned in your video, I thought it was only a sprain until I had an x-ray 3 months later for a separate issue. I ended up getting a bone graft and screw placed in. 6 years later and the range of movement I have is still worse than my other hand and I get a bit of pain especially when I go to the gym.
Is it worth getting it examined again to see if it's healed properly and are there options to help increase the blood flow to the bone?
    Thanks

Pretty much same what happened to me. The doctor missed it completely and didn't even bother calling me to tell that her diagnosis was wrong. She said that it was just a sprain after reading the x-ray in front of me. The radio guy found out that there was a fracture but no one at the hospital bothered contacting me about it. I only found out 7 months later as it was still swelling and feels hurt.

Hi Dr. i have finally ran into a video that could further help my situation. Several years ago, I was playing football at a park and I landed on my wrist. It hurt like hell that whole day. I recall the pain. I left it untreated. Months later, all it would do was pop. So I left it alone. It still would give mild pain when I would pull wrist back. But I didn’t bother for a while. I got an X-ray and they said I was fine. So I obliviously went on. Years later now, maybe close to 10 years. It still pops. And it mildly aches when I twist certain angles. I fear now that I am in a bigger situation with this wrist. It’s on my right wrist and I can still do normal things without pain even workout with weights it’s just certain angles that ache more. My other wrist looks thinner compared to the one where I suspect I have had a scaphoid fracture. But now this really terrifies me to know I’ve left it untreated for all these years...

I broke my scaphoid fracture about 24 years ago during a game of soccer and had surgery back then applying a stainless steel screw which was removed after some time. Recently I suddenly experienced the same pain as before as if it broke again. X-ray and MRI are not indicating fractures (which is commonly overseen! same thing back in 1997!) Good healthcare and proper analysis in my country is very poor and restricted. Since there are only 5 or 6 orthopedic surgeons (most of them unexperienced or super old) in Suriname I have my doubts especially when I got injected with
corticosteroids which did absolutely nothing. The same pain and immobility still in the hand. Any advice to point out certain things to my doctor so he can check additionally? Thanks in advance! greetings from Suriname. South America

Just had my surgery one week ago. May 2014 I fell from altitude on my wrist. I thought it was no big deal as well and I continued doing my stuff. After few months I noticed it isn't healing and the motion became limited with some pain. Especially problems with push-ups and handstand and I couldn't straighten my right arm as the left one anymore. Due to travels and work I didn't do surgery until now. After more than 5 years artrosis has developed and fortunately there is still blood going through my scaphoid bone. Now they scooped some bone from my underarm and put it in the fracture and fixed with a screw. Doctor says no rugby for me for 6 months :'(. So guys, don't be like me and treat it ASAP.

I had a non-union scaphoid fracture from over 10 months ago that, due to insurance reasons, I just had surgery yesterday. They actually used my right hip to harvest the bone for bone graft (the same side as my fracture). I'm not sure why they used my hip, rather than another part of the wrist, but due to the amount of time that had passed, I experienced quite a lot of bone loss. Could that be why the used my hip, perhaps? Maybe they needed a bigger piece of bone than could be grafted from another part of the wrist?
Also, I too thought it was just a horrible sprain, from March '18 until September '18 when I had my first X-ray, I don't think it was completely broken in half, and I continued working as an HVAC installer (with a good wrist brace), because insurance was so expensive! Long story short, a month later I fell asleep without my wrist brace, and I manage to break the scaphoid all the way in half, which I found out when I had another X-ray and a CT scan. I was supposed to have surgery in November, at which point I was out of work, but at the last minute I got a call saying my insurance would not cover it! Anyway, I was FINALLY able to get Medicaid, thank God!
Funny thing is my hip is in more pain that my wrist!

The doctor told me I would have to get cast on for 10-12 weeks but he said because you are young you have a good chance of getting it off early and it did. I’m 15 btw. I broke it when I fell and played a football game with it broken. I almost didn’t go to the doctors but I told my parents that we need to go. I got it off the 8th week. I’m not allowed to have contact for 10 weeks. Since I’ve had it off it has hurt a little bit when I use my hand.

I broke my scaphoid just over 30 year's ago, I neglected it for a sprained wrist didn't seek medical advice for about a month, then was in an experimental fibre glass cast for around 6months but doubt it healed correctly even then. Five years later it was fractured again this time I was given a bone graft they used a piece of bone from my elbow. Again in cast for month's. This time it was much worse , I lost a good deal of flexibility in my wrist,it won't bend to full extent as normal nor rotate properly. I have more or less a constant throbbing low level pain , occasionally very sharp pain which I can mostly ignore during the day but notice at night. I've totally given up on it ever getting better and have just learned to live with it. I believe the surgery may have made it worse. I wouldn't even contemplate further surgical attempts to remedy it for fear of again making the situation worse. I've learned to live with that throbbing pain now, I know I've developed arthritis for sure. I just take some codein and ibufren and aspirin when it decides to antagonise me. I'm in my mid sixties now so I'm just going to put up with it until I pass on. The low level pain is an old friend now and I'd probably miss it. Anyway you get one do something about it right away don't neglect it as I did or you'll be sorry, it's haunted me all my life with low level misery. T

Get your wrist pain checked out, don't ignore it! I broke my scaphoid and it became displaced. It swelled up and bruised for about two weeks after the injury but I assumed it was sprain. After 9 months of not being able to support weight and limited movement I saw a doctor. The two halves never rejoined and I have abnormal bone growth. Now I have to have surgery to remove bone, get a bone graft, and join the scaphoid back together. This could of all been potentially avoided if i just saw a doctor and was put in a cast.

    Dr.Geier,
I fell on June 30 this year didn't think much was wrong at the time, I had fallen at work so I did incident report. later that day when I got home I had a lot of swelling in my ankle for I had twisted it and then fell face first, so of course I put my hands out. The next day I informed HR that I needed to go get an x-ray to make sure everything was ok. I went for the x-ray and they came in and said I had a Intra-Articular Distal Radius Fracture and immediately tried to get in touch with Orthopedic but no luck since it was the weekend of the 4th of July took workers comp 11 days to see Hand Specialist he did x-rays and said the fall had caused an Arthritic Episode but it didn't feel like Arthritis. I had to go back 14 days later and he had decided to give me an Cortisone Injection for the Arthrits and since then it has went down hill. I felt like he hit something when he did the injection and now I have pain that started radiating up my arm it went across my wrist first then radiated up my arm, the doctor sent me for an MRI to start with and said there were no changed since previous one last year but pain didn't get any better and I went back and the last test I had done was an Nerve Conduction Study EMG and the Neurologist said I had damaged my funny bone nerve and so now I don't know what to do. In the very first x-ray that was taken you could see a little line across the Scaphoid bone and I have had a few doctors that are my friend look at it because I was so confused and they said there was a fracture that it was slight but there so I went from half a cast to splint then to CMC brace with no use of thumb pinching or grasping, then after all this the doctor said no use of arm while at work until further notice. I have second opinion coming up but not until November. I am confused with the amount of pain I am having the past 3 nights I have been in so much pain that in 3 days I have had a total of 9 hours sleep and I'm exhausted. I was just wondering what kind of symptoms would you have if the pain could be caused from Avascular Necrosis or what since it's going up my arm and I have been having pain in my shoulder and radiating up into my neck. What do I do ? Because the doctor I have been seeing doesn't have a clue. He didn't see the fracture on x-ray but it looked like it was in the waist of the scaphoid. Any help would be appreciated...

CT scans should be protocol for wrist injury not X-ray.

Almost a year and a half ago I had wrist pain that increased over weeks with no known accident or incident. My primary said it was "tendonitis", and had me rest and wear a splint for 6 weeks. No improvement, so I was referred to a hand surgeon. X-ray showed no fracture, but then, due to continued pain, an MRI revealed a non-displaced fracture through the scaphoid waist. At that point, 4 months after presenting to my primary doctor, the surgeon and I opted for surgical repair. Following 5 weeks in a cast and 8-10 weeks recover, pain was not improved. A CT scan showed a possible protusion of the surgical screw, so another surgery was done to remove it. Another 8 weeks recover and unfortunately the pain continued! Possibly worse. A final MRI last week (14 months after the initial MRI, and 18 months after my initial presentation to my doctor) the MRI was positive for Preiser's. I just found out today. The necrosis is fairly advanced. My surgeon is referring me another hand surgeon who may be able to make a treatment plan for me.

I had my surgery 3 years ago. I might not regain full mobility and get rid of the pain for good as it still hurts when placed in certain forceful position.(e.g defending against an offensive player using the treated hand).
Which has me worrying is that I can't pop my thumb properly anymore, I had to move it around to do so and it hurts so much. Pain is centered at the base of the thumb.

I fractured my scaphoid along the proximal pole approx 5 years ago, playing rugby. Went to accident and emergency the next day, only to be told it wasn't broken. I continued to play rugby, train hard and maintain my usual active lifestyle.
About 9 months after injury I was still having some discomfort so went to my GP who referred me to the fracture clinic.
Got an MRI which did not show the fracture properly but my scaphoid appeared Black in the image.
Got a CT scan which finally showed the fracture.
I was put on the waiting list of surgery which I had approx 4 months later.
I had a bone graft from my radius and a screw put in. This did not work.
I then had a bone graft from my hip and the screw removed. This still did not work.
I then had another bone graft from my radius which once again did not work.
I then had a de nerving operation, which did not receive the pain.
Last month, after 4 operations and 5 years of pain, I had the scaphoid removed and the wrist was partially fused.
The surgery took 4hrs 30 minutes because of the amount of sinnuvitis and scaring. When they tried to screw through my lunate, it fractured also. They had to secure the wrist with k-wires, which subsequently got infected post op.
I have had a horrible time with what is such an unnasuming little bone, PLEASE if you suspect a scaphoid fracture, get it checked out!!
I know have no movement in my left wrist what so ever and I am only 26!

I just fractured my scaphoid this past week, and meet with a surgeon in a few days to go over next steps. Ive been in a thumb cast for 9 days now and its awful! Is the recovery time shorter with surgery? i want to be playing hockey and lifting weights as soon as possible, and a cast doesn't allow me to do that.

I am not sure if it’s a relevant question, but it’s been nearly 5 months I had surgery and the bone is healing in a slow rate, and the range of motion is very very slowly increasing and the strength as well, so can I get at least 80% of range of motion in a few months?

I had it happen almost 30 years ago. I cut the cast off when my job said they didn't have any light duty. Please don't make the same mistake. I'm 59 years old and I don't have much use of the wrist left. And was told fusing the wrist was about the option.

Hi, i had a bike accident 16 weeks ago. Dr. Immediately cast my wrist. After 7 week they removed cast but it was not healed so they again casted it for 7 more week. Still after over 14 week in casting it was not properly, dr was saying that its healing really slow, and now after 14 week they casted my wrist again for 6 week, i want to ask that is this treatment is enough for my scaphoid fracture. Need your consultation

Please advice

I need help. I have a crack in my scaphoid bone after a motocross crash. I have a cast that will be removed after 5 weeks. The doc said, there si 50/50 chance that the bone will heal correctly, but there is a possibility,it wont, and if I want to continue to ride motocross, they will need to put a screw in my scaphoid bone. Should I be worried? What are the chances it wont heal right? I am already missing 5 weeks+rehab,I do not want to miss another 5 or even more. Thanks.

i hurt my scaphoid initially of fall 2014 due to falling during skateboarding (classic). Not until fall of 2016 did i get it checked out. Still have not been able to make into surgery. I've been abusing it continuously since. I am hugely passionate about skateboarding, therefor i am very disappointed in my decisions. I believe my scaphoid to be beyond unhealthy at this point. *One major question i have is - After the healing process from surgery, is the hand reliant to take a fall, during activity?*

I had the cast on for a week and from prior experience I know how anoyying the cast can be so I went for the surgery and it's definitely worth it. Small incision. Mild to intense pain for only about 3 days and now it feels just like normal

Hello doctor , thank you for the video , I broke my wrist in January this year and was under cast for about a month or so. The hand has improved a lot but even after 7 months I still feel some pain in my wrist. I also ho to gym and do very light movement exercises and that has helped my wrist heal but I don't know why it still pains. I would like to know how long does it usually take for the wrist to be absolutely pAin free, thanks :)

    • @DrDavidGeier
      @DrDavidGeier  7 лет назад +1

Assuming the fractures completely heals, which is critical, pain usually goes away in 2-4 months. There are a lot of factors, though, that can influence the development of pain, like stiffness.

I highly recommend to get an X-ray if you have pain in your wrist.
I broke the scaphoid bone over a year and a half ago and sometimes I have pain for months sometimes no pain at all.
Until last June it was hurting a lot so I had to go to the doctor and I Found out It was broken and that I had a cyst so I just had surgery last week they had to take a bone out of my hip and took out the one from my wrist and put the one from the hip instead and put a screw to hold it together. Now I am wearing a temporary cast and will have to wear a cast for 8 weeks after they remove the stitches.

On April 19, 2016 I was riding my friends ATV and I did not realize that it did not have brakes. I went very fast around a corner and tried to slow down so I did not flip the quad but I then noticed that I could not slow down due to faulty brakes. I held on to the handle bars as tightly as i could and I was ejected from the vehicle but I my hands were so locked on the handlebars that at the moment I was ejected, my Scaphoid snapped in half. When I hit the ground I also broke my knee cap into 4 pieces and I also broke my Tibia (leg). I was also not wearing a helmet.
I got my cast of my arm in July and my leg and knee recovered in May. No surgery was necessary because I was recommended the Bone Stimulator. Its been 9 months since the accident and I still cant run, jog, kneel or lift anything heavier than 30lbs for longer than 45 seconds

In June, I hit a door jam w/ the lower part of back of my hand. It hurt like a mofo but I hoped it wasn't a big deal. After 3-4 days the back of my hand, wrist & the fatty pad under my thumb had swollen to 3 times the normal & the bruising=rainbow (I have pics, it was pretty gnarly) Mod-Severe pain in the 'snuff box' and weakened grip. Since x-ray was negative, the work comp Dr said it was a severe bone contusion, given a carpal tunnel brace which made it hurt even worse. told to take Advil for pain (I told him I can't take nsaids) & was sent on my way. Tylenol didn't do anything for pain relief, but I had nothing else. Now, I'm still pain, weak grip, consistent hard bump and tissue swelling near thumb base. The initial swelling and bruising has improved some but I have recurrent/new bruising/swelling almost weekly. I know it doesn't follow the typical mechanism of injury but I was in a car accident 2 weeks prior with some minor pain with thumb movement. I can't help but think might be connected.

I've had surgery and a screw was placed in the bone and even went through physical therapy. I can no longer do regular pushups and I still have pain because I lost the flexibility to bend my wrist back while in the push-up position. Is there anything that can be done to fix this in your professional opinion? It really sucks to lose partial function of a body part you still have.

    Hi.
I had a scaphoid fracture for my right hand. It is non displaced fracture. my doctor has put synthetic cast 4 weeks ago. In my initial x-ray the fracture was not clearly evident. But yesterday after 4 weeks again x-ray is done and not scaphoid fracture is clearly evident. Doctor says bone cells around the bone got decayed. I have a doubt whether this fracture heals with in next 2 weeks. Since I've commitments like I've exams ahead in 1st week of may 2017.
And I have another doubt the cast that has put is wobbles and it's not very tight. Doing activities like eating, washing etc with fractured hand will make my scaphoid heals slowly?
    Kindly guide me.

I had an accident during my work with bike. I went to the doctor and he made me x-ray but didn't saw anything. So I went home and after 3 days I was feeling good so I started again work. I was still having some small pain but I told that will disappear with time. After 3 weeks I was still feeling pain, so I went to the doctor again. He did CT and saw that I have broken scaphoid. So I made surgery 5 weeks after the accident and now I have screw in my bone. At the moment I am with cast for 4 weeks and I hope that everything will be fine.
