No, I had corrupted crazy slots before and it just showed up again, normally it shows 4 bells that you can pick but if you have a corrupted variant then you just get a 5th card option that shows your corrupted bell. So essentially you can either choose to keep your corrupted bell or get the non-corrupted version.
You only get 5 reroll slots if your current bell is corrupted (Normally 4) Basically the 4th slot is a non-corrupted version of your current bell and the extra slot (5th) is your current (corrupted) bell. However the downside variant can change if you pick the 5th option.
