You install whatever source port you want to use for playing the game (ZDoom, Zandronum, GZDoom, etc.). Then, you download the file that is in the link in the description. After that, you extract the file (you will need WinRAR to do it which can be downloaded off the WinRAR website) and ctrl-click the wad file and the texture packs that came with it. Last, you drag the files into the source port and it will give you the list of IWADs that you want to load to play the level.
I'm not a huge fan of playing levels on Nightmare mode because there would be an issue with maxing it out in terms of the kills (enemies resurrect after 30 seconds), causing depletion of resources (ammunition and health). Another issue is that you are unable to warp to another level when playing on such mode (I play levels from pistol start), so that is why I play on Ultra Violence.

This one was already a difficult one. Nightmare would be nearly impossible to do.
