  • How does CPAP really work? Health Ed Solutions lead instructor Mark Dzwonkiewicz gives a quick and dirty breakdown of how a continuous positive airway pressure (CPAP) machine works. Thanks for watching and stay tuned for more lessons in our "One Quick Question" series.
    Here is some more information about Health Ed Solutions:

      Are you sure that during inspiration, the drop in intrathoracic pressure creates pressure on the vena cava? I agree that inspiration leads to increased venous return, but this is because the right atrium pressure decreases and makes it easier for the heart to fill. I dont understand how a drop in intrathoracic pressure leads to an increases pressure on the vena cava. What you are illustrating with the syringe is the pressure on the thoracic wall from the atmosphere, not the pressure on the vena cava.

Thank goodness, my ability to fall asleep, stay asleep, and wake up refreshed has never been better. No longer a mouth breather, the nose mask was the way to go for me. Hey brother, great job. I found out today that I have sleep apnea. Of course, I had the test 2 years ago and apparently someone forgot to notify me. Lol, I'm not kidding. I was originally told they would only contact me IF I had sleep apnea. Chronic fatigue brought me back to the doctor and a review of my test results. Great job explaining CPAP and keeping it entertaining. Thank you.

      For me chronic is actually really bad. I can "sleep" for 8 - 9 hours and get up in the morning, have coffee, and start to nod off within 2 hours if I am not physically moving around. My CPAP pick up has been delayed several times so I haven't received it yet. I had an eye surgery unexpectedly rescheduled that postponed the pick up and then they made a mistake in scheduling and I showed up on a date that only the secretary was in the office Then I had to fly out of state unexpectedly for a family matter. Hopefully, I will be getting it this week. I will let you know if/how much it improves my fatigue. I was an extremely active person that required very little sleep throughout my whole life, it is only the past few years that I am always tired.

I was always taught that during normal Negative pressure inspiration as the thorax expands and comes under negative pressure it actually "SUCKS" blood up into the Vena Cava from the abdomen increasing Pre-load and it is when we exhale creating positive pressure in the thorax it is then that the preload is pushed into the atria? When we intubate or use a BVM and initiate positive pressure ventilation you loose the "SUCK" and reduces the preload consequently lowering the blood pressure?
    Furthermore are there any objective studies not funded by the manufacturer of the ITD that prove its efficacy?

    • @jakobstensasen9776
      @jakobstensasen9776 Год назад

      I think the physiology in the video is wrong. He states that negative intrathoracic pressure increases the pressure on the vena cava and "drives" blood to the heart. What happens is that during inspiration the pressure in the right atrium drops and makes it easier for the heart to fill. A negative pressure in the thorax doesnt put pressure on the vena cava. What he illustrates with the syringe is the pressure from the atmosphere on the thoracic wall.

    I don't understand. You say that "normally", under negative pressure we're compressing the vena cava and providing blood flow to the heart (preload), but then the CPAP machine negates that effect and reduces compression on the vena cava, which reduces the blood flow to the heart. How is this a good thing? You say it's necessary for the heart to "play catch up". How is the heart falling behind and being overwhelmed? You don't explain why it's a good thing to negate the respiratory pump.

    My problem is breathing in. I'm overweight and due to my airway collapsing on the inhale I have an APAP. That said I did notice prior to getting my machine I was waking up with my heart beat in my neck like it was gonna explode and a migrain so fierce I was afraid I needed to go to the hospital. Because of modern technology and medicine I'm sleeping like a baby these days.

    Thanks Mark, I'm trying to help my mum adjust to a CPAP, this video has helped a lot but I am really concerned that she won't be able to breathe out, she can't keep sealing and unsealing the mask while she's sleeping + the intensity is set to increase which I assume will make it harder for her to exhale. Her issue is that she can't exhale, how is a CPAP going to help her?

    • @ianmartin6089
      @ianmartin6089 2 года назад +2

      I’m in the process of cpap trials as well. I can now fall asleep with the mask and keep it on throughout the whole night. Just waiting to feel the affects of it working. At first, I struggled with being able to exhale as well. What worked for me was adjusting the fit of the mask and just relaxing and thinking about things other than my breathing. It then came naturally.

    Are you a doctor? I have a question. I been using a cpap over a year now and my event number each night is 1.5 or less. Just had a night that it was 0.0. Am I getting these numbers because of the machine or am I getting those numbers myself naturally? I originally tested at moderate at 21 events an hour.
    I also had the surgery on my throat and nose this past March in hopes that it helps. Havnt restested yet because of covid.
