I have a theory as to what this song means judging mostly in the lyrics and maybe the video. I think it is about someone who is injured during a parade (as the video suggests.) The singer is the one who is injured. He see's the ambulance and the workers and envisions them as heroes and powerful leaders. In his delusioned state, he sees the electrical tools the nurse is using, whom he asks to be his wife. I think the window part is him trying to get some fresh air inside the ambulance. I don't really know. It's kind of vague, but that's just my interpretation of it. What a great song.

But, the messes are eventually so big that you need an AMBULANCE: this singer is willing to cut off their nose with your electric knife--to spite their face. They're so angry they'll open a window they can't open with their fist. All these things that are progressively larger disasters that are typical of abusive relationships. These men are in their 60's and are still killing it. I saw them when their "I Like Fun" album was released. I have listened to them for decades and am so impressed by how they continue to sound fresh and new with each release.
Damn, this is a good song! My own personal interpretation is that the story is being told in reverse: he actually starts by "getting this window open" and breaking his hand, musing about electric knives and cutting off his own face, and and then referring to himself as a dumbass in retrospect "why does this always seem to happen? Good thing I've got a napkin." In addition, I take the lyrics at face value: this is a guy who "cut off his own nose to spite his face", or overreacted to a basic problem. That problem? Getting locked out of his house or car. He breaks a window to try and get in, cuts or literally breaks his hand, and calls for help. While doing so, he muses on his overreaction, as if saying that if someone else, a "wife" or someone else, "handed him the electric knife", or tried to either make it easier or encourage his behavior, he'd feel less stupid. Finally, when inside the titular (if in reverse) ambulance for stitches, his adrenaline runs out and he finally goes "that was impulsive, and dumb, and I always get myself into trouble like this. But at least I brought this...napkin?" Probably a "Captain Obvious" musing, but that's how I interpret this song: a track about dumbassery and how it's easy to feel dumb after your mistakes.

1969 was my guess, too! That's actually Floral Avenue in Binghamton, NY, just adjacent to Johnson City. The fancy building in the background of some of the shots is the Boulevard Methodist Church on Floral and Grand, built in 1907.
