There was a guy at one of my previous jobs that had his arm effectively torn off because he didn’t unplug a meat grinder before trying to clean it, slipped, hit something and the grinder… did grinding.
You don’t mess around with heavy equipment. That supervisor is criminally liable as far as I’m concerned, dude may not have known the real danger, but that supervisor sure as hell did.
