I am in big trouble. Was diagnosed with Bullous PEMPHIGOID in 2017 via biopsy on my scalp. I was put on 60mgs Prednisone and tapered off over the next year or so. I am 60. Good health overall. NOW WHY am I in Trouble. I HAVE Pemphigus Vulgaris and have shredded skin for years. Necrosis sets in and they slide out. Blood blisters to clearish-red and Nicholskys sign (Sorry, terms I forget) and the other...MY DERMATOLOGIST won't even look. She says she cannot go internal. I understand. So before the Covid hit I had an appointment with My Oral Surgeon. Suffering with daily and monthly blisters popping, appointment FINALLY gets here...it had disappeared. He would not perform a biopsy without a visible reason. I BEGGED HIM TO...but ethically I completely understand.
THAT was 2018. I'm now in bad shape in my mouth. Daily blisters come and go and I'm in personal hell.
Thank you if anyone took time to listen. IM IN THE FIGHT OF MY LIFE...and look normal on the outside, Except for the Bullous Pemphigus I still have and has gotten worse in 2022. Reason for aforementioned October dermatologist appointment. THANK YOU and good-bye.
PS: I ABSOLUTELY LOVE THE EASY LEARNING yet clinically sound videos. Dave G

How can you remember that bullous pemphigoid affects a deeper layer of skin than pemphigus vulgaris?
In bullous pemphigoid, as the name suggests, a bulla (a fluid-filled blister) is formed. For fluid accumulation to occur, blood supply is needed. The superficial layers of the skin lack direct blood supply, which is why the term "bulla" is not used in pemphigus vulgaris. However, in bullous pemphigoid, the presence of a bulla in the name indicates that it affects a deeper layer of the skin, where blood supply is present.
Note: Bulla may also form in pemphigus vulgaris, but these bullae are fragile and small.
In pemphigus vulgaris, bulla formation can happen if the superficial layer of the skin is highly inflamed, as acute inflammation is linked to an increase in blood flow to the region. However, inflammation of the superficial layer also means that the covering of the bulla is weak, making these bullae fragile. Therefore, the term "bulla" is not used in the terminology of pemphigus vulgaris, as the fragility of the bulla means it may not always be observed. As the name suggests, a lot of bullae are seen in bullous pemphigoid, indicating that these bullae are not fragile. This is because, in bullous pemphigoid, the more superficial layer of the skin, which is basically the covering of bullae, is not inflamed. Additionally, bullae contain fluid, which means that blood supply is needed for their formation. Since bullous pemphigoid affects a deeper layer of the skin where blood is present, the bullae seen in bullous pemphigoid are more tense and larger than those seen in pemphigus vulgaris. Larger size and tense bullae means that they are very noticeable, which is why the term Bullous pemphigoid is made from the term "Bulla".
Nikolsky's sign is a skin finding that occurs when the skin's outermost layer is removed after being rubbed slightly. I remember it this way: in a positive Nikolsky's sign, skin nikal jaati hai-"nikal" sounds like "Nikolsky."(Hindi)
Since the covering of the bullae in pemphigus vulgaris is inflamed and fragile, it can be removed by gentle scraping. Therefore, Nikolsky's sign is positive in pemphigus vulgaris. Since bullous pemphigoid spares the more superficial layer of the skin, the topmost layer is not highly inflamed or fragile, so gentle scraping cannot remove it. Therefore, Nikolsky's sign is negative in bullous pemphigoid.
Pemphigus vulgaris is more dangerous than bullous pemphigoid because the fragile nature of the bullae means that the skin can break, introducing bacteria into the body, which can cause numerous complications. Furthermore, the rupture of bullae also means that a significant amount of fluid may be lost, leading to dehydration.
