    Sir if spasticity is very high in left hand than what to do, means patient not able to extend his hand on the couch...

    • @PhysioClassroom
      @PhysioClassroom  4 года назад +2

      That means it's grade 4 on MAS because below that passive movement is possible.
      Actually it is not the spasticity but the hypertonia induced permanent shortening in muscle fibres which appears to a physio as a very high spasticity.
      Botox can be an option for such patients after which the physio can use the NDT concept of treatment easily

    • @PhysioClassroom
      @PhysioClassroom  4 года назад +2

      Yes.
      Wait bearing Will help in gaining co-contraction of muscles around the weak shoulder, elbow and wrist joint thereby facilitating stability.
      Also the sensation associated with weight bearing will make the brain more aware of the paralyzed side upper limb and so more efferent discharges will be produced from the motor Cortex to the hemiplegic side UE.

      There are many strategies to improve gait and retrain DF.
      I would suggest you to read the posture and movement model and therapy concept of NDT.
      This will help you in choosing the right exercises for your patient.
      During initial gait training, DF can be achieved by usage of dynamic splints, FES etc. 🙏
